وحدود التخصّص لا تقلّ أهمية عن قدراته: هو لا يغني عن طبيب الأسنان العام في التسوّس والحشوات وعلاج العصب، ولا يغني عن أخصائي التقويم في تحريك الأسنان، وكثير من شكاوى الفك — وأبرزها ألم المفصل الفكي الصدغي — تتحسّن بعلاج محافظ بلا أي جراحة. الجراحة تُصلح البنية العظمية والأنسجة، ولا تُصلح سبباً سلوكياً مستمرّاً مثل شدّ الفكين وصرير الأسنان.

لا يُبنى القرار على وصف الأعراض ولا على قراءة صفحة: يحتاج الطبيب فحصاً سريرياً وصوراً — بانورامية أو مقطعية ثلاثية الأبعاد — ليرى موقع الجذور وقربها من الأعصاب والجيوب، وحجم العظم المتوفّر، وحالة العضّة. الترشيح لأي عملية يُحدَّد بهذا الفحص لا بغيره.

## خطوات الإجراء

### 1. الفحص السريري وتاريخك الطبي

يفحص الطبيب الفم والأسنان والعضّة وحركة الفك والغدد اللمفاوية، ويسأل عن الأمراض المزمنة والأدوية ونزيفك السابق وحساسيتك، لأن هذه التفاصيل تغيّر خطة التخدير والإجراء نفسه.

### 2. التصوير والتشخيص

تُطلب صورة بانورامية، وعند الحاجة صورة مقطعية ثلاثية الأبعاد، لتحديد موقع الجذور والأعصاب والجيوب وحجم العظم. بعض الكتل تحتاج خزعة أو تحليل نسيجي قبل تحديد العلاج.

### 3. خطة العلاج واختيار التخدير

يشرح لك الطبيب الخيارات وما يفعله كل خيار وما لا يفعله، والبديل غير الجراحي إن وُجد، ثم يُختار التخدير: موضعي في العيادة، أو موضعي مع تهدئة، أو تخدير عام في المستشفى.

### 4. تنفيذ الإجراء

يُعمل تحت ظروف معقّمة، وقد يتطلّب فتح اللثة أو إزالة قدر من العظم أو تجزئة السنّ أو تثبيت العظم بصفائح وبراغي، وينتهي عادة بغسل الموضع وخياطته بقطب تذوب أو تُزال لاحقاً.

### 5. الالتئام والمتابعة

تُشرح لك تعليمات الأيام الأولى وعلامات الخطر، وتُحدَّد مراجعة لمتابعة الالتئام وإزالة القطب، ومراجعات أطول بالصور في حالات الأكياس والكسور وزراعة العظم.

## Procedure steps

### 1. Clinical examination and medical history

The surgeon examines the mouth, teeth, bite, jaw movement and lymph nodes, and asks about chronic illnesses, medications, previous bleeding problems and allergies, because these details change both the anaesthetic plan and the procedure itself.

### 2. Imaging and diagnosis

A panoramic radiograph is taken, and where needed a three dimensional scan, to map roots, nerves, sinuses and bone volume. Some lesions require a biopsy or histopathology before the treatment plan can be finalised.

### 3. Treatment plan and anaesthesia choice

Options are explained together with what each one can and cannot achieve, and any non surgical alternative. Anaesthesia is then chosen: local in the clinic, local with sedation, or general anaesthesia in hospital.

### 4. Performing the procedure

The procedure is carried out under sterile conditions and may involve raising the gum, removing a small amount of bone, sectioning a tooth, or fixing bone with plates and screws. It usually ends with irrigation and sutures that either dissolve or are removed later.

### 5. Healing and follow-up

You receive instructions for the first days along with warning signs, and a review appointment is arranged to check healing and remove sutures. Longer radiographic follow-up is scheduled for cysts, fractures and bone grafts.

## التحضير قبل الإجراء

أخبر الطبيب بكل دوائك حتى ما تعتبره بسيطاً، وخصّ بالذكر مسيّلات الدم ومضادات التخثّر وأدوية هشاشة العظام ومثبّطات المناعة والكورتيزون، ولا توقف أي دواء من نفسك. أخبره إن كنتِ حاملاً أو مرضعاً، وإن كان لديك سكري أو ضغط أو مشكلة في القلب أو صمام صناعي أو مرض في تخثّر الدم. أحضر صورك وتقاريرك السابقة، والتزم بالصيام المطلوب إن كان التخدير عامّاً أو مع تهدئة، ورتّب من يوصّلك ويعيدك. نظّف أسنانك جيداً في الأيام السابقة، وأوقف التدخين قبل الإجراء وبعده لأنه يؤخّر الالتئام، وارتدِ ملابس مريحة وتجنّب المكياج الثقيل والعدسات اللاصقة يوم العملية.

## العناية بعد الإجراء

استرح بقية اليوم، وضع كمادات باردة على الوجه في الساعات الأولى، ونم برأس مرفوع قليلاً. تناول طعاماً طرياً وبارداً أو فاتراً، واشرب كثيراً من السوائل بلا شفّاطة، وتجنّب التدخين والمضمضة العنيفة والبصق والمجهود العنيف في أول يومين. نظّف بقية أسنانك بلطف واستمرّ بما وصفه لك الطبيب من غسول أو ملح دافئ بعد اليوم الأول. \*\*راجع فوراً عند نزيف لا يتوقف بالضغط المتواصل، أو تورّم يزيد بعد اليوم الثالث مع حرارة، أو صعوبة في البلع أو التنفّس أو فتح الفم، أو تنميل مستمرّ في الشفة أو اللسان، أو ألم يسوء بدلاً من أن يخفّ بعد اليوم الثالث، أو خروج قيح أو رائحة كريهة.\*\*

## About this service

Oral and maxillofacial surgery is a surgical field that sits between dentistry and general surgery. It manages diseases, injuries and deformities of the mouth, teeth, jaws, facial bones and the temporomandibular joint, ranging from removing a single impacted tooth under local anaesthesia in a clinic chair to fixing facial fractures or repositioning the jaws in an operating theatre.

The most common reasons for referral are impacted wisdom teeth and retained roots, cysts and lesions inside the jaw bone, deep infections of the mouth and jaw, fractures of the mandible, cheekbone and orbit, insufficient bone before dental implants, a protruding or receding jaw, persistent temporomandibular joint pain, and congenital conditions such as cleft lip and palate.

Its limits matter as much as its capabilities. This specialty does not replace your general dentist for decay, fillings and root canal treatment, and it does not replace an orthodontist for moving teeth. Many jaw complaints, above all temporomandibular joint pain, improve with conservative care and never need an operation. Surgery corrects bone and tissue structure; it does not correct an ongoing behavioural cause such as clenching and grinding.

No decision is made from a description of symptoms or from reading a web page. The surgeon needs a clinical examination plus imaging, a panoramic film or a three dimensional scan, to see where roots sit relative to nerves and sinuses, how much bone is available, and how the bite closes. Candidacy is determined by that assessment and nothing else.

Before agreeing to any procedure, ask four questions: is there a non surgical alternative, what type of anaesthesia is planned, how long is the recovery and how will it affect your work, and which complications are most likely in your particular case. Clear answers are a good sign that you are in the right hands.

## Preparation

Tell your surgeon about every medicine you take, even those you consider minor, and mention blood thinners, anticoagulants, bone density medicines, immunosuppressants and steroids in particular. Never stop a medicine on your own. Say if you are pregnant or breastfeeding, and disclose diabetes, high blood pressure, heart disease, an artificial valve or any bleeding disorder. Bring previous images and reports, follow the fasting instructions if sedation or general anaesthesia is planned, and arrange someone to drive you home. Keep your mouth as clean as possible in the days before, stop smoking before and after because it delays healing, wear comfortable clothing, and avoid heavy make-up and contact lenses on the day.

## Aftercare

Rest for the remainder of the day, apply cold packs to the face during the first hours, and sleep with your head slightly raised. Eat soft, cool or lukewarm food, drink plenty of fluids without a straw, and avoid smoking, vigorous rinsing, spitting and strenuous effort for the first two days. Clean your remaining teeth gently and continue any rinse or warm salt water your surgeon advised after the first day. \*\*Seek care immediately if bleeding does not stop with sustained pressure, if swelling increases after the third day together with fever, if you struggle to swallow, breathe or open your mouth, if numbness in the lip or tongue persists, if pain worsens instead of easing after day three, or if there is pus or a foul smell.\*\*
