تُجرى الجراحة غالباً بتخدير موضعي وربّما مع مهدّئ، ويُعالَج قطاع من الفم في كل جلسة. تُزال الخيوط عادة بعد أسبوع إلى أسبوعين، ويبلغ التورّم والانزعاج ذروتهما في اليومين الأولين ثم يتراجعان. ولن تعود اللثة إلى شكلها السابق تماماً: توقّع أن تبدو الأسنان أطول، وأن تزيد حساسية الجذور مؤقتاً، وأن تحتاج فرشاة بين الأسنان لمساحات صارت أوسع.

حدود الجراحة صريحة: \*\*هي لا تشفي التهاب دواعم السن شفاءً نهائياً ولا تُعيد كل العظم المفقود\*\*؛ غايتها تقليل عمق الجيب حتى يصبح في متناول التنظيف اليومي، وإيقاف تقدّم الفقد. والترقيع العظمي لا ينفع في كل شكل عيب — العيوب الواسعة المستوية ليست مرشّحة كالعيوب العميقة المحاطة بجدران. وليست الجراحة إجراءً تجميلياً، ولا تعالج التسوّس، ولا تبيّض الأسنان. ونتائجها تنهار إذا استمرّ التدخين أو بقي السكري غير مضبوط أو أُهملت جلسات الصيانة.

الترشيح يُحدَّد بالفحص السريري وقياس كل جيب وبالصور الشعاعية، مع مراجعة الأدوية والأمراض المزمنة، لا بقراءة صفحة ولا برأي عامّ. وفي بعض الحالات يكون القرار الأصدق هو الخلع والتعويض بدل جراحة تحاول إنقاذ سنّ ميئوس منه على حساب عظم يحتاجه التعويض لاحقاً.

## خطوات الإجراء

### 1. إعادة التقييم بعد العلاج غير الجراحي

تُعاد قياسات الجيوب ونزيف السَّبر بعد التنظيف العميق، وتُقارَن بالقياسات الأولى لتحديد المواضع التي لم تستجب، ويُقيَّم مستوى العناية المنزلية لأن الجراحة على فم غير نظيف لا تُثمر.

### 2. التخطيط الجراحي وتحضير المريض

يُختار نوع الإجراء لكل قطاع بحسب شكل العيب العظمي في الصور، وتُراجع الأدوية والأمراض المزمنة والتدخين، ويُشرح لك المتوقّع وحدوده وبديله، ثم تُوقَّع الموافقة المستنيرة.

### 3. التخدير ورفع السديلة

بتخدير موضعي يُصمَّم الشقّ على حدّ اللثة وتُرفع السديلة بلطف عن العظم، بقدر يكفي للوصول إلى قاع الجيب مع الحفاظ على أكبر قدر ممكن من نسيج اللثة والحليمات بين الأسنان.

### 4. التنظيف المباشر وإجراءات العظم

يُزال النسيج الملتهب والجير المتبقّي على الجذور بالرؤية المباشرة، ثم تُسوّى الحواف العظمية غير المنتظمة أو يُوضع طُعم عظمي وغشاء في العيوب المناسبة الشكل فقط. ولا يُوضع الطُعم في كل موضع.

### 5. الخياطة والمتابعة وجلسات الصيانة

تُعاد السديلة وتُخاط في الوضع المخطَّط له، وتُزال الخيوط بعد أسبوع إلى أسبوعين. تُقاس الجيوب من جديد بعد أشهر، ويدخل المريض برنامج صيانة كل بضعة أشهر، لأن انقطاع الصيانة هو أشهر أسباب فقد النتيجة.

## Procedure steps

### 1. Reassessment after non-surgical therapy

Pocket depths and bleeding on probing are re-measured after deep cleaning and compared with the baseline chart to identify the sites that have not responded, and home care is assessed, since surgery in a mouth that is not kept clean does not deliver.

### 2. Surgical planning and patient preparation

The type of procedure for each section is chosen according to the shape of the bony defect on radiographs, medications, chronic illnesses and smoking are reviewed, the expected outcome, its limits and the alternatives are explained, and informed consent is taken.

### 3. Anaesthesia and flap elevation

Under local anaesthesia the incision is designed along the gum margin and the flap is gently elevated off the bone, only as far as needed to reach the base of the pocket while preserving as much gum tissue and interdental papilla as possible.

### 4. Direct debridement and bone procedures

Inflamed tissue and residual calculus on the roots are removed under direct vision, then irregular bone margins are recontoured, or a bone graft and membrane are placed only in defects whose shape is suitable. Grafting is not performed at every site.

### 5. Suturing, review and maintenance

The flap is repositioned and sutured as planned, and sutures are removed after one to two weeks. Pockets are re-measured some months later and the patient enters a maintenance programme every few months, since lapsed maintenance is the commonest reason results are lost.

أعطِ الفريق قائمة كاملة بأدويتك، وخصوصاً مضادات التخثّر ومضادات تصفيح الصفيحات وأدوية هشاشة العظام والكورتيزون وأدوية المناعة، ولا توقف أيّاً منها من نفسك؛ التوقيت قرار طبيبك المعالج مع طبيب اللثة. اذكر السكري ومستوى ضبطه، وأي مرض قلبي أو صمام صناعي، وأي حساسية دوائية. أنجز جلسات التنظيف الموصوفة وأثبت عناية منزلية جيّدة قبل تحديد موعد الجراحة، فهذا يُغيّر نتيجتك أكثر من أي تفصيل تقني. توقّف عن التدخين قبل الجراحة وفي أسابيع الشفاء. تناول طعاماً خفيفاً قبل الموعد إن كان التخدير موضعياً، ورتّب من يُوصلك إن كنت ستأخذ مهدّئاً، وجهّز في المنزل أطعمة طرية وكمّادات ثلج ويومَي راحة.

## العناية بعد الإجراء

ضع كمّادات باردة على الوجه من الخارج بفواصل في اليوم الأول، ونم على وسادة مرتفعة في الليلة الأولى، وتوقّع أن يبلغ التورّم ذروته في اليومين الأولين ثم يتراجع. لا تُفرّش موضع الجراحة حتى يسمح الطبيب، ونظّف بقية الفم بشكل طبيعي، واستخدم الغسول الموصوف كما شُرح لك فقط. تجنّب المصّ بالشاليموه والأطعمة القاسية والساخنة والحارّة، ولا ترفع الشفة لتفحّص المكان ولا تشدّ الغرز بلسانك. لا تُمارس مجهوداً بدنياً عنيفاً في الأيام الأولى. امتنع عن التدخين، واضبط سكّر الدم، والتزم بمواعيد إزالة الخيوط وجلسات الصيانة اللاحقة. حساسية الجذور وظهور الأسنان أطول أمران متوقَّعان. \*\*راجع فوراً عند نزيف لا يتوقّف بالضغط بشاش نظيف عشرين دقيقة، أو تورّم يتزايد بعد اليوم الثالث مع حرارة مرتفعة أو خروج قيح، أو ألم يزداد بدل أن يخفّ، أو صعوبة في البلع أو التنفّس أو فتح الفم، أو انكشاف الغشاء أو الطُعم أو انفتاح الجرح، أو تنميل مستمرّ لا يزول.\*\*

## About this service

Periodontal surgery is a family of procedures performed on the gums and the bone supporting the teeth, for patients in whom non-surgical therapy has not brought periodontitis under control. The best known is flap surgery: the gum tissue is lifted slightly off the bone to reach the base of deep pockets, the inflamed tissue and residual calculus on the roots are removed under direct vision, and the tissue is then repositioned and sutured so the pocket becomes shallower and easier to clean.

The same family includes other procedures: recontouring bone deformed by the disease, bone grafting and guided tissue regeneration with a membrane in contained defects of suitable shape, excision of overgrown gum tissue, and specific techniques for pockets around multi-rooted molars. The choice is made at a reassessment visit after deep cleaning, based on which pockets remain deep and bleeding, how the pattern of bone loss appears radiographically, and how effective home care has become.

Surgery is usually performed under local anaesthesia, sometimes with sedation, treating one section of the mouth per session. Sutures are typically removed after one to two weeks, and swelling and discomfort peak in the first two days before settling. The gums will not return to exactly their previous appearance: expect the teeth to look longer, root sensitivity to increase temporarily, and interdental brushes to become necessary for spaces that are now wider.

The limits are explicit. Periodontal surgery does not cure periodontitis permanently and does not rebuild all the bone that has been lost. Its purpose is to reduce pocket depth to a level that daily cleaning can reach, and to stop further loss. Bone grafting does not suit every defect shape, since broad flat defects are far less favourable than deep defects with surrounding walls. It is not a cosmetic procedure, it does not treat decay, and it does not whiten teeth. Its results deteriorate if smoking continues, if diabetes stays poorly controlled, or if maintenance visits are abandoned.

Suitability is determined by clinical examination, pocket-by-pocket measurement and radiographs, together with a review of medications and chronic illnesses, not by reading a page or a general opinion. In some cases the more honest decision is extraction and replacement, rather than surgery that tries to save a hopeless tooth at the cost of bone that a future restoration will need.

## Preparation

Give the team a complete list of your medicines, in particular anticoagulants, antiplatelet drugs, bone-density medication, steroids and immune-modifying drugs, and do not stop any of them yourself; the timing is a decision for your treating physician together with the periodontist. Mention diabetes and how well controlled it is, any heart condition or prosthetic valve, and any drug allergy. Complete the prescribed cleaning appointments and demonstrate good home care before the surgery date is set, because this influences your outcome more than any technical detail. Stop smoking before surgery and through the healing weeks. Eat lightly beforehand if only local anaesthesia is planned, arrange a driver if you will be sedated, and prepare soft food, ice packs and two rest days at home.

## Aftercare

Apply cold packs to the outside of the face at intervals on the first day, sleep with your head raised on the first night, and expect swelling to peak in the first two days before easing. Do not brush the surgical site until your periodontist allows it, keep the rest of the mouth clean as usual, and use the prescribed rinse exactly as demonstrated. Avoid straws, hard, very hot and spicy food, do not lift your lip to inspect the site, and do not pull at the sutures with your tongue. Avoid strenuous physical activity in the first days. Do not smoke, keep blood sugar controlled, and attend the suture removal and later maintenance visits. Root sensitivity and teeth appearing longer are expected. \*\*Seek care immediately if bleeding does not stop after twenty minutes of pressure with clean gauze, if swelling increases after the third day with fever or pus, if pain increases instead of easing, if you have difficulty swallowing, breathing or opening your mouth, if a membrane or graft becomes exposed or the wound opens, or if numbness persists.\*\*
