## Procedure steps

### 1. Reviewing the previous surgical history

The earlier operative report is obtained to identify which muscles were adjusted and by how much, since planning a revision depends on which muscles remain untouched.

### 2. Prism measurement in all gaze directions

The angle is measured with prisms at near and distance and in different directions of gaze, to define the pattern of the residual deviation and whether it is constant or variable.

### 3. Testing for restriction and adhesions

Eye movement is examined for mechanical restriction from fibrosis and the degree of resistance is judged, because scarring changes the surgical approach and the expected effect.

### 4. Non-surgical and staged options

Prisms in glasses for double vision and temporary muscle injection are discussed, and staged surgery may be chosen instead of one large intervention.

### 5. Surgery and close follow-up

Adhesions are released and the planned muscles are adjusted, sometimes using adjustable sutures, with closer than usual follow-up to judge how stable the result is.

## About this service

Revision surgery for failed strabismus operations is a repeat procedure carried out on a patient who has already had squint surgery without reaching an acceptable eye position. The situation takes three forms: a deviation remaining in the original direction, a deviation that has flipped to the opposite direction after overcorrection, or a new deviation appearing in another direction over time.

\*\*Needing a second operation is usually not evidence of a surgical mistake.\*\* Strabismus is not a fixed geometric measurement. Muscle response to adjustment varies from person to person, muscle strength can change over the years, and a squint may be accompanied by weak binocular vision that lets the eye drift again. Repeat surgery is therefore a recognised part of the treatment pathway in some cases rather than a strange exception to it.

Assessment before revision is more detailed and takes longer than the first assessment. The previous operative report is requested to establish which muscles were operated on and by how much; the angle of deviation is measured with prisms in all directions of gaze; eye movement is examined for restriction caused by adhesions; and double vision and the brain's ability to fuse the two images are evaluated. Prisms incorporated into glasses may be offered as a non-surgical answer to double vision, and a temporary muscle injection may be used to estimate the likely outcome or to address a small deviation. Suitability for revision and its timing are decided by clinical examination and measurement of the deviation, not by reading about it and not by the patient's wish alone.

The limits of a second operation must be stated with complete clarity. Scarred tissue from the first surgery makes the procedure harder and its outcome less predictable. \*\*The aim is to improve alignment and reduce double vision, not to promise perfect straightness, and some cases need more than one intervention or a staged approach.\*\* This surgery also does not correct long-standing amblyopia and cannot raise the acuity of a weak eye, because that belongs to childhood treatment and not to the muscles.

## Preparation

Bring every document you have from the previous surgery: the operative report, the dates, the name of the procedure, and any measurements of the deviation taken before and after it. If the paperwork is not available, say so plainly rather than guessing, because guessing how many muscles were operated on is dangerous. Tell the surgeon about any other eye operation, about thyroid disease, myasthenia, neurological conditions or diabetes, and about every medicine you take including blood thinners. Describe precisely when the current deviation appeared, whether it varies through the day or with tiredness, whether you see double and in which direction, and whether you tilt your head automatically to see better. Above all, ask directly what a realistic goal for this operation is and how likely a further stage will be needed.

## Aftercare

Expect more redness and swelling than after the first operation, because scarred tissue reacts more, and redness may persist for weeks. Use the prescribed drops exactly on schedule and do not stop them because the eye feels better. Do not rub or press on the eye, and avoid swimming, dust and water in the eye until your surgeon allows it. Double vision may appear or shift in the early days before settling, and a temporary prism may be added to your glasses to help. Attend every one of the close follow-up appointments, because early measurement of the deviation is what determines whether further adjustment is needed. \*\*Seek care immediately for severe or increasing pain, a clear drop in vision, heavy coloured discharge, increasing eyelid swelling, fever, or double vision that worsens day by day.\*\* Remember that the stability of the result is judged after weeks, not in the mirror on the first day.
