Facial assessment and realistic goal setting The doctor assesses the pattern of volume loss and the degree of laxity, explains that the result develops gradually over months and that Sculptra is not an instant filler, and rules out anyone seeking a rapid change before an imminent event. ### 2. Reconstitution and dilution The product is reconstituted, diluted and left for the required time before use according to the manufacturer's instructions, since inadequate preparation raises the risk of nodules. This is a safety step that cannot be shortened. ### 3. Disinfection and mapping the injection sites The face is cleansed and disinfected and the treatment zones and target depth are mapped, deliberately avoiding areas where the product is not used such as the lips and the periorbital region, with topical or local anaesthesia as needed. ### 4. Deep-plane injection The product is injected with a fine needle or a cannula into the deep subcutaneous plane in evenly distributed amounts, using aspiration and continuous movement to reduce local pooling that could later form bumps. ### 5. It is placed in a deep layer of the skin, where it produces a controlled local response that prompts fibroblasts to build new collagen over weeks and months. The injected material itself is then broken down, and the collagen the body has produced is what accounts for the result. This difference explains its distinctive behaviour: there is no instant before-and-after. What you see immediately after a session is swelling from the fluid used to dilute the product, which settles within two to three days before gradual build-up begins. For that reason it is typically used for diffuse age-related volume loss in areas such as the temples, cheeks, jaw angles and upper face, and to improve skin quality over broad zones — not to sculpt precise contours in a single visit. Its limits should be stated plainly. Sculptra gives no immediate result and is unsuitable for anyone who wants a change before an event in the coming days. It is not used in the lips and is not advised in the delicate area around the eyes because of the risk of nodules. It does not tighten established sagging and is no substitute for surgical lifting. And unlike hyaluronic acid fillers it cannot simply be dissolved, because no reversal agent exists for it. This makes the injector's experience and correct deep placement the single most important safety factor. Among its more notable possible complications are delayed nodules or bumps beneath the skin; their likelihood is reduced by correct dilution and preparation, injection at the appropriate depth, and massage as the doctor advises. The procedure is postponed when there is active infection or inflammation in the area, active autoimmune disease or a tendency to keloid scarring, and pregnancy and breastfeeding are discussed. Suitability is decided by clinical examination, not on request. ## Preparation Tell the doctor about all your medications and supplements, especially anticoagulants, antiplatelet agents and supplements that increase bruising, and do not stop a prescribed medicine on your own. Report any active autoimmune disease, any history of keloid or thickened scarring, any active infection including dental or sinus infection, pregnancy or breastfeeding, and any previous injection in the same area — particularly permanent or unidentified products. Use gentle cool compresses for swelling on the first day, avoid high heat, sauna and steam for the first week, avoid strenuous exercise and alcohol for the first two days, and do not sleep on your face on the first night.
