## Procedure steps

### 1. Falls history and associated symptoms

The patient is asked how many times they fell in the past year and the circumstances of each event: where, when, and whether there was dizziness, palpitations, loss of consciousness or tripping over an object. Loss of consciousness or palpitations before the fall points the assessment towards the heart and its rhythm, while tripping points towards vision, gait and the home environment.

### 2. Physical examination and standing blood pressure

Muscles, joints and the range of motion at hip, knee and ankle are examined, along with sensation and foot reflexes, balance and eye movements. Blood pressure is measured while seated and again after one and three minutes of standing to detect orthostatic hypotension, and the pulse and heart are examined.

### 3. In-clinic gait and balance tests

Short supervised tests are performed: standing from a chair, walking a set short distance, turning, returning and sitting; holding progressively narrower foot stances; and repeated sit-to-stand without using the hands. Results are documented so that later progress can be measured against a baseline.

### 4. Medication, vision and home environment review

All medicines and supplements are reviewed for anything that causes dizziness, drowsiness, blood pressure drops or hypoglycaemia, and adjustment or reduction is proposed by the prescriber. Vision and hearing checks are recommended, and the home is discussed in detail: night lighting, rugs, door thresholds, the bathroom, and how stairs are managed.

### 5. Exercise, rehabilitation and follow-up plan

A strength and balance exercise programme is prescribed for regular practice several times a week, with supervised rehabilitation where weakness is marked or fear of walking is limiting. A correctly sized walking aid is selected and its use taught, reassessment dates are set to measure progress and adjust the plan, and bone health is evaluated where indicated.

## About this service

Assessment of mobility difficulty and fall risk is a structured evaluation that measures how a person walks, balances and rises from a chair, and searches for the treatable causes that make them likely to fall. It is not requested only after a fracture; it is appropriate after a first fall, after a near-fall, or whenever someone reports unsteadiness or fear of walking outdoors.

A fall in an older adult is rarely a random accident and never an inevitable consequence of age. It is usually the sum of several small factors: weak thigh muscles, impaired balance, a blood pressure drop on standing, reduced vision, numb soles, knee osteoarthritis, or a sedating medicine that slows reaction time. The assessment is therefore deliberately broad. Muscles, joints, nerves, inner ear, heart and eyesight are examined, blood pressure is measured sitting and standing, and the medication list is reviewed for sedatives, sleeping tablets, blood pressure and diabetes drugs and older antihistamines, because polypharmacy itself is among the strongest drivers of falls.

In clinic, quick practical tests are used: observing gait pattern, stride length and steadiness, rising from a chair to walk a short distance and return, holding narrowed foot-stance positions to gauge postural stability, and repeated sit-to-stand to estimate lower-limb strength. The examiner notes whether the person reaches for furniture, inspects footwear and the height and condition of any cane or walker, and asks about stairs, bathroom layout and lighting at home.

The output is a practical plan rather than a label: a strength and balance exercise programme as the cornerstone, adjustment or withdrawal of risky medicines by the prescriber, correction of vision, review of vitamin D and bone health including whether osteoporosis treatment is indicated, physiotherapy or rehabilitation where needed, and correctly fitted walking aids.

The limits matter. This assessment does not prevent every fall, and it does not by itself treat a neurological condition such as Parkinson's disease, peripheral neuropathy or an inner-ear disorder; it detects them and directs them to the right treatment. It also does not treat osteoporosis, and fracture prevention may need a separate medical plan. Every decision rests on clinical examination and investigations, not on self-judgement.

## Preparation

Wear the shoes you actually walk in daily rather than new ones, because the footwear itself is part of the assessment. Bring your cane or walker if you use one, and bring and use your glasses and hearing aid. Bring all your medicines in their boxes, including supplements, herbal products and eye drops, since some medicines are a direct cause of unsteadiness. Before the visit, write down the details of every fall in the past year: approximate date, location, whether dizziness or palpitations came first, whether you lost consciousness, and whether you could get up unaided. Tell the doctor about dizziness when rising from bed, about fear of going outdoors, and about holding on to furniture at home. Wear comfortable clothing that allows the knees and ankles to be examined and free walking. Bring a companion, and do not hide any fall out of fear of losing your independence; hiding it is what increases the risk.

## Aftercare

Start the strength and balance programme without delay, since it is the single most effective way to reduce repeat falls; practise it several times a week, progress gradually, and stay near a stable support at first. Make the home modifications within days rather than months: lighting on the route to the bathroom, removal of loose rugs and cables, bathroom grab bars, and a secure stair rail. Get out of bed in two stages by sitting for a minute before standing beside a support. Always use your prescribed cane or walker, even indoors, drink enough fluid and do not skip meals. Do not stop or change any medicine on your own, and attend the reassessment appointment.

Seek care immediately if there is: \*\*a fall with a head strike, headache or vomiting, especially while on blood thinners\*\*; \*\*a fall preceded by loss of consciousness, palpitations or chest pain\*\*; \*\*inability to bear weight, or severe hip or shoulder pain after the fall\*\*; \*\*new limb weakness or numbness, slurred speech or double vision, which needs emergency services\*\*; \*\*rapid deterioration in walking over days or weeks\*\*; \*\*urinary retention or loss of control together with leg weakness\*\*.
