## Procedure steps

### 1. Confirming drug resistance and documenting seizure type

The treatment history is reviewed to confirm that appropriate medicines were used correctly, the seizure types, frequency and triggers are documented, and non-epileptic events are excluded, since that distinction changes the whole decision.

### 2. Localising seizure onset with imaging and EEG

The patient is admitted for several days of video-EEG monitoring until enough seizures are recorded, high-resolution MRI and functional imaging are obtained, and language and memory are assessed neuropsychologically to establish what must be protected.

### 3. Invasive electrode monitoring when required

If the non-invasive results disagree, fine electrodes are implanted inside the skull or on the brain surface in a separate operation, and the patient is monitored for several days to define the seizure onset zone and map function before any resection.

### 4. Resection, disconnection or device implantation

The procedure is chosen according to the findings: removal of the causative area, disconnection of the pathways along which seizures spread, or implantation of a stimulation device that modulates electrical activity. It is performed under microscopic magnification with functional maps to protect speech, movement and vision.

### 5. Follow-up, medication adjustment and rehabilitation

Seizures are recorded in a careful diary after surgery, medication is initially maintained and only later considered for gradual reduction once stability is proven, with reassessment of memory and language, psychological and social follow-up, and support in returning to study or work.

## About this service

Epilepsy surgery is a group of neurosurgical procedures directed at the area of the brain from which seizures arise: that area is either removed, disconnected from the rest of the network, or influenced by an implanted device that modulates its electrical activity. It is offered to people whose seizures are not controlled by medication, and never as a first option.

Epilepsy is described as drug-resistant when two appropriate medicines, used correctly and tolerated, have failed to stop the seizures. At that point pre-surgical evaluation begins: several days of video monitoring with electroencephalography, high-resolution MRI, functional imaging, and neuropsychological assessment of memory and language. When surface studies are not conclusive, electrodes may be implanted inside the skull to localise the seizure onset precisely.

The decision is collective by nature. An epileptologist, neurosurgeon, neuroradiologist, neuropsychologist and experienced nursing team weigh the potential benefit against the risk to an essential function such as speech, memory or visual field. Candidacy is decided by clinical examination and the results of this detailed evaluation, not by reading a web page.

The limits are explicit. Not all epilepsy is operable, particularly when the onset is widespread, multifocal, or inside a critical functional area. Surgery does not mean stopping medication afterwards; continuing it for a period is necessary, and any reduction is gradual and decided by the treating physician alone. Neuromodulation devices aim to reduce the number and severity of seizures rather than to abolish them. Surgery also does not correct learning difficulties or the psychological conditions that often accompany epilepsy; those need a parallel plan.

One dangerous myth must be corrected: \*\*never put anything in the mouth of a person having a seizure\*\*, and do not try to restrain them by force, because this causes injuries to teeth, jaw and fingers. Instead turn the person onto their side, protect the head, move hard objects away, note the time the seizure began, and call emergency services if it lasts more than five minutes or repeats without recovery of awareness.

## Preparation

Never stop or reduce your antiseizure medicines on your own at any stage, as this can cause a prolonged and dangerous seizure. Keep a detailed seizure diary recording date, time, pattern, duration and triggers, and bring video recordings of your seizures if you have them, as these are very useful to the team. Tell the team about all your medicines, supplements and blood thinners. Sleep well before appointments unless you are asked to do the opposite during monitoring. Complete the MRI and neuropsychological assessments on time. Arrange a companion to stay with you during the monitoring days, and bring comfortable clothing and toiletries, as the admission may be long. Ask the team about driving, work and study rules both before and after surgery.

## Aftercare

Continue your antiseizure medicines exactly as prescribed and never adjust them yourself; stopping early is the commonest cause of seizures after surgery. Record every seizure or unusual sensation in your diary. Keep a regular sleep pattern, avoid sleep deprivation, exhaustion and alcohol, and follow the team's rules on driving, swimming and working at heights. Care for the wound, and do not eat or drink while you feel a brief lapse of awareness coming. Teach your family correct first aid: turn the person onto their side, protect the head, \*\*put nothing in the mouth and do not restrain them\*\*.

\*\*Call emergency services immediately\*\* if \*\*a seizure lasts more than five minutes\*\* or \*\*seizures repeat without recovery of awareness in between\*\*, if breathing difficulty or blue discolouration follows, or if a head injury occurs during the seizure. \*\*Attend the emergency department\*\* for fever with neck stiffness, severe increasing headache with vomiting or drowsiness, new one-sided weakness or difficulty speaking, fluid leaking from the wound or wound opening, or a sudden change in vision or mood.
