## Procedure steps

### 1. Precise diagnosis first

The clinician measures mouth opening, tracks the movement path, checks joint sounds, the chewing and neck muscles and the bite, and distinguishes muscular pain from true joint pain, disc displacement and joint stiffness, because each has a completely different treatment.

### 2. A structured conservative programme

An adequate conservative plan is applied for long enough: jaw rest, soft food, compresses, exercises and physiotherapy, managing stress and grinding, and a properly fitted, monitored appliance when indicated. Most cases stop here and improve.

### 3. Imaging when there is no improvement

If the condition does not improve, appropriate imaging is requested, magnetic resonance imaging to show the disc and any effusion, or a computed tomography scan to assess bone and ankylosis, to establish whether the problem is genuinely structural.

### 4. Minimally invasive intervention

Joint lavage through fine needles may be performed to flush inflammatory mediators and release minor adhesions, or arthroscopy may be used for diagnosis and treatment together, usually under local anaesthesia with sedation or short general anaesthesia.

### 5. Open surgery as a last resort

Open surgery is reserved for defined situations: repairing or removing the disc, releasing a bony ankylosis, managing recurrent dislocation, or rarely replacing the joint. It is followed by a long exercise programme, without which the result suffers.

## About this service

Temporomandibular joint surgery covers a range of procedures directed at the joint that connects the lower jaw to the skull in front of the ear. They extend from washing the joint through fine needles and arthroscopy, up to open surgery to repair or remove the disc, release a bony ankylosis, or, rarely, replace the joint.

Before any discussion of surgery, one point must be clear: the great majority of temporomandibular joint pain improves with conservative care and never needs an operation. Conservative treatment is not passive waiting but a plan: explaining the condition, resting the jaw, a period of soft food, warm or cold compresses, exercises and physiotherapy for the jaw and neck, addressing stress and tooth grinding, and correcting habits such as prolonged gum chewing, nail biting and sleeping face down.

An important warning: splints and night guards made without a diagnosis, without proper fitting and without supervised adjustment, particularly ready made ones or those produced by an unqualified provider, can increase pain and shift the bite in ways that are hard to reverse. An appliance is one tool inside a plan built on examination, not a solution you simply buy.

Surgery is considered only in defined situations: severe persistent pain that disrupts eating and sleep and has not responded to an adequate, structured conservative programme; a genuine locked joint preventing mouth opening; bony ankylosis; repeated dislocation that cannot be controlled; or structural changes documented on imaging and arthroscopy. The decision rests on clinical examination and appropriate imaging, often magnetic resonance imaging to visualise the disc, not on symptoms described over the phone.

The limits are explicit. Surgery does not switch off muscular pain driven by clenching and grinding, and it does not stop the grinding itself. Painless clicking with normal mouth opening usually needs no treatment at all and never justifies an operation. No intervention guarantees complete pain relief, and a long programme of exercises and physiotherapy after surgery is often an essential part of the outcome.

## Preparation

Tell your clinician everything you have already tried and for how long: medicines, splints and guards, physiotherapy, injections, and describe any appliance currently in your mouth, who made it and when. Disclose all medicines and any chronic illness, arthritis or clotting disorder, and any previous anaesthetic problem. Bring earlier images, reports and any magnetic resonance imaging result. Before the visit, note down details about your pain: when it worsens, whether you wake with a tired jaw, whether there is clicking or locking, and roughly how wide you can open. These details change the diagnosis. If sedation or general anaesthesia is planned, follow fasting rules, arrange a driver, and prepare soft food at home.

## Aftercare

Keep to soft food for the period your clinician specifies, avoid chewing gum, nuts and hard bread, do not bite with the front teeth, and do not test your progress by opening your mouth as wide as possible. Use compresses as advised, sleep on your back or side without pressure on the jaw, and perform your physiotherapy exercises gradually and consistently, since they are part of the treatment rather than an optional extra. Never adjust an appliance yourself or replace it with a ready made guard. Continue treatment for stress and grinding if it is part of your plan. \*\*Seek care immediately if you suddenly cannot open or close your mouth, if there is swelling with fever in front of the ear, numbness or weakness of facial movement, a sudden change in how your teeth meet, severe increasing pain after the procedure, or clear hearing loss or discharge from the ear.\*\*
