صار ERCP اليوم إجراءً علاجياً بالأساس لا تشخيصياً؛ فالتشخيص المحض تكفّل به الرنين المغناطيسي للقنوات والسونار عن طريق المنظار، ويُحفَظ ERCP لمن يحتاج عملاً داخل القناة فعلاً. أشهر دواعيه حصوة عالقة في القناة الصفراوية مع اصفرار أو التهاب قنوات صاعد، وتضيّق يحتاج توسيعاً أو دعامة، وتسريب صفراء بعد جراحة المرارة، وأخذ عيّنة أو تفريش من تضيّق مشتبه به.

وحدوده يجب أن تُقال بصراحة: ERCP لا يستأصل المرارة ولا يمنع تكوّن حصوات جديدة، فسحب حصوة من القناة الصفراوية لا يُغني عن استئصال المرارة لاحقاً إن كانت هي مصدر الحصى. وهو ليس فحصاً لألم بطن غير مفسَّر ولا لعسر الهضم، ولا يعالج التهاب البنكرياس المزمن، ولا يُطلب لمجرد ارتفاع بسيط في وظائف الكبد بلا دليل على انسداد.

هذا الإجراء أعلى من التنظير المعتاد في احتمال المضاعفات، وأشهرها التهاب البنكرياس بعد الإجراء، ثم النزيف بعد شقّ الحلمة، والالتهاب، والانثقاب. هذه أحداث نادرة لكنها حقيقية، ولها إجراءات وقاية يتّبعها الفريق أثناء الجلسة وبعدها. ولهذا يوازن الطبيب دائماً بين الفائدة المتوقّعة والخطر، ويبقى الترشيح محسوماً بالفحص السريري والتصوير والتحاليل لا بقراءة مقال.

يُجرى ERCP تحت تهدئة عميقة أو تخدير عام، فيلزمك مرافق ويُمنع عليك قيادة السيارة بعده، وقد يقتضي مبيتاً للمراقبة في حالات مختارة. وإن رُكّبت لك دعامة فاعرف نوعها وموعد سحبها أو تبديلها، فإهمال هذا الموعد سبب معروف لمشاكل لاحقة.

## خطوات الإجراء

### 1. التقييم والتصوير قبل الإجراء

تُراجَع الأعراض ووظائف الكبد وصورة الدم والتخثّر، ويُستكمل التصوير بالسونار أو الرنين للقنوات، ليتأكّد الفريق أن هناك فعلاً ما يستوجب عملاً داخل القناة لا تشخيصاً يكفيه فحص أقل خطراً.

### 2. التخدير والاستلقاء والمراقبة

يُوضع كانيولا وريدية وتبدأ تهدئة عميقة أو تخدير عام مع مراقبة النبض والأكسجين والضغط، وتُستلقى على طاولة أشعة بوضعية تسمح بتصوير القنوات أثناء العمل.

### 3. الوصول إلى الحلمة والتقنيط والصبغة

يُوجَّه المنظار إلى الجزء الأول من الأمعاء الدقيقة وتُحدَّد حلمة فاتر، ثم تُقنَّط بسلك دقيق وقنية وتُحقن صبغة، فتظهر القناة الصفراوية أو قناة البنكرياس على الأشعة ويتبيّن موضع الانسداد أو التضيّق.

### 4. العمل العلاجي داخل القناة

حسب ما يظهر: شقّ صغير في الحلمة لتوسيع المخرج، وسحب الحصوة بسلة أو بالون، وتفتيتها إن كانت كبيرة، وتوسيع التضيّق، وتركيب دعامة لتصريف الصفراء، وأخذ خزعة أو تفريش من موضع مشتبه به.

### 5. المراقبة بعد الإجراء وخطة ما بعده

تُراقب لساعات بعد الإفاقة لرصد الألم أو الحرارة أو النزيف، وقد تُطلب تحاليل أو مبيت. ثم يُوضع ما بعد الإجراء: موعد استئصال المرارة إن لزم، وموعد سحب الدعامة أو تبديلها، ونتيجة العيّنة.

## Procedure steps

### 1. Pre-procedure assessment and imaging

Symptoms, liver tests, blood count and clotting are reviewed and imaging with ultrasound or MR cholangiography is completed, to confirm that work inside the duct is genuinely needed rather than a diagnosis a lower-risk test could provide.

### 2. Anaesthesia, positioning and monitoring

An intravenous line is placed and deep sedation or general anaesthesia is started with monitoring of pulse, oxygen and blood pressure. You lie on a fluoroscopy table in a position that allows the ducts to be imaged during the work.

### 3. Reaching the ampulla, cannulation and contrast

The endoscope is advanced to the first part of the small bowel and the ampulla is identified, then cannulated with a fine guidewire and catheter. Contrast is injected so the bile or pancreatic duct is outlined on X-ray and the site of obstruction or narrowing becomes clear.

### 4. Therapeutic work inside the duct

Depending on the findings: a small cut in the ampulla to widen the outlet, stone extraction with a basket or balloon, fragmentation of large stones, dilation of a stricture, placement of a stent to drain bile, and biopsy or brushing of a suspicious area.

### 5. Recovery and the plan afterwards

You are observed for several hours after waking to detect pain, fever or bleeding, and blood tests or an overnight stay may be required. The follow-on plan is then set: gallbladder surgery if needed, the date for stent removal or exchange, and the biopsy result.

## التحضير قبل الإجراء

الصيام عن الطعام والسوائل قبل الموعد إلزامي لأن الإجراء يُجرى تحت تهدئة عميقة أو تخدير عام؛ التزم بالتوقيت الذي يُعطى لك بلا اجتهاد شخصي. أخبر الطبيب بكل أدويتك، وبالأخص مسيّلات الدم ومضادات التخثّر وأدوية السكري، فبعضها يحتاج إيقافاً أو تعديلاً قبل الإجراء وقد يُطلب تحليل تخثّر. أخبره أيضاً إن كان لديك حساسية لصبغة الأشعة أو لدواء أو للاتكس، أو أمراض قلب أو رئة، أو صمام قلبي، أو ناظم خطوة، أو إن كنتِ حاملاً — فالتصوير بالأشعة جزء من الإجراء. اذكر أي جراحة سابقة في المعدة أو المرارة أو القنوات، فذلك يغيّر مسار الوصول. أزل الأسنان المتحركة والحلي قبل الدخول، وارتدِ ملابس مريحة. ورتّب مرافقاً بالغاً يعيدك إلى البيت، واستعد لاحتمال المبيت للمراقبة إذا اقتضت حالتك ذلك.

## العناية بعد الإجراء

توقّع بعد الإفاقة نعاساً وانتفاخاً وألماً خفيفاً في الحلق، ويبدأ الطعام والشراب تدريجياً بإذن الفريق لا قبله. \*\*راجع الطوارئ فوراً عند ألم شديد أعلى البطن ينفذ إلى الظهر مع قيء — وهو أهم علامة على التهاب البنكرياس بعد الإجراء — أو حرارة وقشعريرة أو اصفرار متزايد في العين والجلد، أو تقيّؤ دم أو براز أسود قطراني، أو ألم صدر وضيق نفس، أو انتفاخ وتصلّب في البطن\*\*. هذه علامات المضاعفات النادرة: التهاب البنكرياس والالتهاب الصاعد والنزيف والانثقاب، وكلها تُدار أفضل عند التقاطها مبكراً. لا تقود السيارة ولا تشرب كحولاً ولا توقّع أوراقاً مهمة في يوم التهدئة، ولا تبقَ وحدك في تلك الليلة. واستأنف مسيّلات الدم في الموعد الذي يحدّده الطبيب. وإن رُكّبت لك دعامة فسجّل موعد سحبها أو تبديلها والتزم به، ولا تؤجّل استئصال المرارة إن كان موصى به.

## About this service

Endoscopic retrograde cholangiopancreatography, or ERCP, combines endoscopy and X-ray imaging in a single session. A specialised side-viewing endoscope is passed through the mouth, oesophagus and stomach into the first part of the small intestine, where the bile duct and pancreatic duct open through a small orifice called the ampulla of Vater. The opening is cannulated with a fine wire and catheter, contrast is injected, the ducts are imaged with X-rays, and the problem is then treated from within during the same session.

ERCP today is primarily therapeutic rather than diagnostic. Purely diagnostic imaging of the ducts is now done with magnetic resonance cholangiography or endoscopic ultrasound, and ERCP is reserved for patients who genuinely need work inside the duct. The commonest indications are a stone impacted in the bile duct with jaundice or ascending cholangitis, a stricture requiring dilation or a stent, a bile leak after gallbladder surgery, and sampling or brushing of a suspicious narrowing.

Its limits must be stated plainly. ERCP does not remove the gallbladder and does not prevent new stones from forming, so clearing a stone from the duct does not remove the need for later gallbladder surgery if the gallbladder is the source. It is not an investigation for unexplained abdominal pain or indigestion, it does not treat chronic pancreatitis, and it should not be requested for a mildly abnormal liver profile with no evidence of obstruction.

This procedure carries a higher risk of complications than routine endoscopy. The best known is post-procedure pancreatitis, followed by bleeding after sphincterotomy, infection and perforation. These events are uncommon but real, and the team follows specific preventive measures during and after the session. For that reason the expected benefit is always weighed against the risk, and suitability is settled by clinical assessment, imaging and laboratory results rather than by reading about the technique.

ERCP is performed under deep sedation or general anaesthesia, so you need an escort and must not drive afterwards, and selected cases require an overnight stay for observation. If a stent is placed, know its type and the date it must be removed or exchanged, because missing that appointment is a well-recognised cause of later problems.

## Preparation

Fasting from food and fluids before the appointment is mandatory, because the procedure is done under deep sedation or general anaesthesia; follow the timing you are given exactly. Tell your doctor about all your medications, especially blood thinners, anticoagulants and diabetes drugs, as some must be stopped or adjusted beforehand and clotting tests may be requested. Also report allergy to X-ray contrast, medicines or latex, any heart or lung disease, a heart valve, a pacemaker, or pregnancy, since fluoroscopy is part of the procedure. Mention previous surgery on the stomach, gallbladder or bile ducts, as this changes the route of access. Remove dentures and jewellery before entering the room and wear comfortable clothing. Arrange an adult escort to take you home, and be prepared for a possible overnight stay for observation if your condition requires it.

## Aftercare

After waking expect drowsiness, bloating and a mildly sore throat; food and drink are restarted gradually only with the team's approval. Seek emergency care immediately for severe upper abdominal pain radiating to the back with vomiting, the key sign of post-procedure pancreatitis, or for fever and chills, deepening yellowing of the eyes and skin, vomiting blood or black tarry stools, chest pain and breathlessness, or a distended, rigid abdomen. These indicate the rare complications of pancreatitis, ascending infection, bleeding and perforation, all of which are managed best when caught early. Do not drive, drink alcohol or sign important documents on the day of sedation, and do not spend that night alone. Restart blood thinners on the date your doctor specifies. If a stent was placed, record the date for removal or exchange and keep it, and do not postpone gallbladder surgery if it has been recommended.
