الوسائل تُصنَّف بحسب طريقة عملها ومدّتها. هرمونية قصيرة المدى مثل الحبوب المركّبة وحبوب البروجستين والإبر الشهرية أو ربع السنوية، وهرمونية طويلة المدى مثل الشريحة تحت الجلد واللولب الهرموني، وغير هرمونية مثل اللولب النحاسي والواقي الذكري والوسائل الحاجزة، ودائمة مثل ربط الأنابيب للمرأة وقطع القناة المنوية للرجل، إضافة إلى وسائل تعتمد على تتبّع أيام الخصوبة، والوسيلة الطارئة التي تُستخدم بعد جماع غير محمي.

الفرق العملي بين الوسائل ليس في نسبة نظرية بل في مدى اعتمادها على الالتزام اليومي. الوسائل طويلة المدى تعمل من تلقاء نفسها بعد التركيب ولذلك يكون أداؤها في الحياة الواقعية الأقرب إلى أدائها المثالي، أما الحبوب فتضعف فعاليتها كلما تكرّر نسيان الجرعة أو تكرّر القيء والإسهال. لذلك يُسأل عن نمط حياتك وقدرتك على الالتزام قبل الاختيار.

الاختيار الطبي يعتمد على أمور محدّدة: هل ترضعين وعمر الطفل، والعمر والتدخين، وضغط الدم، والصداع النصفي مع أعراض بصرية، وسابقة تجلّط في الساق أو الرئة، وأمراض الكبد، وسرطان الثدي، والسكري ومدّته، وغزارة الدورة أو آلامها، ومتى تخطّطين للحمل القادم. بعض الوسائل تُفيد أعراضاً أخرى مثل تخفيف غزارة الدورة أو آلامها، وبعضها لا يناسب حالات محدّدة إطلاقاً، وهذا ما يجعل الاستشارة ضرورية.

ولحدود الوسائل أهمية توازي فعاليتها: لا شيء من هذه الوسائل يحمي من الأمراض المنقولة جنسياً إلا الواقي، ووسائل منع الحمل لا تعالج تأخّر الإنجاب ولا تسبّبه بعد التوقّف عنها، ورجوع الخصوبة يكون سريعاً بعد معظم الوسائل ومتأخّراً بعض الأشهر بعد الإبرة طويلة المفعول. الوسيلة الطارئة ليست وسيلة إجهاض وليست بديلاً عن وسيلة منتظمة، والرضاعة وحدها ليست حماية موثوقة إلا بشروط صارمة وضمن أشهر محدودة. الترشيح لأي وسيلة يُحدَّد بالفحص السريري والتاريخ الطبي، لا بالقراءة ولا بتجربة قريبة أو صديقة.

## خطوات الإجراء

### 1. الاستشارة والتقييم الطبي

تُسجَّل خطتك الإنجابية والرضاعة وتاريخ الدورة وغزارتها، والأمراض المزمنة والصداع النصفي وسابقة التجلّط وأمراض الكبد وسرطان الثدي، وكل أدويتك، ويُقاس ضغط الدم والوزن. هذه المعلومات هي التي تستبعد وسائل وتفتح أخرى.

### 2. استعراض الخيارات والموازنة بينها

تُعرَض الوسائل المناسبة لحالتك مع شرح طريقة العمل والمدّة وما يعتمد منها على الالتزام اليومي، والآثار الجانبية المتوقّعة مثل تغيّر نمط النزف، وسرعة رجوع الخصوبة، وأيّها يفيد غزارة الدورة أو آلامها، وأيّها يناسب الرضاعة.

### 3. استبعاد الحمل والتحضير للبدء

يُستبعَد وجود حمل قائم بالتاريخ أو بفحص حمل عند الشك. قبل تركيب اللولب يُجرى فحص حوض ويُعالَج أي التهاب مهبلي أولاً، وقد يُطلب سونار لتقييم شكل الرحم. تُحدَّد لحظة البدء المناسبة ومتى تحتاجين حماية إضافية في الأيام الأولى.

### 4. البدء أو التركيب

الوسائل الفموية والحاجزة تبدأ بشرح دقيق للاستخدام وما تفعلينه عند نسيان جرعة. تركيب اللولب أو الشريحة يُجرى في العيادة خلال دقائق مع تخدير موضعي للشريحة، وقد تحدث مغص أو نزف بسيط بعد اللولب لأيام. تُشرَح طريقة التأكّد من مكان الوسيلة.

### 5. المتابعة وإمكانية التبديل

تُحدَّد مراجعة خلال الأسابيع الأولى وبعدها دورياً لتقييم التحمّل والنزف والضغط. تغيّر نمط النزف في الأشهر الأولى شائع مع الوسائل الهرمونية ولا يعني الفشل. إن لم تُريحك الوسيلة فالتبديل متاح ومخطَّط له، ولا يُقاس نجاح التنظيم بوسيلة واحدة تُفرَض عليك.

## Procedure steps

### 1. Consultation and medical assessment

Your pregnancy plans, breastfeeding status, menstrual pattern and bleeding heaviness are recorded, along with chronic conditions, migraine, previous clots, liver disease and breast cancer, plus all your medications, and blood pressure and weight are measured. This information is what rules some methods out and opens up others.

### 2. Reviewing and comparing the options

The options suitable for you are presented, with how each works, how long it lasts, which ones depend on daily compliance, the expected side effects such as a change in bleeding pattern, how quickly fertility returns, which ones also help heavy or painful periods, and which are compatible with breastfeeding.

### 3. Excluding pregnancy and preparing to start

An existing pregnancy is excluded by history or by a pregnancy test when in doubt. Before fitting an intrauterine device a pelvic examination is performed and any vaginal infection is treated first, and an ultrasound may be requested to assess the shape of the uterus. The right starting point is chosen, along with whether you need extra protection during the first days.

### 4. Starting or fitting the method

Oral and barrier methods begin with precise instructions on use and on what to do if a dose is missed. Fitting an intrauterine device or an implant is done in the clinic within minutes, with local anaesthesia for the implant, and cramping or light bleeding for a few days is common after an IUD. You are shown how to check the method is in place.

### 5. Follow-up and switching

A review is arranged in the first weeks and then periodically to assess tolerance, bleeding and blood pressure. A change in bleeding pattern during the first months is common with hormonal methods and does not mean failure. If a method does not suit you, switching is available and planned; successful family planning is not measured by forcing one method on you.

## التحضير قبل الإجراء

اكتبي قبل الاستشارة ما يهمّك فعلاً: هل تريدين تأجيل الحمل سنة أم سنوات، وهل انتهيتِ من الإنجاب، وهل ترضعين وعمر طفلك. سجّلي تاريخ آخر دورة ونمطها وغزارتها وآلامها. أحضري قائمة أدويتك ومكمّلاتك كلها، وأخبري الطبيب عن التدخين، وارتفاع الضغط، والصداع النصفي خصوصاً إن رافقته أعراض بصرية، وسابقة تجلّط في الساق أو الرئة، وأمراض الكبد أو المرارة، والسكري ومدّته، وسرطان الثدي في تاريخك أو عائلتك، وعمليات الحوض السابقة، وأي نزف مهبلي غير مفسَّر. إن كنتِ تفكّرين باللولب فأخبري الطبيب بأي التهاب مهبلي حالي أو إفرازات كريهة. لا تبدئي أي وسيلة هرمونية من الصيدلية بلا تقييم، ولا تستخدمي وصفة قريبة أو صديقة.

## العناية بعد الإجراء

التزمي بطريقة الاستخدام كما شُرحت لك بالضبط، واعرفي مسبقاً ما تفعلينه عند نسيان حبة أو تأخير موعد الإبرة أو حدوث قيء وإسهال، ومتى تحتاجين حماية إضافية أو وسيلة طارئة. توقّعي تغيّراً في نمط النزف في الأشهر الأولى مع الوسائل الهرمونية: نزف خفيف متقطّع أو دورات أخفّ أو انقطاعها مع بعض الوسائل، وهذا شائع ولا يعني فشل الوسيلة. بعد تركيب اللولب توقّعي مغصاً ونزفاً بسيطاً أياماً، وتعلّمي تحسّس الخيطين، وحدّدي مراجعة بعد أول دورة. راجعي الطبيب قبل إضافة أي دواء جديد لأن بعض الأدوية تُضعف فعالية الوسائل الهرمونية، وقيسي ضغطك دورياً.

راجعي فوراً عند: \*\*تأخّر الدورة مع ألم في أسفل البطن من جهة واحدة أو دوخة أو إغماء، فقد يكون حملاً خارج الرحم وهذه حالة طوارئ\*\*، أو \*\*ألم وتورّم واحتقان في ساق واحدة، أو ألم صدر أو ضيق نفس مفاجئ، أو صداع شديد جديد مع اضطراب في الرؤية أو ضعف في أحد الأطراف أو تلخبط في الكلام، أو اصفرار العين، أو نزف مهبلي غزير يُبلّل فوطة كل ساعة، أو حرارة مع ألم حوضي وإفرازات كريهة بعد تركيب اللولب، أو عدم تحسّس خيط اللولب أو الإحساس بجزء صلب منه في المهبل\*\*.

## About this service

Family planning means a woman or a couple deciding when to have children, how many, and how long to leave between pregnancies, and contraception is the set of tools that carries out that decision. The clinic service is not simply handing over a method; it is a consultation that assesses your health and reproductive history, matches you to the most suitable option, and explains how it is used, what to expect from it and what it does not do.

Methods are grouped by how they work and how long they last. Short-acting hormonal options include combined pills, progestogen-only pills and monthly or three-monthly injections. Long-acting hormonal options include the subdermal implant and the hormonal intrauterine device. Non-hormonal options include the copper intrauterine device, condoms and barrier methods. Permanent options are tubal ligation for women and vasectomy for men. There are also fertility-awareness methods based on tracking fertile days, and emergency contraception used after unprotected intercourse.

The practical difference between methods is not a theoretical figure but how much each one depends on daily compliance. Long-acting methods keep working on their own once fitted, so their real-life performance stays close to their ideal performance. Pills lose effectiveness whenever doses are missed or when vomiting and diarrhoea recur. That is why your daily routine and your ability to keep to a schedule are discussed before any choice is made.

The medical choice depends on specific factors: whether you are breastfeeding and the baby's age, your age and smoking, blood pressure, migraine with visual aura, any previous clot in the leg or lung, liver disease, breast cancer, diabetes and its duration, heavy or painful periods, and when you plan your next pregnancy. Some methods also help other symptoms, such as reducing heavy or painful bleeding, while others are entirely unsuitable in particular conditions, which is exactly why the consultation matters.

The limits are as important as the effectiveness. None of these methods protects against sexually transmitted infections except the condom. Contraception does not treat difficulty conceiving, and it does not cause infertility after you stop it; fertility returns quickly after most methods and may take some months after the long-acting injection. Emergency contraception is not a method of abortion and is not a substitute for regular contraception, and breastfeeding alone is not reliable protection except under strict conditions and for a limited number of months. Which method suits you is decided by clinical assessment and medical history, not by reading or by a relative's or friend's experience.

## Preparation

Before the consultation, write down what actually matters to you: whether you want to delay pregnancy by a year or by several years, whether you have completed your family, and whether you are breastfeeding and your baby's age. Note the date of your last period and its pattern, heaviness and pain. Bring a complete list of your medications and supplements, and tell the clinician about smoking, high blood pressure, migraine and especially migraine with visual symptoms, any previous clot in the leg or lung, liver or gallbladder disease, diabetes and its duration, breast cancer in you or your family, previous pelvic surgery, and any unexplained vaginal bleeding. If you are considering an intrauterine device, mention any current vaginal infection or offensive discharge. Do not start a hormonal method bought over the counter without assessment, and never use a relative's or friend's prescription.

## Aftercare

Use your method exactly as instructed, and know in advance what to do if you miss a pill, delay an injection or have vomiting and diarrhoea, and when you need extra protection or emergency contraception. Expect a change in bleeding pattern in the first months with hormonal methods: light irregular spotting, lighter periods, or no periods with some methods. This is common and does not mean the method has failed. After an intrauterine device is fitted, expect cramping and light bleeding for a few days, learn to feel the threads, and book a review after your first period. Check with your clinician before adding any new medication, because some drugs reduce the effectiveness of hormonal methods, and have your blood pressure checked periodically.

Seek care immediately if you have: \*\*a missed period with one-sided lower abdominal pain, dizziness or fainting, which may be an ectopic pregnancy and is an emergency\*\*, or \*\*pain, swelling and redness in one leg, sudden chest pain or breathlessness, a new severe headache with disturbed vision, weakness in a limb or slurred speech, yellowing of the eyes, heavy vaginal bleeding soaking a pad every hour, fever with pelvic pain and offensive discharge after an IUD fitting, or inability to feel the IUD threads or feeling a hard part of it in the vagina\*\*.
