## Procedure steps

### 1. History and external examination

The doctor asks about the colour and timing of the bleeding and its relation to defaecation, plus pain, itching, discharge, bowel pattern, medications and family history. The perianal skin is then inspected for tags, a fissure, a fistula opening, swelling or skin changes.

### 2. Digital rectal examination

A gentle lubricated finger examination assesses sphincter tone and checks for a mass, focal tenderness or impacted stool. It precedes insertion of the anoscope and yields information that inspection alone cannot, and the procedure may be stopped at this stage if pain is severe.

### 3. Inserting the anoscope

The lubricated anoscope is inserted slowly and then withdrawn gradually while the full circumference of the canal is inspected. You are asked to breathe calmly, and may be asked to strain gently so internal haemorrhoids or prolapse become visible. A sensation of pressure or an urge to defaecate is expected and does not mean harm is being done.

### 4. Findings and possible minor procedures

The findings are documented: the grade of internal haemorrhoids, the site and orientation of a fissure, a fistula opening, congestion or inflammation. Depending on the case and after explanation and your consent, a small biopsy may be taken or an internal haemorrhoid banded in the same session.

### 5. Explanation and next steps

The doctor explains what was found and, just as importantly, what this examination cannot exclude, decides whether you need a full colonoscopy, imaging or blood tests, and sets an initial plan for regulating stool and pain along with a review appointment.

## About this service

Anoscopy is a short diagnostic examination in which the doctor inserts a lit, rigid tube a few centimetres long — an anoscope — into the anal canal to view its wall and the lowest part of the rectum directly. It is normally done in the clinic without anaesthesia, using a lubricant, and in most cases needs no fasting and no escort.

It is requested for bleeding with bowel motions, anal pain or burning, persistent itching, a lump or protrusion, discharge staining underwear, or to follow a known fissure, fistula or haemorrhoids. Its main value is precise localisation of the source: internal haemorrhoids, an anal fissure, skin tags, inflammatory changes or a fistula opening. That distinction changes management completely, because haemorrhoid treatments do nothing for a fissure or a fistula.

Its most important limitation must be stated plainly: \*\*anoscopy views only a few centimetres, so it does not examine the colon and does not rule out cancer of the colon or the upper rectum.\*\* If bleeding comes with a change in bowel habit, weight loss, anaemia or a family history, or if you are of an age where screening is indicated, a full colonoscopy is what is needed. The presence of haemorrhoids does not exclude a second cause occurring at the same time, and this assumption is one of the commonest reasons a diagnosis is delayed. Likewise, anoscopy does not replace MRI in assessing complex or recurrent fistulas.

The examination is uncomfortable but usually tolerable and lasts minutes. It may be postponed or abandoned when pain is very severe — as with an acute fissure or an abscess — because the pain itself prevents an adequate look; the pain is treated first and the examination repeated, or it is performed under anaesthesia in theatre if necessary. During the examination a simple procedure may be carried out after explanation and your consent, such as taking a small biopsy or banding an internal haemorrhoid.

Whether you need this examination, and what wider tests should follow it, is decided by clinical examination and history rather than by interpreting your own symptoms.

## Preparation

This examination usually requires no fasting, but you may be asked to empty your bowels on the morning of the appointment or to use a small enema an hour or two beforehand — ask the clinic for its specific instructions and follow them. Tell the doctor if you take blood thinners or aspirin, which matters particularly if a biopsy or haemorrhoid banding might be done in the same session. Mention any allergy to local anaesthetic or latex, and say if you are pregnant. Do not apply creams or suppositories on the morning of the examination unless instructed, as they mask what needs to be seen. Wear comfortable clothing, and before the appointment write down a precise description of your symptoms: the colour of the blood, whether it is on the paper, in the water or mixed with stool, when it began and how it relates to pain — these details guide the diagnosis more than most people expect.

## Aftercare

In most cases you can return to work and your normal day straight after the examination: there is no general anaesthetic and no recovery period. You may notice mild discomfort or slight spotting of blood for a few hours, a little more if a biopsy was taken or a haemorrhoid was banded. A warm sitz bath in plain water eases the discomfort. If you had banding or a biopsy, avoid straining, heavy lifting and vigorous exercise for two days, and keep to softeners, fibre and water so the stool does not become hard. Complete any topical treatment or laxative for the full period prescribed, not just a day or two. Most importantly, complete the tests your doctor recommended — colonoscopy, blood tests or imaging — even if your symptoms improve, because relief of a symptom does not exclude its cause. \*\*Seek care immediately for heavy or continuing bleeding, severe worsening pain, fever and chills, inability to pass urine, or dizziness with pallor and a fast pulse.\*\*
