وأهمّ ما يجب فهمه هو حدود هذا التخصّص: \*\*جراحة الغدد الصماء تُعالج الزيادة لا النقص.\*\* فهي تُوقف إفرازاً زائداً صادراً عن ورم أو تضخّم محدَّد، ولا دور لها في نقص الإفراز — كخمول الغدة الدرقية أو قصور الغدة الكظرية أو نقص هرمون النمو — وهذه تُعالج ببدائل هرمونية دوائية. وهي ليست بديلاً عن المتابعة: كثير من المرضى يحتاجون بعد الجراحة هرموناً بديلاً أو مراقبة مخبرية طويلة الأمد. وهي ليست الخيار الأول لكل عقيدة أو كتلة؛ فمعظم عقيدات الدرقية الصغيرة والكتل الكظرية الصامتة تُتابع بالتصوير ولا تُستأصل. كما أن الجراحة توقف السبب ولا تُصلح مضاعفات تأخّرت طويلاً، مثل هشاشة عظام متقدّمة أو تلف كلوي سابق.

وسلامة هذه الجراحات تعتمد على ضبط الحالة هرمونياً قبل غرفة العمليات، فالدخول إلى التخدير بغدة غير مضبوطة يرفع الخطر بدرجة واضحة — كما في فرط نشاط الدرقية غير المضبوط أو ورم القواتم غير المُحضَّر دوائياً. ولهذا يكون العمل عادة فريقياً: طبيب الغدد الصماء، وجرّاح الغدد، وطبيب الأشعة، وطبيب الأنسجة، ومعهم طبيب الأورام في الحالات الورمية.

الترشيح للجراحة ونوعها يُحدَّدان بالفحص السريري والتحاليل والتصوير، لا بقراءة صفحة؛ وقد ينتهي التقييم إلى أن الأفضل لك دواء أو متابعة دورية بلا جراحة إطلاقاً.

## خطوات الإجراء

### 1. الاستشارة والتقييم الهرموني

يُراجع الجرّاح شكواك وتاريخك المرضي والعائلي وأدويتك، ويُطلب تحليل الهرمونات المناسب للغدة المعنية لإثبات ما إذا كان هناك إفراز زائد فعلاً، لأن الأعراض وحدها لا تكفي للتشخيص.

### 2. التصوير وتحديد الموقع

يُستخدم التصوير بالموجات فوق الصوتية أو المقطعي أو الرنين، وربما تصوير وظيفي، لتحديد موقع الكتلة وحجمها وعلاقتها بالأعصاب والأوعية، وقد تُؤخذ خزعة بالإبرة عند وجود اشتباه بورم.

### 3. ضبط الحالة قبل العمل

تُضبط الهرمونات والضغط والسكر والكالسيوم والبوتاسيوم قبل التخدير، وتُراجَع مسيّلات الدم والكورتيزون، ويُقيَّم القلب والرئة؛ وتأخير أسابيع لضبط الحالة أفضل من عملية مبكّرة غير آمنة.

### 4. العملية

تُجرى تحت تخدير عام بأسلوب قليل التوغّل عبر ثقوب أو شقّ صغير حيث يكون ذلك مناسباً، أو بالجراحة المفتوحة في الكتل الكبيرة والمشتبهة، مع حماية الأعصاب المجاورة، وقد تُقاس هرمونات أثناء العمل للتأكد من نجاح الاستئصال.

### 5. المتابعة الهرمونية بعد الجراحة

تُراقَب الهرمونات والكالسيوم والصوت ووظائف الحياة بعد العملية، وتُضبط جرعات البدائل الهرمونية عند الحاجة، ثم تُوضع خطة متابعة دورية بالتحاليل والتصوير حسب نوع المرض ونتيجة الأنسجة.

## Procedure steps

### 1. Consultation and hormonal assessment

The surgeon reviews your complaint, personal and family history and medications, and requests the hormone tests relevant to the gland in question to establish whether there really is overproduction — symptoms alone are never enough.

### 2. Imaging and localisation

Ultrasound, CT or MRI, and sometimes functional imaging, are used to define the site and size of the lesion and its relationship to nerves and vessels. A needle biopsy may be taken when malignancy is suspected.

### 3. Pre-operative optimisation

Hormones, blood pressure, glucose, calcium and potassium are optimised before anaesthesia, blood thinners and steroids are reviewed, and heart and lungs are assessed. Waiting weeks to stabilise a patient is safer than operating early on an unprepared gland.

### 4. The operation

Surgery is performed under general anaesthesia, minimally invasively through small incisions where appropriate, or open for large and suspicious lesions, with careful protection of adjacent nerves. Hormone levels may be measured during the operation to confirm the excision has succeeded.

### 5. Post-operative hormonal follow-up

Hormones, calcium, voice and vital signs are monitored after surgery, replacement doses are adjusted where needed, and a schedule of laboratory and imaging follow-up is set according to the disease and the pathology result.

## العناية بعد الإجراء

تختلف فترة التعافي بحسب الغدة والأسلوب الجراحي، والقاعدة العامة: امشِ مبكراً، تنفّس بعمق كما يُعلّمك الفريق، اعتنِ بالجرح كما يُوصى، وتجنّب رفع الأثقال والجهد الشديد في المدّة التي يحدّدها الجرّاح. لا تُعدّل جرعات هرمونك البديل أو أدوية الضغط والسكر بنفسك بعد العملية، فبعضها يُصبح أكثر من حاجتك، والتعديل يكون بالتحاليل لا بالإحساس. احضر مواعيد سحب الدم حتى لو كنت بحال جيدة، واحمل معك قائمة أدويتك وبطاقة تُبيّن إن كنت تعتمد على بدائل الكورتيزون. \*\*راجع فوراً عند: تورّم سريع في الرقبة أو ضيق نفَس أو صعوبة بلع شديدة، بحّة مستمرة، تنميل حول الفم أو تشنّج في اليدين، ضعف شديد أو دوخة أو إغماء أو تقيّؤ متكرّر، خفقان شديد أو ارتجاف أو ارتفاع حرارة، ألم صدر أو ضيق نفَس، تورّم وألم في ساق واحدة، أو احمرار وتصريف من الجرح.\*\* ولا تنتظر موعدك المُجدول إن ظهرت هذه العلامات.

## About this service

Endocrine surgery is a focused subspecialty within general surgery dealing with surgical disease of the hormone-producing glands: the thyroid, the parathyroid glands, the adrenal glands, hormone-secreting tumours of the pancreas and digestive tract, and some masses of the neck and upper chest. It is a field in which the decision is biochemical before it is anatomical: hormone tests establish that a problem exists, imaging locates it, and only then is it decided whether surgery is better than medication or surveillance.

The commonest referrals include neck nodules and enlargements that cause pressure symptoms or carry suspicious imaging features; primary hyperparathyroidism with high calcium, kidney stones and bone loss; adrenal tumours that are hormone-secreting, large or suspicious; pancreatic neuroendocrine tumours causing repeated low blood sugar or other hormonal syndromes; and glandular overactivity occurring within inherited syndromes that affect more than one gland.

The most important point to understand is the boundary of the field: \*\*endocrine surgery treats excess, not deficiency.\*\* It stops overproduction arising from a defined tumour or enlargement, and it has no role in underproduction — an underactive thyroid, adrenal insufficiency or growth hormone deficiency — which are treated with hormone replacement. It is also not a substitute for follow-up: many patients need replacement hormone or long-term laboratory monitoring after surgery. And it is not the first option for every nodule or mass; most small thyroid nodules and silent adrenal lesions are followed with imaging rather than removed. Surgery stops the cause but does not repair complications that have been allowed to develop over years, such as advanced osteoporosis or established kidney damage.

Safety in these operations depends heavily on getting the hormonal state under control before theatre, because going to anaesthesia with an unprepared gland clearly raises risk — as in uncontrolled thyroid overactivity or a phaeochromocytoma that has not been medically prepared. For this reason care is usually delivered by a team: the endocrinologist, the endocrine surgeon, the radiologist and the pathologist, joined by an oncologist in cancer cases.

Whether surgery is right for you, and which operation, is determined by clinical examination, laboratory testing and imaging rather than by reading a page — and assessment may well conclude that medication or periodic surveillance, with no surgery at all, is the better path.

## Preparation

Bring your previous hormone results and imaging reports in a readable format, and write down how your complaint has evolved over time. Tell the team about every medicine and supplement you take, especially thyroid medication, steroids, blood pressure and diabetes drugs, blood thinners and aspirin, and calcium or iodine supplements, and stop nothing on your own. Report pregnancy, possible pregnancy or breastfeeding, any drug or contrast allergy, previous experience with anaesthesia, and any family history of endocrine disease or inherited tumours, since that may widen the investigation plan. Expect a fasting period before surgery set by the anaesthetic team, stop smoking as long before as possible, and arrange someone to accompany you and to help at home during the first days.

## Aftercare

Recovery differs with the gland and the surgical approach, but the general rules hold: walk early, do the breathing exercises the team teaches you, care for the wound as instructed, and avoid heavy lifting and strenuous effort for the period your surgeon specifies. Do not adjust your replacement hormone or your blood pressure and diabetes medication yourself after surgery — some doses become more than you need, and adjustment is guided by blood tests rather than by how you feel. Attend for blood tests even when you feel well, and carry a list of your medicines plus a card stating whether you depend on steroid replacement. \*\*Seek care immediately for: rapid neck swelling, breathlessness or severe difficulty swallowing; persistent hoarseness; numbness around the mouth or hand spasm; profound weakness, dizziness, fainting or repeated vomiting; marked palpitations, tremor or fever; chest pain; swelling and pain in one leg; or redness and discharge from the wound.\*\* Do not wait for your scheduled appointment if any of these appear.
