## Procedure steps

### 1. Coordination with the treating team and medical screening

Before any exercise, your diagnosis, treatment plan, most recent blood results and any sites of metastasis are reviewed, and the oncologist's opinion on permitted loading is sought. Contraindications are documented: low platelets or neutrophils, anaemia, a central line, recent surgery, or severe fatigue that warrants postponement. This step is a safety requirement, not a formality.

### 2. Assessing capacity, fatigue and balance

What you can actually do is measured: comfortable walking distance, limb strength, balance and the ability to rise from a chair, the degree of fatigue and its effect on your day, and any numbness or tingling in the feet after chemotherapy. Small measurable goals are set, such as adding walking minutes each week, rather than general fitness targets.

### 3. Aerobic activity in short doses

Walking or a stationary bike at an intensity that still allows you to talk, split into several short bouts through the day rather than one long session, and built up gradually. The dose is reduced on treatment or nausea days rather than cancelled where your doctor allows, and is stopped and reported if you develop dizziness, unusual breathlessness or palpitations.

### 4. Light resistance, balance and flexibility

Light resistance work using body weight or an elastic band for the large muscle groups two to three times a week, with explicit allowance for bone metastases by avoiding high axial loads on the doctor's decision. Balance training is added to protect against falls, along with gentle stretching and graded shoulder and arm work after breast or lymph node surgery.

### 5. Follow-up and adjusting the plan with treatment cycles

Measurements are repeated periodically and the dose is adjusted up or down with each treatment cycle and with changes in blood results and symptoms, with rescreening whenever a new symptom appears. You and your family are taught the stop signals and when to contact the doctor. No duration is fixed and no session count is promised; everything is estimated initially and reassessed.

## About this service

Exercise for cancer patients is a prescribed, graded physical activity programme designed for someone during or after cancer treatment, coordinated with the treating team and based on the tumour type and stage, the treatment being given and current blood results. It is not a general fitness class but a plan that accounts for fatigue, changing blood counts, sites of metastasis, central lines and recent surgery, and that is adjusted with every treatment cycle.

It begins with an assessment of capacity, balance, strength and fatigue level, then low to moderate aerobic activity in short doses that accumulate through the day, light resistance work to preserve the muscle mass that the disease and its treatment consume, and flexibility and balance training to protect against falls, which matters particularly with chemotherapy-induced peripheral neuropathy. After breast or lymph node surgery, carefully graded shoulder and arm exercises are added.

Benefit is cumulative and comes from consistency rather than from any single session, and the commonest obstacle is stopping altogether on tired days. Where the doctor allows it, the right response is to reduce the dose rather than cancel it, because complete rest increases fatigue and accelerates muscle loss. Devices, electrical stimulation and massage support comfort and symptoms but do not replace movement, and physiotherapy here is not massage.

The limits must be stated with complete honesty. \*\*Exercise is not a cancer treatment, it is not a substitute for surgery, chemotherapy, radiotherapy or targeted drugs, and it does not shrink a tumour.\*\* Its role is reducing fatigue and improving strength, balance, sleep, mood and the ability to tolerate treatment. It also cannot be relied on to prevent lymphoedema, although it can be performed safely alongside it. Safety conditions apply: the programme is modified or postponed with severe anaemia, low platelets or low neutrophils, fever, nausea and dehydration, or bone metastases that require reduced axial loading by the doctor's decision. Dose and duration are estimated initially and reassessed, and suitability is decided by clinical examination and your oncologist's agreement rather than by reading.

## Preparation

Bring your diagnosis, treatment plan, most recent blood results and imaging reports, and tell the therapist when your chemotherapy or radiotherapy sessions fall, because the dose is planned around your difficult days. Declare any recent surgery, a central line or implanted port, any bone metastases or previous fracture, any numbness or weakness in the hands and feet, and your medications including steroids and blood thinners. Wear comfortable clothing and closed, non-slip shoes, and bring water and somewhere to sit when you tire. Do not come after a long fast, and do not exercise at all if you have fever, chills, bleeding or new breathlessness — inform your treating team first and obtain the doctor's approval before continuing.

## Aftercare

Make consistency your goal rather than intensity: short daily doses that add up beat one exhausting attempt, and on tired days reduce the duration rather than cancelling movement altogether where your doctor allows. Drink enough water, monitor your weight and appetite, and record how tired you feel after each activity so the dose can be adjusted with your therapist. If you have had lymph node surgery, keep to the prescribed arm exercises and report any new swelling or heaviness. Stop and contact your treating team immediately if you develop: \*\*new bone pain, or persistent night pain that worsens and does not settle with rest — this may indicate bone metastasis and needs assessment and imaging before any loaded exercise\*\*, \*\*progressive numbness or weakness in the legs or a change in bladder control — an emergency\*\*, \*\*fever with chills\*\*, \*\*breathlessness, chest pain or palpitations\*\*, or unexplained bleeding or bruising.
