## Procedure steps

### 1. Referral and safety screening

Your doctor defines the region and the clinical question to be answered, then a detailed safety questionnaire covers implants, devices and metal. This form is not a formality but an essential safety condition before entry.

### 2. Preparation and metal removal

All metal, jewellery, watches, cards and hearing aids are removed and left outside the room. Fasting for a few hours is requested only for abdominal and biliary studies, and kidney function is checked and a cannula placed if contrast is planned.

### 3. Positioning in the scanner

You lie on a table that moves into the scanner tunnel, a receiver coil is placed over the region being imaged, and you are given ear protection and a hand bell. The radiographer sees and hears you throughout the scan.

### 4. Image sequences and contrast if needed

Several sequences are acquired, each lasting a few minutes and producing loud repetitive noise. You are asked to stay completely still, since movement spoils a sequence and forces a repeat. Contrast may be injected midway, after which some sequences are repeated.

### 5. Reporting and clinical correlation

A radiologist reviews the sequences and issues a detailed report, which returns with the images to your referring doctor to be matched against your examination and symptoms — a finding that does not explain your symptoms is not treated for its own sake.

## About this service

Magnetic resonance imaging uses a strong magnetic field and radio waves — not ionising radiation — to produce highly detailed cross-sectional images of soft tissue. It is usually chosen when the problem lies in a tissue that X-rays cannot show, or when finer detail is needed than a CT scan can provide.

MRI is strongest for the brain, spinal cord and nerves, the intervertebral discs, cartilage, ligaments, tendons and muscle, bone marrow, the abdominal and pelvic organs and bile ducts, the uterus and ovaries, and in selected cases the heart and breast. An intravenous gadolinium-based contrast agent may be used to highlight inflammation, tumours or vessels; this differs from the iodine-based contrast used in CT.

Its limits should be stated plainly. MRI does not depict cortical bone and calcification as well as CT, it is not the appropriate test in acute trauma or acute bleeding or for imaging the lungs and air, and it is not the fastest emergency investigation. It does not measure function: it does not measure hearing, muscle power or nerve conduction — those require other tests such as nerve conduction studies. Most importantly, \*\*an MRI finding is not automatically the cause of your pain\*\*: disc bulges, degenerative change and silent tendon tears are found in a large proportion of people with no symptoms at all, so a surgical decision is never based on an image alone but on the image matching the neurological examination and the symptoms. MRI alone also cannot prove that a mass is cancer — a biopsy is what settles that.

Safety is the core of preparation, because the magnetic field is always active, even between scans. You will complete a safety questionnaire asking about a pacemaker or defibrillator, cochlear implants, older aneurysm clips, nerve stimulators, insulin pumps, valves, stents and screws, and any metallic fragment — especially in the eye — or a history of welding work. Some modern devices are classified as conditionally safe and the exact model must be verified rather than assumed. All metal is left outside the scan room. The scanner is loud and ear protection is provided, and people who feel enclosed in the tunnel should discuss this with the centre in advance. In pregnancy, non-urgent MRI is deferred, particularly in the early months, gadolinium is avoided unless necessary, and kidney function is assessed before contrast.

Finally, MRI is a tool read alongside clinical examination and history; it does not diagnose on its own. The decision to perform it, and which sequences are required, belongs to your referring doctor rather than to a web page.

## Preparation

Tell the centre in advance about any implanted device or metal in your body: a pacemaker, defibrillator, cochlear implant, nerve stimulator, insulin pump, aneurysm clips, heart valve, screws or plates, or a metallic fragment — particularly in the eye — as well as any work in welding or metalwork, and bring the device card if you have one so the exact model can be verified. Report pregnancy, possible pregnancy or breastfeeding, any drug allergy or previous contrast reaction, and kidney disease, since gadolinium requires assessment of kidney function. Fasting is usually unnecessary except for abdominal, biliary and bowel studies, for which the centre will state the duration. If you feel enclosed in small spaces, cannot stay still, or have pain that prevents lying flat, say so before the appointment — solutions exist, including pre-arranged sedation. Bring earlier imaging for comparison, wear clothing without metal buttons or zips, and expect a longer examination than a plain X-ray.

## Aftercare

An MRI without contrast leaves no after-effects: you return to normal activity, eating and medication immediately, and no magnetic field or radiation remains in your body. If contrast was used, drink fluids as the centre advises, watch the cannula site, and tell your doctor about any delayed allergic symptoms; breastfeeding can usually continue after gadolinium according to your doctor's advice. If you were sedated, do not drive, operate machinery or sign important documents that day, and have someone accompany you home. Keep the images and report on digital media and take them to your referring doctor, and do not interpret the report yourself: phrases such as 'disc bulge', 'degenerative change' or 'non-specific signal' are common in people without symptoms and do not in themselves mean surgery is needed. \*\*Seek emergency care immediately — before any report is issued — if you develop: rash, itching, swelling of the face or lips or breathlessness after contrast; severe dizziness or fainting; increasing pain or swelling at the cannula site; a sudden severe new headache; or limb weakness, loss of sensation, loss of bladder or bowel control, or fever with severe back pain.\*\*
