## Procedure steps

### 1. Initial assessment

Your psychological, medical and social history is gathered; symptoms are described along with their onset and what worsens or eases them; and you are asked about sleep, appetite, concentration, work, relationships and any thoughts of harming yourself. Brief validated scales establish a baseline, and the need for parallel medical assessment is considered.

### 2. Formulation and goal setting

The therapist explains, in plain language, their understanding of what keeps the problem going, and together you agree specific measurable goals — sleeping better, returning to work, facing a situation you avoid — instead of a vague wish to feel better. What will be measured, how, and when the plan will be reviewed are all defined.

### 3. Therapeutic contract

The chosen method and the reason for it are explained, together with the expected number, frequency and length of sessions, the cancellation and lateness policy, the limits of contact between sessions, and confidentiality with its exceptions. You are entitled to ask about the therapist's qualifications and experience with your problem and to decline any part you are uncomfortable with.

### 4. Work inside and between sessions

Sessions involve learning skills and applying them: graded confrontation of what you avoid, training in emotion regulation and relaxation, processing a painful experience in safe steps, and practical problem-solving. Tasks are assigned between sessions because change happens in your daily life, not only in the consulting room.

### 5. Measuring progress and ending

Scales are repeated periodically for an objective comparison with baseline, and the plan is reviewed if progress stalls, with the method adjusted or a referral made. As you improve, a relapse-prevention plan is built that names your early warning signs and what to do about them, and the final sessions are spaced further apart.

## About this service

An individual psychotherapy session is a structured meeting between you and a qualified therapist, usually lasting forty-five to sixty minutes, in which a defined problem is worked on using a recognised therapeutic method with explicit goals and a written plan — not an open-ended conversation without direction.

Individual sessions are used for anxiety disorders, obsessive-compulsive difficulties and phobias, depression, post-traumatic reactions, panic attacks, sleep problems, anger and emotional control, grief and loss, work and relationship difficulties, and coping with chronic illness or persistent pain. The first sessions gather your history, measure symptom severity with validated scales, and build a formulation of why the problem persists; a suitable method is then chosen and measurable goals, an approximate duration and a way of tracking progress are agreed. Suitability and choice of method are decided by clinical assessment, not by reading online.

You are entitled to know what a session does not do. It is not simply venting, ready-made advice, or judgement of other people. It does not erase a painful memory and it does not change other people — it changes your relationship with what happens and how you respond to it. It does not deliver results in a single visit; most people need several sessions before a tangible shift. A psychologist who is not a physician does not prescribe medication, so if something calls for medical or pharmacological assessment you are referred to the appropriate doctor; combining psychotherapy with medical follow-up is routine, not a failure.

Confidentiality is the foundation of the work. What is said in the room stays there, with exceptions explained to you from the outset: a serious risk to your life or someone else's, abuse of a child or a vulnerable adult, or a court order. The most damaging myth is that seeking psychological help is weakness, stigma or evidence of madness. In reality, delay is what lengthens treatment and raises its cost, and most people who attend are healthy individuals facing a pressure or a symptom that can be worked on.

The real work happens between sessions: exercises, recording thoughts and situations, graded exposure to what you avoid, and adjustments to sleep and activity. Those who complete these tasks progress faster; those who only attend tend to stay where they are. If you feel no progress after a reasonable number of sessions, say so openly — reviewing the plan, changing the method or referring you to another specialist is a normal part of good care.

## Preparation

Before the first session, write down three things: exactly what troubles you, when it started, and what you want to change in your daily life. Bring a list of your current medication and any medical reports, since some psychological symptoms have physical causes that must be excluded. If you can, log a week of your sleep, mood and distressing situations beforehand — it saves time and sharpens the assessment. Arrive with enough time for a full session without rushing; if the session is remote, choose a quiet, private, closed room and use headphones to protect your privacy. Ask from the outset about the therapist's qualifications, the method they will use, the expected duration, and confidentiality and its limits. Do not expect a solution in the first session, which is mainly for assessment, and be candid even about embarrassing details, because what is not said cannot be treated. If you take prescribed medication, do not stop it on your own because psychotherapy has started; discuss any change with the doctor who prescribed it.

## Aftercare

\*\*If you have thoughts of harming yourself or ending your life, or feel you may lose control, this needs urgent help immediately: do not wait for the next session — contact your therapist, go to the nearest emergency department, or ask someone you trust to stay with you, and do not remain alone.\*\* Seek urgent help too if you have thoughts of harming another person, become severely confused or start hearing voices or having unfamiliar thoughts, stop eating and drinking, or cannot care for a child or a dependent person. After each session: feeling tired or emotionally sensitive for a few hours is normal, especially when a painful topic has been worked on; keep some quiet time afterwards and avoid scheduling a demanding task immediately. Write down what you took from the session in your own words, and do the agreed tasks consistently — they are the largest source of change. Keep regular sleep and daily activity, reduce evening caffeine, and avoid alcohol or substances as a way of calming down, since they worsen symptoms. If symptoms increase or the approach does not feel right for you, tell your therapist honestly instead of dropping out, and never stop prescribed medication on your own.
