## Procedure steps

### 1. Reviewing the congenital history and old records

Everything available is gathered: operative reports from childhood surgery and the names of the procedures performed, earlier echocardiograms, and previous medications. These records change the plan substantially, so bring them however old they appear.

### 2. Advanced assessment with echo, MRI and catheterisation

Detailed echocardiography, cardiac MRI to quantify ventricular volumes and shunt fraction, exercise testing, catheterisation to measure pulmonary pressure and resistance where needed, and rhythm recording. These measurements determine whether intervention will actually help.

### 3. Team decision: surgery, catheter intervention or surveillance

The adult congenital cardiologist meets with the cardiac surgeon, an electrophysiologist and the anaesthetist, and the surgical option is compared with catheter closure or dilatation and with medical surveillance. Risks, alternatives and timing are explained to you individually before consent.

### 4. Surgery or reoperation

The operation is performed under general anaesthesia, usually with cardiopulmonary bypass. In reoperations it may be planned with prior computed tomography to avoid adhesions and vessels lying close to the breastbone. The steps differ greatly by defect: closing a shunt, repairing a valve, replacing a conduit or enlarging an outflow pathway.

### 5. Lifelong follow-up and life planning

Regular appointments with an adult congenital cardiologist including echocardiography and rhythm monitoring, discussion of pregnancy, sport, travel and anaesthesia for any other operation, endocarditis prevention for those who need it, and an up-to-date medical summary you carry at all times.

## About this service

Adult congenital heart surgery treats structural heart defects a patient was born with that have persisted into adult life. It covers two groups. The first is defects never identified in childhood that present late, such as an atrial septal defect, a bicuspid aortic valve, or a localised narrowing of the aorta. The second is patients operated on as children who need a further intervention years later, for residual valve regurgitation, narrowing of a surgical conduit, or replacement of a conduit that the patient has outgrown.

The most important message in this field is that the phrase completely cured in childhood is inaccurate for most defects. Repair improves anatomy and function, but it does not remove the need for regular lifelong follow-up with a specialist in adult congenital heart disease. Late problems can progress quietly, including rhythm disturbance, valve regurgitation, raised pulmonary artery pressure, and dilatation of a ventricle or the aorta, long before the patient feels anything, which is why they are found by surveillance rather than by symptoms.

Assessment here is broader than in other cardiac fields: detailed echocardiography, cardiac magnetic resonance imaging to measure ventricular volumes, flow and shunt fractions, diagnostic catheterisation in specific cases to measure pulmonary artery pressure and resistance, exercise testing, and rhythm monitoring. The decision is made within a multidisciplinary team that always weighs surgery against catheter intervention and medical surveillance, because not every adult defect requires an operation.

The limits are explicit. Surgery corrects the anatomy, but it does not restore heart muscle or pulmonary vessels damaged by years of overload. If pulmonary hypertension has progressed beyond a certain stage, closing a defect may become unhelpful or even harmful, and that is decided by measurement rather than preference. Surgery also does not remove the risk of arrhythmia or the need some patients have for medication or a device, it does not end follow-up, and it does not make every conduit permanent, as some require later replacement.

Adults face particular issues that should be planned ahead. Pregnancy and delivery need pre-conception counselling, because some defects make pregnancy high risk. Permitted sport is decided individually according to anatomy and cardiac function. Endocarditis prevention in some patients means informing the dentist before any procedure. And travel or anaesthesia for any other operation requires accurate information about your anatomy, so always carry an up-to-date medical report.

## Preparation

\*\*Do not stop any anticoagulant, antiplatelet drug or cardiac medication on your own; the timing is a joint decision of your cardiologist and surgeon.\*\* Bring all your old records: the childhood operative report, the names of the procedures performed, and echocardiogram and MRI discs, as these are the most valuable things you can give the team. Report any rhythm disturbance or previous fainting, any implanted device, any past endocarditis, all your medicines and supplements, and your drug allergies. Have a dental examination and treat any infection before the date. If you are considering pregnancy, discuss it before surgery rather than after. Stop smoking, practise breathing exercises, fast according to the anaesthetist's instructions, and bring a companion and a written medication list.

## Aftercare

Protect your breastbone for several weeks as your surgeon directs: do not lift, push or pull, do not push up on your arms to stand, brace your chest when coughing or sneezing, and do not drive until cleared. Walk daily with increasing distance, record your weight, and inspect and clean the wound as instructed. Take all your medicines on time and stop none of them yourself, and never skip an echocardiogram or rhythm monitoring appointment even when you feel entirely well, because most late congenital heart problems are detected by surveillance before symptoms appear. Inform your dentist and any other doctor about your cardiac anatomy before any procedure, and carry an up-to-date summary. Discuss sport, pregnancy and travel with your team rather than with the internet. \*\*Go to emergency care immediately for fainting or severe dizziness on exertion, a fast irregular heartbeat, increasing blue discolouration of the lips or extremities, new breathlessness or breathlessness lying flat, coughing up blood, persistent fever with night sweats after any valve or conduit procedure, abdominal and ankle swelling with rapid weight gain, or sudden limb weakness or slurred speech.\*\*
