الإجراء يُنفَّذ من خلال الغشاء الشفّاف الذي يغطّي بياض العين، فلا تُفتح كرة العين ولا تُنزع من محجرها كما يتخيّل بعض المرضى، ولا تُستخدم فيه أشعة الليزر. وهو لا يمسّ القرنية ولا العدسة ولا الشبكية، ولذلك فهو لا يعالج قصر النظر ولا طول النظر ولا اللابؤرية؛ من يحتاج نظارة قبل العملية يحتاجها بعدها كذلك.

الترشيح للجراحة يُحدَّد بفحص عيون كامل يقيس مقدار الانحراف في اتجاهات النظر المختلفة ويقيّم حركة العضلات وحدّة البصر في كل عين ووجود كسل بصر أو رؤية مزدوجة. يُصحَّح الخطأ الانكساري بالنظارة أولاً ويُعالَج كسل البصر إن وُجد، لأن بعض الحالات تتحسّن بالنظارة وحدها ولا تحتاج جراحة أصلاً. الجراحة عند الأطفال تُجرى تحت تخدير عام، وعند البالغين قد تكون تحت تخدير موضعي أو عام.

حدود الإجراء يجب أن تكون واضحة قبل الموافقة عليه. \*\*الجراحة تُقوّم وضع العين ولا تُعالج بنفسها كسل البصر؛ العين الكسولة تبقى ضعيفة الرؤية بعد استقامتها إن لم يُعالَج الكسل في وقته.\*\* ولا تُوجد ضمانة لاستقامة تامة من عملية واحدة: \*\*قد يبقى انحراف صغير أو ينشأ انحراف بالاتجاه المعاكس، وقد تحتاج الحالة أكثر من تدخّل واحد للوصول إلى نتيجة مقبولة، وهذا احتمال متوقّع لا خطأ جراحي.\*\* الجراحة عند البالغ ليست تجميلية فقط؛ فهي قد تُخلّص من الرؤية المزدوجة وتوسّع مجال النظر المشترك.

## خطوات الإجراء

### 1. الفحص وقياس الانحراف

يقيس الطبيب زاوية الحول في اتجاهات النظر المختلفة وقرباً وبعداً، ويقيّم حركة كل عضلة وحدّة البصر في كل عين على حدة.

### 2. تصحيح النظارة وعلاج كسل البصر أولاً

يُوصف التصحيح البصري الكامل ويُعالَج كسل البصر بالتغطية أو القطرة إن وُجد، وتُعاد القياسات بعد استقرار الحالة قبل اتخاذ قرار الجراحة.

### 3. التخطيط الجراحي واختيار العضلات

يُحدّد الطبيب أي العضلات تُضعَف وأيها تُشدّ ومقدار التعديل بالمليمتر، وهل تُجرى الجراحة على عين واحدة أو على العينين.

### 4. الجراحة نفسها

يُفتح الغشاء الشفّاف على بياض العين وتُفصل العضلة المستهدفة وتُعاد خياطتها في الموضع المخطّط، ثم يُغلق الغشاء بخيوط تذوب عادة.

### 5. المتابعة وتقييم النتيجة

تُتابع العين خلال الأسابيع الأولى ثم على فترات أطول لقياس الانحراف المتبقّي وتقييم الرؤية المشتركة، وتقرير الحاجة إلى تدخّل إضافي.

## Procedure steps

### 1. Examination and measuring the deviation

The ophthalmologist measures the angle of deviation in different gaze directions and at near and distance, and assesses each muscle's movement and the vision in each eye separately.

### 2. Glasses and amblyopia treatment first

Full optical correction is prescribed and any amblyopia is treated with patching or drops, then measurements are repeated once the situation is stable before any decision to operate.

### 3. Surgical planning and muscle selection

The surgeon decides which muscles to weaken and which to strengthen, the amount of adjustment in millimetres, and whether one eye or both eyes are operated on.

### 4. The operation itself

The transparent membrane over the white of the eye is opened, the target muscle is detached and re-sutured at the planned position, and the membrane is closed with sutures that usually dissolve.

### 5. Follow-up and assessing the result

The eye is reviewed over the first weeks and then at longer intervals to measure any residual deviation, assess binocular vision and decide whether further intervention is needed.

## التحضير قبل الإجراء

أخبر الطبيب بكل جراحة سابقة على عينيك، وخصوصاً أي عملية حول سابقة، لأن الالتصاقات تغيّر التخطيط تماماً. اذكر كل دواء تتناوله ومنه مسيّلات الدم والأسبرين ومكمّلات الأعشاب، ولا توقف أي دواء من تلقاء نفسك بل اسأل عن ذلك. أخبره عن أمراض القلب أو السكري أو الغدة الدرقية أو الأمراض العصبية أو العضلية، وعن أي مشكلة سابقة مع التخدير في العائلة. التزم بمدة الصيام التي يحدّدها طبيب التخدير قبل العملية. أحضر نظارتك الحالية ووصفتها وتقارير الفحوص السابقة. رتّب من يقود بك ومن يرافق الطفل بعد العملية. وقبل التوقيع، اسأل صراحة عن الانحراف المتبقّي المحتمل واحتمال الحاجة إلى تدخّل ثانٍ لاحقاً.

## العناية بعد الإجراء

توقّع احمراراً واضحاً في بياض العين وإحساساً بجسم غريب ودمعاً وحكة خلال الأيام الأولى، وقد يستمرّ الاحمرار أسابيع حتى يزول تدريجياً. استخدم القطرات الموصوفة بالمواعيد المحدّدة، ولا تفرك العين ولا تضغط عليها، وتجنّب الماء والصابون داخل العين والسباحة والغبار الكثيف حتى يسمح الطبيب. عند الأطفال قصّ الأظافر وراقب الطفل حتى لا يفرك عينه. عُد إلى النشاط الخفيف تدريجياً وتجنّب الرياضات العنيفة ورفع الأثقال في الفترة التي يحدّدها الطبيب. الرؤية المزدوجة قد تظهر مؤقّتاً في الأيام الأولى وتتحسّن غالباً، فأبلغ طبيبك بها. \*\*راجع فوراً عند ألم شديد متزايد أو تراجع واضح في الرؤية أو إفرازات صفراء أو خضراء أو تورّم متزايد في الجفن أو حرارة أو رؤية مزدوجة تسوء بدل أن تتحسّن.\*\* ولا تفوّت مواعيد المتابعة حتى إن بدت العين مستقيمة.

## About this service

Strabismus correction is a surgical procedure in which the surgeon adjusts the tension of the muscles that move the eyeball so the visual axes of both eyes become as parallel as possible. One muscle is tightened or another weakened by changing where it attaches to the wall of the eye, which alters the force pulling the eye in a given direction.

The operation is carried out through the transparent membrane covering the white of the eye. The eyeball itself is not opened and it is not removed from its socket, as many patients imagine, and no laser is involved. The cornea, the lens and the retina are not touched, which is why the surgery does not treat short-sightedness, long-sightedness or astigmatism; a patient who needed glasses before the operation will still need them afterwards.

Suitability is decided by a full eye examination that measures the deviation in different directions of gaze and assesses muscle movement, visual acuity in each eye, and the presence of amblyopia or double vision. Any refractive error is corrected with glasses first and amblyopia is treated if present, because some deviations improve with glasses alone and need no surgery at all. In children the procedure is performed under general anaesthesia; in adults it may be done under local or general anaesthesia.

The limits must be clear before consent is given. \*\*Surgery straightens the position of the eye but does not by itself treat a lazy eye; an amblyopic eye will still see poorly after it has been aligned if the amblyopia was not treated in time.\*\* Nor is perfect alignment from a single operation guaranteed. \*\*A small residual deviation may remain or a deviation may appear in the opposite direction, and more than one procedure is sometimes needed to reach an acceptable result. This is an expected possibility, not a surgical error.\*\* In adults the operation is not purely cosmetic: it can relieve double vision and widen the field of single binocular vision.

## Preparation

Tell your surgeon about every previous eye operation, and in particular any earlier strabismus surgery, because scar tissue changes the plan completely. List all medicines you take, including blood thinners, aspirin and herbal supplements, and do not stop any of them on your own initiative but ask first. Mention heart disease, diabetes, thyroid disorders and any neurological or muscular condition, along with any family history of problems under anaesthesia. Follow the fasting period set by the anaesthetist. Bring your current glasses, their prescription and reports from previous examinations. Arrange someone to drive you home, or to accompany a child afterwards. Before signing consent, ask openly about the residual deviation that may remain and about the possibility of needing a second procedure later.

## Aftercare

Expect marked redness of the white of the eye, a foreign body sensation, watering and itching during the first days, and redness that may take weeks to fade completely. Use the prescribed drops exactly on schedule, do not rub or press on the eye, and keep water, soap, swimming pools and heavy dust away from it until your surgeon allows. With children, trim the nails and watch the child so the eye is not rubbed. Return to light activity gradually and avoid contact sports and heavy lifting for the period your surgeon specifies. Temporary double vision can appear in the early days and usually improves, but report it. \*\*Seek care immediately for severe or increasing pain, a clear drop in vision, yellow or green discharge, increasing eyelid swelling, fever, or double vision that is getting worse rather than better.\*\* Never miss follow-up appointments, even if the eye looks straight.
