الترشيح لا تحدّده درجة النظارة وحدها ولا يُقرَّر من قراءة مقال، بل يحتاج فحصاً سريرياً كاملاً يشمل خرائط طبوغرافية وتوموغرافية للقرنية، وقياس سماكتها، وفحص انكسار بعد توسيع الحدقة، وتقييم الدموع والجفاف، وفحص قاع العين. ويُشترط عادة أن تكون الدرجة مستقرّة نحو سنة على الأقل، وأن تكون سماكة القرنية وشكلها كافيين، وأن تخلو العين من القرنية المخروطية أو التهاب نشط أو جفاف شديد غير مضبوط. أما الحمل والرضاعة فيؤجّلان القرار عادة لأن الدرجة قد تتغيّر مؤقتاً.

ومن المهمّ معرفة ما لا يفعله الليزر: هو لا يمنع ضعف النظر القريب الذي يبدأ عادة بعد الأربعين، فقد تحتاج نظارة قراءة لاحقاً حتى لو نجح تصحيح البعد؛ ولا يعالج أمراض الشبكية ولا الجلوكوما ولا الساد؛ ولا يوقف تقدّم قصر النظر عند من لم تستقرّ درجته؛ ولا يُلغي حاجة شديدي قصر النظر إلى فحص شبكية دوري، لأن خطر تمزّق الشبكية مرتبط بطول العين لا بالنظارة؛ ولا يضمن في كل حالة تخلّصاً تامّاً من النظارة، فقد تبقى درجة بسيطة أو تعود جزئياً مع الزمن.

الأعراض المتوقّعة في الأسابيع الأولى جفاف وحرقة وحساسية للضوء وهالات حول الأضواء ليلاً، وتتراجع تدريجياً عند معظم الناس. والتعافي البصري أسرع في تقنيات الشريحة والشقّ الصغير، وأبطأ وأكثر إزعاجاً في تقنيات السطح لأن سطح القرنية يحتاج أياماً ليُغلق. أما المضاعفات غير الشائعة فتشمل نقص التصحيح أو زيادته، وعدم انتظام سطح القرنية، ومشاكل الشريحة عند تلقّي ضربة على العين — ولذلك تُتابع العين بمواعيد محدّدة، ولا يُترك ألم شديد أو تدهور رؤية بلا مراجعة.

## خطوات الإجراء

### 1. تقييم كامل قبل القرار

قياس درجة النظر مرّتين على الأقل (إحداهما بعد توسيع الحدقة)، وخرائط القرنية وسماكتها، وقياس الحدقة والدموع، وفحص الضغط وقاع العين. من هذه القياسات تُحدَّد التقنية المناسبة أو يُستبعد الليزر ويُطرح بديل مثل زرع عدسة داخل العين.

### 2. التحضير يوم الجلسة

تُقطَّر مادة مخدّرة موضعية في العين — لا حاجة عادة إلى تخدير عام أو إبر — وتُنظَّف منطقة الجفون ويُثبَّت الجفن بلطف بمُبعِد، ويُطلب منك تثبيت النظر على ضوء صغير. قد يُعطى مسكّن خفيف للتوتّر بقرار الطبيب.

### 3. تجهيز سطح القرنية

يختلف هذا الجزء بحسب التقنية: إزالة الطبقة السطحية في تقنيات السطح، أو عمل شريحة رقيقة ورفعها في تقنية الشريحة، أو تشكيل عدسة نسيجية دقيقة داخل القرنية واستخلاصها من شقّ صغير. هذه الخطوة هي ما يفرّق بين الأسماء التجارية المختلفة للإجراء.

### 4. إعادة تشكيل القرنية بالليزر

يعمل الليزر البارد وفق خطّة محسوبة على قياساتك فيُزيل كمّيات مجهرية من النسيج لتصحيح قصر النظر أو طوله أو الانحراف. الوقت الفعلي للليزر ثوانٍ إلى نحو دقيقة لكل عين، ويتوقّف تلقائياً إن فقد تتبّع العين.

### 5. الإغلاق والفحص والمتابعة

تُعاد الشريحة إلى موضعها أو تُوضع عدسة لاصقة ضمادية في تقنيات السطح، ثم يُفحص السطح بالمجهر وتُعطى خطّة القطرات. تُحدَّد مراجعة في اليوم التالي ثم بعد أسبوع وشهر وثلاثة أشهر عادة لمتابعة الرؤية والجفاف واستقرار القرنية.

## Procedure steps

### 1. Full pre-operative assessment

Refraction is measured at least twice (once after dilation), together with corneal maps and thickness, pupil size, tear film, intraocular pressure and a dilated retinal examination. These measurements decide which technique fits, or rule the laser out and open the discussion of an alternative such as an implantable lens.

### 2. Preparation on the day

Anaesthetic drops are instilled; general anaesthesia and injections are not usually needed. The lid area is cleaned, the lids are gently held with a speculum, and you are asked to fixate on a small target light. A mild sedative may be given at the surgeon's discretion.

### 3. Preparing the corneal surface

This step is what actually differs between the branded names: removal of the epithelium in surface techniques, creation and lifting of a thin flap in flap techniques, or shaping a fine intrastromal lenticule and removing it through a small incision.

### 4. Laser reshaping

The excimer laser follows a plan computed from your own measurements, removing microscopic amounts of tissue to correct myopia, hyperopia or astigmatism. Actual laser time is seconds to about a minute per eye, and tracking systems pause the treatment if the eye moves out of range.

### 5. Closure, check and follow-up

The flap is repositioned, or a bandage contact lens is placed after surface treatments. The surface is checked at the microscope, a drop schedule is issued, and visits are typically arranged for the next day, then at about one week, one month and three months to follow vision, dryness and corneal stability.

أوقف العدسات اللاصقة قبل الفحص وقبل العملية بالمدّة التي يحدّدها طبيبك — اللينة عادة عدّة أيام، والصلبة أو النفّاذة للغازات عدّة أسابيع — لأن العدسة تغيّر شكل القرنية وتُفسد القياسات التي تُبنى عليها خطّة الليزر. أخبر الطبيب بكل أدويتك، وبأي جفاف عين أو حساسية أو التهاب متكرّر في الجفن، وبأمراضك المزمنة وأمراض المناعة، وبأي عملية أو إصابة سابقة في العين. لا تضع مكياج عيون ولا كريمات ولا عطراً على الوجه يوم الجلسة، واغسل وجهك ورموشك جيداً. تناول طعاماً خفيفاً واحضر بمرافق يقود بك إلى البيت، فلن تستطيع القيادة بعد الجلسة. وإن كنتِ حاملاً أو مرضعاً فأخبري الطبيب قبل تحديد الموعد.

## العناية بعد الإجراء

استخدم القطرات بالمواعيد المكتوبة ولا تنقص منها لأنك «تحسّنت»، ولا تفرك عينك إطلاقاً في الأسابيع الأولى — خاصّة بعد تقنية الشريحة. نم على ظهرك أوّل ليلتين، والبس نظارة شمسية في الخارج، وتوقّع حرقة ودموعاً وتشوّشاً وحساسية للضوء لأيام (أطول في تقنيات السطح). امتنع عن السباحة والساونا وحمّامات البخار نحو أسبوعين، وعن مكياج العيون نحو أسبوع، وعن الرياضات التي فيها تلامس أو خطر ضربة على العين للمدّة التي يحدّدها طبيبك. قلّل ساعات الشاشة في الأيام الأولى وأكثِر من القطرات المرطّبة. \*\*راجع طبيبك فوراً عند ألم شديد متزايد لا تخفّفه المسكّنات، أو تدهور مفاجئ في الرؤية، أو احمرار شديد مع إفراز أو صديد، أو تورّم الجفن مع حرارة، أو إن تلقّيت ضربة على العين أو أحسست أن شيئاً «تحرّك» داخلها.\*\*

## About this service

Laser vision correction reshapes the front surface of the cornea by a precisely calculated amount so that light focuses on the retina and dependence on glasses or contact lenses is reduced. Several techniques share this name: surface treatments, in which the thin outer layer is removed before the laser reshapes the tissue beneath it; flap-based treatments, in which a thin corneal layer is lifted, treated and repositioned; and small-incision treatments, in which a fine lenticule of tissue is created and extracted without lifting a flap.

Candidacy is never decided by a prescription number alone, and never from reading an article. It requires a full clinical examination: corneal topography and tomography, corneal thickness measurement, refraction after pupil dilation, tear film and dry-eye assessment, and a dilated retinal examination. Doctors usually expect the prescription to have been stable for about a year, the cornea to be thick and regular enough, and the eye to be free of keratoconus, active inflammation or severe uncontrolled dryness. Pregnancy and breastfeeding usually postpone the decision, because refraction can shift temporarily.

It is equally important to know what the laser does not do. It does not prevent the near-vision decline that typically begins after the age of forty, so reading glasses may still be needed later even if distance vision is corrected. It does not treat retinal disease, glaucoma or cataract, and it does not slow progression in an eye whose prescription is still changing. Highly myopic patients still need periodic retinal checks, because retinal risk follows the length of the eye rather than the glasses. And no technique can promise complete freedom from glasses in every case: a small residual prescription may remain, or partial regression may appear over the years.

Expected symptoms in the first weeks include dryness, burning, light sensitivity and halos around lights at night, which fade gradually in most people. Visual recovery is faster after flap and small-incision techniques and slower and more uncomfortable after surface techniques, because the corneal surface needs several days to heal. Less common complications include under- or over-correction, an irregular corneal surface, and flap problems after a direct blow to the eye. This is why scheduled follow-up matters, and why severe pain or falling vision must never be left unchecked.

## Preparation

Stop wearing contact lenses for the period your doctor specifies before both the assessment and the procedure — usually several days for soft lenses and several weeks for rigid or gas-permeable lenses — because lenses change the corneal shape and distort the measurements the laser plan is built on. Tell your doctor about all your medications, any dry eye, allergy or recurrent lid inflammation, chronic and autoimmune conditions, and any previous eye surgery or injury. Do not wear eye make-up, face cream or perfume on the day, and wash your face and lashes well. Eat a light meal and bring someone to drive you home, as you will not be able to drive afterwards. If you are pregnant or breastfeeding, say so before the appointment is booked.

## Aftercare

Use your drops exactly as scheduled, and do not cut them short because you feel better. Do not rub the eye at all in the first weeks, especially after a flap technique. Sleep on your back for the first two nights, wear sunglasses outdoors, and expect burning, watering, blurring and light sensitivity for some days — longer after surface techniques. Avoid swimming, saunas and steam rooms for about two weeks, eye make-up for about a week, and contact sports or any activity risking a blow to the eye for as long as your surgeon advises. Limit screen time in the first days and use lubricating drops generously. \*\*Seek care immediately if you develop severe or increasing pain not relieved by painkillers, a sudden drop in vision, marked redness with discharge or pus, lid swelling with fever, or if the eye is struck or you feel that something has shifted inside it.\*\*
