It is the step that establishes the nature of the tissue definitively; imaging — mammography, ultrasound or MRI — describes and estimates probability but does not settle the question. For that reason no treatment decision about the breast, and no plan for surgery or medication, is made before a clear pathology report is available. The technique is chosen according to the size, position and imaging appearance of the lesion: fine-needle aspiration to draw fluid or cells; core biopsy with a wider needle that removes a cylinder of tissue, the most commonly used because it gives a full structural diagnosis; and vacuum-assisted biopsy for fine calcifications. Guidance may be by ultrasound, mammography or MRI, whichever shows the area best. A tiny metallic marker is usually left inside the biopsy site so the location can be identified precisely if surgery or further treatment is needed later; it is small and safe. The procedure is usually done under local anaesthesia in a clinic or radiology unit, typically takes 15 to 30 minutes with a short stay for pressure and dressing, and the sample is then sent to the laboratory. It is normal for the wait for results to feel heavier than the procedure itself; anxiety during these days is an understandable reaction, not a weakness. Bring someone with you to the results appointment, hear the report from your doctor rather than trying to interpret a printout alone, and do not search for your prognosis in online groups. Psychological support at this stage is part of care rather than a luxury, and you are entitled to ask for it. The limits, stated plainly: a biopsy is **diagnostic, not therapeutic**. It does not remove or treat the lump, except in limited situations where a small lesion is excised by vacuum device, and nobody can promise you a result before the report is issued. Correcting a dangerous and widespread myth: **a needle does not spread cancer or stir it up**, and postponing a biopsy because of that belief delays diagnosis and causes real harm. A benign result is also not fully reassuring when imaging is strongly suspicious; in that case the biopsy is repeated or performed surgically. The specimen may need additional tissue tests that take further days. Finally, the decision to biopsy is not measured against a relative's or friend's experience. Both the choice of technique and the need for a biopsy at all are determined by clinical examination together with the imaging. If the result requires a next step, the plan is usually discussed by a multidisciplinary team rather than decided by one person. ## Preparation Tell your doctor if you take blood thinners, antiplatelet medicines, or supplements and herbs that increase bleeding, and do not stop any of them on your own; ask about timing instead. Report allergies to local anaesthetic, dressings or metals, pregnancy or breastfeeding, and any pacemaker if MRI is planned. Bring all previous breast imaging on discs rather than the reports alone, because comparison with older studies is essential. Do not apply deodorant, powder or cream to the breast and underarm on the day of a mammogram. Wear a separate top and a comfortable supportive bra, arrange for someone to come with you because anxiety is normal, and do not fast unless deeper sedation has been requested. Write your questions down beforehand. ## Aftercare Apply cold packs intermittently over the first 24 hours, wear a supportive bra because it reduces pain, and take only the painkiller your doctor prescribed. Avoid vigorous exercise, heavy lifting, hot baths and swimming for one to two days, and keep the dressing dry as instructed. Expect bruising, swelling and mild pain that ease over a few days, with a firm area that may persist for weeks.
