## Procedure steps

### 1. Diagnosis before the surgical decision

The pathway starts with examination of the neck, endoscopy of the throat and larynx, and ultrasound, followed by fine-needle aspiration or a CT scan when needed, plus thyroid function and calcium tests. Surgery is not performed on a neck mass before its nature and origin are known.

### 2. Voice assessment and anaesthesia

The vocal cords are assessed endoscopically beforehand to document their baseline state, particularly in thyroid surgery and after any previous neck operation. The procedure is usually performed under general anaesthesia with a breathing tube, and intraoperative nerve monitoring may be used to identify the laryngeal nerve.

### 3. Incision, exposure and nerve protection

An incision is made in one of the natural skin creases so the scar is less conspicuous, the layers are separated carefully, and the major vessels and nerves — the recurrent laryngeal nerve, its superior branch, and the tongue and shoulder nerves depending on the region — are identified and preserved before anything is removed. This step is the core of safety in neck surgery.

### 4. Excision and histological analysis

The lobe, gland, mass or targeted nodes are removed according to the plan, and every specimen is sent for histology because the final diagnosis rests on it alone. Bleeding is controlled and the wound is closed in layers; a small drain may be left and is usually removed the next day.

### 5. Postoperative monitoring

The neck is monitored to detect any haematoma early, voice and breathing are assessed, and blood calcium is measured after total removal alongside watching for symptoms of a low level. The medication plan, hormone replacement and follow-up dates are explained before discharge, and the histology result is given at a later visit.

## About this service

Neck surgery is an umbrella term for operations performed on neighbouring organs packed into a small space crowded with vessels, nerves and glands: thyroid and parathyroid surgery, excision of neck masses and cysts, lymph node dissection, approaches to the salivary glands, and drainage of deep neck infections. There is therefore no single operation called neck surgery; there is a plan built on the specific diagnosis, the site of the mass and its relationship to the structures around it.

Diagnosis always comes before surgery. \*\*A neck lump that lasts more than two weeks in an adult, hoarseness beyond three weeks, or progressive difficulty swallowing must be investigated to exclude a tumour\*\* — not watched, and not treated with repeated courses of antibiotics. Standard assessment includes clinical examination, endoscopy of the throat and larynx, and ultrasound, often with fine-needle aspiration or a CT scan, plus thyroid function tests when the thyroid is suspected.

In thyroid surgery the risks must be stated honestly. The recurrent laryngeal nerve that moves the vocal cord runs immediately alongside the gland; injuring it causes hoarseness or a weak voice that may be temporary or may be permanent, and injury to both nerves, though rare, threatens the airway and needs urgent intervention. After total removal the parathyroid glands may be bruised or removed with the specimen, so blood calcium falls and causes tingling around the mouth and in the fingertips with cramping of the hands; this needs monitoring and replacement that can be prolonged. Lifelong thyroid hormone replacement is also required after total thyroidectomy.

The limits matter too. A biopsy or removal of a single lymph node is a diagnostic step that establishes the nature of the disease and is not in itself cancer treatment; draining an abscess does not replace medical therapy; removing a gland does not remove the need for follow-up and blood tests; and the incision leaves a permanent scar that fades but never disappears. Suitability and the type of operation are decided by clinical examination, imaging and biopsy results, not by reading or by comparison with someone else.

## Preparation

Tell your doctor about every medicine you take, especially blood thinners, anticoagulants, antiplatelet drugs, thyroid medication, steroids and herbal supplements, and never stop any of them on your own; only the prescriber decides. Report any previous neck surgery or radiotherapy to the neck, and any hoarseness, voice change, difficulty swallowing, breathlessness or recent snoring. Mention heart disease, high blood pressure, diabetes, clotting disorders and allergies to medicines or dressings, and bring your blood tests, imaging and any needle-aspiration report. Stop smoking before and after the appointment, as it increases wound and airway complications. Follow the fasting period set by the anaesthetic team and arrange for someone to stay with you afterwards.

## Aftercare

Keep your head raised on an extra pillow, avoid heavy lifting, deep bending and extending the neck backwards in the first days, and walk around the house early to reduce clot risk. Take your medicines, hormone replacement and calcium exactly as prescribed, never change them yourself, and attend your blood tests on time. Keep the wound clean and dry, and start with fluids then soft food if swallowing is sore. \*\*Seek care immediately — or call an ambulance — if you develop difficulty breathing or a harsh noise on breathing in, rapidly increasing neck swelling or a choking tightness under the wound which may be bleeding, tingling around the mouth or in the fingertips or cramping of the hands which signal low calcium, worsening hoarseness or a clearly weak voice or repeated choking on liquids, or fever with pus and spreading redness around the wound.\*\*
