## Procedure steps

### 1. Assessing perfusion and mapping the blockage

The surgeon feels the pulses in both limbs and measures the ankle-to-arm pressure ratio, then adds duplex ultrasound or CT angiography to define the length and site of the blockage and the state of the arteries below it. The heart, lungs and kidneys are also assessed before any surgical decision is taken.

### 2. Anaesthesia and preparing the conduit

The operation is carried out under general or regional anaesthesia, depending on your condition and the anaesthetist's judgement. If a vein from your own leg is to be used, it is assessed and harvested through limited incisions and checked for length and calibre. If it is unsuitable, a synthetic tube is used after the difference has been explained to you.

### 3. Exposing the artery above and below the blockage

The surgeon opens two sites, one above and one below the blockage, and frees the artery at each while protecting the neighbouring nerves and lymphatic channels. An anticoagulant is given during the operation before the flow is temporarily interrupted with clamps.

### 4. Bypass or endarterectomy, then checking flow

The surgeon joins the two ends of the graft to the artery above and below the blockage with fine sutures, or opens the artery, clears the plaque and patches the wall. After the clamps are released, the return of pulse and flow is confirmed by palpation and by ultrasound or on-table imaging, and any kink or residual narrowing is corrected before closure.

### 5. Closure, monitoring and the walking programme

The wounds are closed in layers, and the colour, warmth and pulses of the limb are checked regularly during the first hours. Mobilisation starts as early as your condition allows, to prevent venous clots, and is followed by a graded walking programme together with a clear plan for smoking, blood sugar, cholesterol and blood pressure, and scheduled imaging surveillance of the graft.

## About this service

Surgery for peripheral artery disease is a group of open operations that restore blood flow to a limb whose artery has been narrowed or blocked by atherosclerotic plaque. The best known is bypass grafting, in which the patient's own vein or a synthetic tube carries blood from a healthy point above the blockage to a healthy point below it. The other is endarterectomy, in which the plaque is cleared from inside the artery when the diseased segment is short and suitably placed.

Surgery is considered for people whose walking is limited by muscle pain that interferes with daily life despite a structured walking programme and medical treatment, and for people who have reached limb-threatening ischaemia: pain at rest that wakes them at night, a wound that will not heal, or darkening of the toes. The surgical option is always weighed against catheter treatment with a balloon and a stent. The choice depends on the length and site of the blockage, the state of the arteries below it, whether the heart and lungs can tolerate an operation, and whether a suitable vein is available.

The limits of surgery must be stated plainly. A bypass goes around a blockage; it does not cure atherosclerosis. The rest of the arterial tree — heart, brain and kidneys — remains exposed to the same disease, and the graft itself may narrow or clot later on. Smoking is the single strongest modifiable factor in arterial disease, and continuing to smoke after surgery shortens the life of a graft more than anything else. Surgery also cannot bring back tissue that has already died, and it does not repair diabetic nerve damage.

If you have diabetes, remember that the absence of pain is not proof of safety: nerve damage can hide a wound or poor circulation until it is advanced, which is why regular foot checks are non-negotiable. Suitability for surgery is decided by clinical examination, limb pressure measurement and vascular imaging — not by reading.

## Preparation

Stop smoking before surgery and ask for real help to do it — this is the single most powerful decision in your hands, and it affects the outcome more than any medicine. Bring a written list of all your medicines and tell the surgeon explicitly about blood thinners, antiplatelet drugs and diabetes treatment, then follow the written instructions on stopping or continuing them rather than guessing. Get your blood sugar, blood pressure and cholesterol under control. Complete the requested heart, lung and kidney tests, and report any new chest pain or breathlessness. Keep your feet and nails clean, and do not cut your own nails if your sensation is reduced. Fast for the period the anaesthetic team specifies, and prepare your home for recovery: wide comfortable shoes, a sleeping place that avoids stairs, and someone to help you in the first days.

## Aftercare

Expect pain and swelling around the wounds that settle gradually. Get moving as early as your team allows, breathe deeply, and walk short distances often, increasing them step by step according to your programme. Keep the wounds clean and dry and put nothing of your own on them. Do not warm the foot with hot water or a hot bottle, never walk barefoot, and inspect both feet daily, using a mirror if you cannot see the soles. If you have diabetes, remember that \*\*the absence of pain does not mean the foot is safe\*\*, because nerve damage hides wounds. Do not stop antiplatelet or cholesterol medicines on your own.

Seek care immediately if: \*\*the limb becomes cold, pale, painful and pulseless\*\* (possible acute graft occlusion — here hours decide whether the limb survives) · a wound bleeds, opens further, discharges pus or smells · you develop fever or shivering · redness spreads up the leg · a toe develops new dark discolouration · \*\*a leg suddenly becomes painful, swollen and warm\*\* (possible deep vein thrombosis — do not massage the leg and do not wait) · \*\*chest pain or breathlessness\*\* follows a leg clot (possible pulmonary embolism — go to the emergency department at once).
