## Procedure steps

### 1. Clinical examination and confirming the diagnosis

The doctor palpates the tender point and the nodule at the base of the finger, watches for catching or locking during grip and release, measures range of motion, and examines the wrist and other digits to exclude nerve compression or palmar contracture. The diagnosis is mainly clinical; blood sugar or thyroid tests may be requested.

### 2. A trial of non-surgical care

Activities that involve repeated forceful gripping are modified, a night splint holding the finger straight is prescribed, and tendon-gliding exercises are taught. A local anti-inflammatory injection may be suggested at the doctor's discretion, and the response is reviewed after an interval before any surgical decision.

### 3. Local anaesthesia and preparation

The procedure is done as a day case under local anaesthesia of the palm, so you stay awake and can move the finger when asked. The skin is disinfected and draped, and a tourniquet or a wide-awake technique may be used depending on the chosen approach.

### 4. Releasing the tendon sheath

Through a small palmar incision at the base of the finger, the first annular ligament is divided lengthwise to widen the tunnel while the surgeon carefully protects the cutaneous nerves and vessels. You are then asked to bend and straighten the finger to confirm the locking has gone before the skin is closed with superficial sutures.

### 5. Rehabilitation and follow-up

Early finger movement is encouraged from the same day within the limits of pain, with the hand elevated to reduce swelling. Sutures usually come out after about ten to fourteen days, hand therapy may be prescribed if the joint is stiff or the problem is long-standing, and gripping work is resumed gradually.

## About this service

Trigger finger is a narrowing of the tendon sheath in the palm at the base of the finger. The flexor tendon can no longer glide freely through its tunnel, so the finger clicks as it straightens, or locks in a bent position and has to be released with the other hand. The operation for it is a release of the first annular ligament at the mouth of the tunnel; it is not surgery on the joint and not surgery on a nerve.

The condition is most common after the age of forty and more frequent in women. It is associated with diabetes, an underactive thyroid, rheumatoid arthritis, and work that involves repeated forceful gripping or vibrating tools. Typical complaints are pain and swelling at the base of the finger, a small nodule that can be felt in the palm, morning stiffness that eases with movement, and repeated catching that may become a fixed lock if neglected. More than one finger, or both hands, can be affected.

Management usually begins without surgery: modifying activity to reduce repeated hard gripping, a night splint that holds the finger straight, and tendon-gliding exercises. A local anti-inflammatory injection may be offered at the doctor's clinical discretion. Surgery is discussed when non-surgical care fails, when locking keeps returning, or when the finger stays locked. It is normally a day-case procedure under local anaesthesia through a small palmar incision, or by a percutaneous needle technique in selected cases and experienced hands.

The limits are clear. Release opens the tunnel and resolves the locking, but it does not by itself restore full motion in a joint that has been stiff for many months; that needs hand therapy. It does not treat numbness caused by nerve compression at the wrist, which is a separate condition that can coexist. It does not correct Dupuytren's contracture, and it does not prevent another finger from becoming a trigger finger later. People with diabetes may take longer to regain full range. Candidacy is decided by clinical examination, not by reading.

## Preparation

Tell the doctor about all your medicines, especially blood thinners, aspirin and herbal supplements, and about any allergy to local anaesthetic, dressings or antiseptics. Mention if you have diabetes and bring your recent readings, since good control lowers the risk of infection and delayed healing. Report any numbness in the hand, wrist pain, previous finger stiffness or earlier hand surgery. Remove rings and bracelets before the appointment and leave them off until swelling settles, trim your nails and wash the hand well, and do not apply creams or cosmetics to the palm on the day. Wear a top with loose sleeves, arrange transport if you will need more than local anaesthesia, and ask when you can return to work and when the sutures will be removed.

## Aftercare

Keep the hand raised above heart level for the first few days to limit swelling, and move the finger gently from the same day, because keeping it still causes stiffness and adhesions. Keep the dressing clean and dry, do not soak it while showering until you are told it is safe, and put no herbal remedies or oils on the wound. Avoid heavy lifting, forceful gripping and vibrating tools for roughly two to four weeks depending on your job, follow the prescribed hand exercises, and attend for suture removal on time.

\*\*Contact your doctor immediately if there is: spreading redness, local heat, pus from the wound or a fever; severe pain not relieved by your painkiller; persistent numbness or loss of sensation in the fingertip; inability to actively bend the finger when you could before; or painful swelling of the whole finger.\*\* Persistent numbness may mean a cutaneous nerve injury and loss of active bending may mean the tendon has slipped, and neither should be left to the next routine visit.
