## Procedure steps

### 1. Screening and suitability interview

Before joining, an individual interview assesses your problem, its severity, your goals and how much you would gain from a group of this composition, and screens out situations that need individual work or medical assessment first, such as risk to life or unstable symptoms. The method, duration and number of meetings are explained.

### 2. Agreeing the group contract

At the first meeting the rules are set out clearly: confidentiality and its shared responsibility, regular attendance and timekeeping, respect for each member's turn, your right to stay silent or decline to answer, no discussion of members outside the room, and no financial or private relationships between them. The legal exceptions to confidentiality are also explained.

### 3. How a session runs

A session opens with a brief review of the week's tasks, moves to a defined topic or skill, and then to interactive work: sharing an experience, role-playing a situation, practising a skill, and guided feedback that the therapist regulates so the discussion stays constructive, safe and fairly distributed across meetings.

### 4. Practice and between-session tasks

What is learned in the room is applied outside it: trying a situation you used to avoid, using a communication skill with a specific person, recording situations and reactions. The results are reviewed at the next meeting, and this cycle of practice and application is what turns a session from a conversation into treatment.

### 5. Individual review and ending

At intervals a short individual review measures your progress and allows discussion of what you prefer not to raise in front of the group; parallel individual therapy or referral to a physician may be suggested at that point. At the end, a maintenance plan is built: skills to keep using, early warning signs, and specific steps to take if symptoms return.

## About this service

A group therapy session is a structured therapeutic meeting led by a qualified therapist and attended by a limited number of participants — usually six to ten — who share a problem or treatment goal and work on it using a recognised method and clear ground rules, rather than in an open-ended conversation.

Groups are used for social and generalised anxiety, depression, anger management, grief and loss, chronic pain and chronic illness, communication and problem-solving skills, parenting groups for managing children's behaviour, and aftercare groups that support maintained change. Their power comes from ingredients an individual session cannot supply: discovering that you are not alone, learning from someone a step ahead of you, receiving honest feedback from peers, and practising a skill in front of real people rather than a single therapist.

Patients need to understand the limits of the format. A group is not a place to disclose every private detail, not an arena for mutual advice, and not a substitute for individual therapy during an acute crisis, when there is risk to your life, with unstable psychotic symptoms, or for a condition that requires in-depth individual work. In those situations individual sessions come first and a group may be added later. Speaking time is not divided exactly equally in every meeting, which is deliberate, since the work accumulates across sessions. Confidentiality in a group is also a shared responsibility: the therapist is professionally bound, participants pledge to it, but it cannot be guaranteed to the same degree as in an individual session — which is why you retain the right to choose what you share and when.

A common myth holds that group therapy is a cheaper, lower-quality option for those who do not merit individual sessions. In reality, for some problems — social anxiety and communication skills in particular — it is the preferred treatment, because practising within a group is itself the active ingredient. Another myth is that hearing other people's problems adds to your burden; far more often it reduces isolation and reveals solutions that were not visible before.

Before joining, an individual screening interview confirms that the group suits you and that you suit the group, and the rules are explained: regular attendance, timekeeping, respecting others' turns, not repeating outside what is said inside, and avoiding financial or private relationships between members outside sessions. Keeping these rules is what makes the room safe enough for people to change in it.

## Preparation

Before joining, ask: what is the group's topic and method, how many members, how long each meeting lasts and how many there are, who leads it and with what qualifications, and whether there is an individual screening interview. Decide on one clear goal you want from the group and write it down. Decide in advance the limits of what you wish to share; you are entitled to start by listening and to open up gradually, and you are never required to disclose everything. Respect the timing and arrive a little early, since lateness disrupts the group's focus. Tell the therapist beforehand about any medical or medication issue that could affect your participation, and never stop prescribed medication on your own. If the group meets remotely, choose a closed room, use headphones, and do not record the session or photograph anyone — recording breaches other people's confidentiality. Commit to the core rule: what is said in the room stays in the room, and you do not repeat members' names or details to anyone outside it, not even to someone you trust.

## Aftercare

\*\*If you have thoughts of harming yourself or ending your life, or feel you may lose control, this needs urgent help immediately: do not wait for the next meeting — contact the group therapist, go to the nearest emergency department, or ask someone you trust to stay with you, and do not remain alone.\*\* Seek urgent help too if you have thoughts of harming others, become severely confused or start having unfamiliar thoughts or hearing voices, or stop eating and sleeping for days. After each meeting: feeling emotionally stirred or tired is normal, especially when someone's story touches your own experience; allow quiet time afterwards and write down what you gained and what you want to raise next time. Do the agreed tasks — they are the main engine of change. Protect other members' confidentiality absolutely and never pass on their names or details. Do not compare your pace with others, since paths differ; and if the group does not feel right for you or something said in it caused harm, tell the therapist openly rather than dropping out silently — the plan may be adjusted or individual therapy added. Never stop prescribed medication on your own because you feel better in the group.
