لكن مقارنتها بالتلبيسة المعدنية الجاهزة يجب أن تكون صادقة: التاج الزركوني يحتاج إزالة قدر أكبر من نسيج السنّ ليجلس بسماكته، ويحتاج سنّاً بارتفاع كافٍ وسيطرة جيّدة على الرطوبة والدم أثناء التثبيت، وهو أكثر عرضة للانفلات أو الكسر عند الأطفال الذين يصرّون على أسنانهم بقوّة، والتلبيسة المعدنية تبقى أعلى تحمّلاً في هذه الحالات وإن كانت أقل جمالاً. لذلك الاختيار بين النوعين قرار سريري بحسب السنّ والطفل، ويُحدَّد بالفحص والصور لا بالتفضيل الجمالي وحده.

حدود التلبيسة يجب أن تكون واضحة للأهل: التاج يحمي سنّاً واحدة ولا يمنع التسوّس في بقيّة الأسنان ولا في حافة اللثة إن أُهمل التفريش، ولا يمكن تبييضه أو تعديل لونه لاحقاً، وهو حلّ لسنّ مؤقتة بطبيعتها فيسقط مع السنّ اللبنية في وقتها الطبيعي. كذلك قد يحتاج طفل صغير جداً أو لديه أسنان كثيرة مصابة إلى مهدّئ أو تخدير عام لإنجاز العمل بأمان، وهذا يُقيَّم طبياً قبل البدء لا في يوم الجلسة.

## خطوات الإجراء

### 1. التقييم وقرار نوع التلبيسة

يُقيَّم امتداد التسوّس وارتفاع ما تبقّى من السنّ وحيوية اللبّ بالفحص والصور، وتُقارَن الخيارات مع الأهل: حشوة، تلبيسة زركونية، تلبيسة معدنية، أو خلع مع حافظ مسافة، مع شرح المزايا والحدود لكل خيار.

### 2. التخدير الموضعي والعزل

يُخدَّر موضع العمل بعد تخدير سطحي، ويُعزل السنّ بحاجز مطاطي عندما يكون ذلك ممكناً لأن الجفاف والسيطرة على اللثة شرط أساسي لنجاح التثبيت وثبات التاج.

### 3. إزالة التسوّس وعلاج اللبّ إن لزم

يُنظَّف كل النسيج المتسوّس، وإن وصل الالتهاب إلى اللبّ يُجرى بتر اللبّ ويُوضع حشو مناسب. إن كانت السنّ متهدّمة أكثر من قدرة التاج على الاستناد يُناقَش الخلع مع حافظ مسافة كبديل واقعي.

### 4. تحضير السنّ وتجربة التاج

يُنحَت السنّ بمقدار يسمح بجلوس التاج بلا ضغط على اللثة ولا رفع للإطباق، ثم تُجرَّب مقاسات جاهزة حتى يُختار الأنسب شكلاً وحجماً ولوناً، ويُتحقّق من إغلاق الطفل لأسنانه بشكل طبيعي.

### 5. التثبيت وفحص الإطباق والمتابعة

يُثبَّت التاج بإسمنت مناسب مع تنظيف الفائض كلّه من حول اللثة، ويُفحص الإطباق وحركة الفكّ، وتُشرح العناية المنزلية. تُحدَّد مراجعة دورية لتقييم حافة اللثة وثبات التاج وحالة الأسنان المجاورة.

## Procedure steps

### 1. Assessment and choosing the crown type

The extent of decay, the remaining tooth height, and pulp vitality are assessed clinically and radiographically, and the options are compared with the parents: a filling, a zirconia crown, a metal crown, or extraction with a space maintainer, with the advantages and limits of each explained.

### 2. Local anaesthesia and isolation

The area is anaesthetised after a topical agent, and the tooth is isolated with a rubber dam whenever feasible, because a dry field and control of the gum are essential for successful cementation and crown retention.

### 3. Caries removal and pulp treatment if needed

All carious tissue is removed, and if inflammation has reached the pulp, pulp treatment is carried out and an appropriate base placed. If the tooth is broken down beyond what a crown can be supported on, extraction with a space maintainer is discussed as a realistic alternative.

### 4. Tooth preparation and crown try-in

The tooth is reduced enough for the crown to seat without pressing on the gum or raising the bite, then prefabricated sizes are tried until the best fit in shape, size, and shade is selected, and the child's natural closure is checked.

### 5. Cementation, bite check, and follow-up

The crown is cemented with a suitable material, all excess is cleaned from around the gum, the bite and jaw movement are checked, and home care is explained. A review is scheduled to assess the gum margin, crown retention, and the neighbouring teeth.

## العناية بعد الإجراء

امنع الطفل من الأكل حتى يزول الخدر تماماً، وراقبه لئلّا يعضّ شفته أو لسانه المخدّر. قد تكون اللثة حول التاج محمرّة وحسّاسة يومين وربّما تنزف قليلاً عند التفريش، وهذا يهدأ عادة مع التنظيف اللطيف المنتظم لا مع تركه. فرّش أسنان طفلك مرّتين يومياً بإشراف بالغ بمعجون مناسب لعمره، ومرّر الخيط بين الأسنان الملاصقة للتاج لأن التسوّس يبدأ من الحافّة تحت اللثة. امنع العلك واللبان اللاصق والحلويات القاسية والثلج ولحس الأصابع على التاج، وامنع الشرب من زجاجة سكرية ليلاً. إن كان طفلك يصرّ على أسنانه أخبر الطبيب لأنه قد يوصي بوسيلة حماية. \*\*راجع فوراً عند سقوط التاج أو تحرّكه، أو ألم مستمر أو ألم عند العضّ، أو تورّم في الوجه أو اللثة، أو حرارة، أو حبّة صغيرة مع سائل قريبة من السنّ، أو نزف لثوي لا يهدأ بعد أيام من التنظيف الجيّد.\*\*

## About this service

Zirconia crowns for children are white crowns made of zirconium oxide that cover a badly damaged primary tooth to restore its shape and chewing function until it is shed naturally. They are most often used after pulp treatment of a primary tooth or when decay is too extensive for a conventional filling to hold.

Indications include wide decay involving more than one surface, a tooth after pulp treatment, a fractured front tooth after a fall or knock, enamel developmental defects or severe discolouration, and teeth in high-caries-risk children where repeated fillings have failed. Their main advantage is that they are tooth-coloured, resistant to staining, and highly polished, which reduces plaque accumulation, and most types are prefabricated in a range of sizes selected during the same appointment, so no laboratory stage is needed.

The comparison with a prefabricated metal crown should nevertheless be honest. A zirconia crown requires more tooth tissue to be removed so that its thicker wall can seat properly, it needs adequate remaining tooth height and good control of moisture and bleeding during cementation, and it is more prone to debonding or fracture in children who grind their teeth forcefully. Metal crowns remain more durable in those situations even though they are less attractive. Choosing between the two is therefore a clinical decision based on the tooth and the child, determined by examination and radiographs rather than by appearance alone.

The limits should be clear to parents. A crown protects one tooth; it does not prevent decay in the other teeth or at the gum margin if brushing is neglected. Its colour cannot be whitened or altered afterwards. It is a restoration for a tooth that is temporary by nature, so it is shed along with the primary tooth at the normal time. In addition, a very young child or one with many affected teeth may need sedation or general anaesthesia to complete the work safely, and that is assessed medically before starting rather than on the day of treatment.

## Preparation

Give the dentist your child's full medical history: heart, kidney, or blood conditions, asthma, seizures, allergies to any drug or material, current medications, and any special needs. Mention whether your child grinds their teeth at night or bites nails or pencils, because this influences the choice of crown. Book a time when the child is rested and give a light meal before the visit unless fasting was requested, since the appointment can run slightly long. Prepare the child with simple language about a tooth getting a protective white cap, avoid words such as drilling and needle, and do not promise that nothing will be felt. Ask in advance how many visits are expected and what the alternative is if the tooth turns out to be unsuitable for a crown. Bring previous radiographs and reports, and if sedation or anaesthesia is planned, ask about fasting instructions at least a day beforehand.

## Aftercare

Do not let your child eat until the numbness has completely gone, and supervise them so they do not bite a numb lip or tongue. The gum around the crown may look red and tender for a couple of days and may bleed slightly on brushing; this usually settles with gentle regular cleaning rather than by leaving it alone. Brush twice daily under adult supervision with an age-appropriate paste, and use floss between the crown and the adjacent tooth, because decay starts at the margin under the gum. Avoid chewing gum, sticky and hard sweets, ice, and finger sucking on the crown, and stop sugary night-time bottles. If your child grinds their teeth, tell the dentist, as a protective appliance may be advised. \*\*Seek care promptly if the crown comes off or feels loose, if there is persistent pain or pain on biting, facial or gum swelling, fever, a small blister with discharge near the tooth, or gum bleeding that does not settle after several days of good cleaning.\*\*
