لكن الصورة الكاملة تقتضي وضوحاً: تراجعت هذه العملية عالمياً خلال العقد الأخير لأنّ \*\*نزول الوزن معها أقل في المتوسط وأقل ثباتاً على المدى الطويل\*\* مقارنة بالتكميم أو تحويل المسار، ولأنّ نسبة لا يُستهان بها من المرضى يحتاجون لاحقاً إزالة الحلقة أو تحويلها إلى عملية أخرى. كما أنّها تعتمد اعتماداً كبيراً على انتظام زيارات الضبط والتزام غذائي دقيق، ولها مضاعفات خاصة بها: انزلاق الحلقة، توسّع الجيب أو المريء، تعرية الحلقة نحو داخل المعدة، ومشاكل في المنفذ أو أنبوبه.

وما لا تفعله العملية: لا تحرق دهوناً موضعية ولا تشدّ الجلد المترهّل، ولا تُعالج نهم الأكل أو الأكل العاطفي بلا دعم نفسي وسلوكي، ولا تناسب من لديه ارتجاع شديد أو فتق حجابي كبير أو مشاكل في حركة المريء، ولا تُغني عن تغيير نمط الطعام والنشاط. والطعام الطري عالي السعرات كالمثلجات والعصائر يمرّ عبر الحلقة بسهولة، فيمكن أن يفشل الوزن في النزول بلا أي خلل في الجهاز.

الترشيح يُحدَّد بتقييم متعدد التخصصات: جرّاح سمنة، أخصائي تغذية، تقييم نفسي، وأحياناً غدد صمّ وأمراض جهاز هضمي، مع حساب مؤشر كتلة الجسم ومراجعة الأمراض المصاحبة وتنظير علوي للمعدة. الفحص السريري وحده هو ما يُقرّر مناسبة هذه العملية دون غيرها، لا مقارنة بين تجارب الآخرين.

## خطوات الإجراء

### 1. التقييم متعدد التخصصات

يُحسب مؤشر كتلة الجسم وتُراجع الأمراض المصاحبة كالسكري والضغط وانقطاع النفس النومي، وتُجرى تحاليل وتنظير علوي وتقييم تغذوي ونفسي. تُشرح البدائل بصدق: التكميم وتحويل المسار والخيارات التنظيرية والدوائية، ولماذا قد تكون الحلقة أو لا تكون الأنسب لحالتك.

### 2. التخدير والدخول بالمنظار

تُجرى العملية تحت تخدير عام عبر بضعة شقوق صغيرة في البطن، ويُنفخ التجويف بغاز ثاني أكسيد الكربون لإتاحة الرؤية. يُدخل المنظار والأدوات الدقيقة، ويُفحص أعلى المعدة والحجاب الحاجز بحثاً عن فتق يستدعي إصلاحاً في الوقت نفسه.

### 3. وضع الحلقة حول أعلى المعدة

يُفتح نفق ضيّق خلف الجزء الأعلى من المعدة تحت الوصلة المريئية بقليل، ثم تُمرّر الحلقة حول المعدة وتُغلق بحجم مبدئي محسوب. الموضع الدقيق مهم: الحلقة العالية جداً أو المنخفضة جداً تزيد خطر الانزلاق ومشاكل البلع.

### 4. التثبيت ووضع المنفذ

تُثبَّت الحلقة بخياطة معدية على نفسها لتقليل احتمال الانزلاق، ويُوصل أنبوبها بمنفذ صغير يُزرع على العضلة تحت جلد البطن ويُخاط في مكانه. عبر هذا المنفذ ستُجرى تعديلات الحلقة لاحقاً بإبرة في العيادة.

### 5. الإغلاق ثم التعديلات التدريجية

تُغلق الشقوق ويُراقب المريض، ويخرج عادةً في اليوم نفسه أو بعد ليلة واحدة. تبدأ التعديلات بعد أسابيع من الشفاء وتُكرّر بحسب الشبع ونزول الوزن وقدرة البلع؛ ومن دون هذه المتابعة المنتظمة لا تعمل الحلقة كما يُفترض.

## Procedure steps

### 1. Multidisciplinary assessment

Body mass index is calculated, associated conditions such as diabetes, hypertension and sleep apnoea are reviewed, and blood tests, an upper endoscopy and nutritional and psychological assessments are completed. Alternatives are explained honestly — sleeve, bypass, endoscopic and medical options — and why a band may or may not suit your case.

### 2. Anaesthesia and laparoscopic access

The operation is performed under general anaesthesia through a few small abdominal incisions, with carbon dioxide insufflation to create working space. The laparoscope and fine instruments are introduced and the upper stomach and diaphragm are inspected for a hernia that may need repair at the same time.

### 3. Placing the band around the upper stomach

A narrow tunnel is created behind the upper stomach a little below the oesophageal junction, then the band is passed around the stomach and closed at a calculated initial size. Exact positioning matters: a band placed too high or too low increases the risk of slippage and swallowing problems.

### 4. Fixation and port placement

The band is secured with a gastro-gastric stitch to reduce the chance of slippage, and its tubing is connected to a small port implanted on the abdominal muscle beneath the skin and sutured in place. Later adjustments are made through this port with a needle in clinic.

### 5. Closure and staged adjustments

The incisions are closed and the patient is monitored, usually going home the same day or after one night. Adjustments begin a few weeks after healing and are repeated according to satiety, weight loss and swallowing comfort; without this regular follow-up the band cannot work as intended.

## التحضير قبل الإجراء

أخبر الفريق بكل أدويتك ومكمّلاتك، خصوصاً مسيّلات الدم وحبوب منع الحمل وأدوية السكري وأدوية الحرقة، ولا تُعدّل جرعة من نفسك — ضبط أدوية السكري قبل الصيام يجريه طبيبك. اذكر أي حساسية دوائية وأي جراحة بطن سابقة وأي تجربة سيئة مع التخدير. التزم بالحمية التحضيرية التي يصفها أخصائي التغذية إن طُلبت، فهي تُصغّر الكبد وتُسهّل العمل بالمنظار. أوقف التدخين قبل العملية وبعدها لأنه يزيد مضاعفات التنفّس والتقرّح. أكمل التنظير والتحاليل والتقييم النفسي والتغذوي وأحضر تقاريرها. التزم بالصيام حسب تعليمات طبيب التخدير. رتّب مرافقاً وإجازة أسبوع إلى أسبوعين، واعرف مسبقاً جدول مواعيد الضبط لأنه جزء أساسي من العلاج لا اختيار.

## العناية بعد الإجراء

التزم بالتدرّج الغذائي الذي يضعه أخصائي التغذية: سوائل صافية ثم كاملة، فمهروس، فطعام لين، ثم صلب على مدى أسابيع. اقضم قضمات صغيرة جداً وامضغ طويلاً وكُل ببطء وتوقّف عند أول شبع، ولا تشرب مع الأكل بل قبله أو بعده بنحو نصف ساعة. تجنّب المشروبات الغازية والقش والخبز الطازج واللحوم الجافة في المراحل الأولى، وابتعد عن السعرات السائلة لأنها تمرّ عبر الحلقة بلا شبع. خذ الفيتامينات والمعادن كما يوصف لك والتزم بتحاليل المتابعة. تحرّك وامشِ مبكّراً لتقليل خطر التجلّطات، وتدرّج في الرياضة بإذن الطبيب، ولا تفوّت مواعيد ضبط الحلقة. \*\*راجع فوراً عند: عدم القدرة على تمرير الطعام أو قيء مستمر بعد كل بلعة، أو ألم بطني أو صدري شديد، أو حرارة، أو تسرّع نبض أو ضيق نفس، أو انتفاخ وألم أو احمرار في موضع المنفذ، أو قيء دموي أو براز أسود.\*\*

## About this service

Gastric banding is a weight-loss operation in which an adjustable silicone ring is placed around the upper part of the stomach, creating a small pouch that fills with a modest amount of food and slows its passage into the rest of the stomach, so fullness comes sooner. What distinguishes it from other bariatric operations is that it \*\*does not cut or staple the stomach and does not reroute the intestine\*\*; it is adjustable by adding or withdrawing saline through a small port implanted under the abdominal skin, and it is removable.

The full picture, however, requires honesty. Use of the band has declined worldwide over the past decade because \*\*average weight loss is smaller and less durable\*\* than with sleeve gastrectomy or gastric bypass, and because a substantial proportion of patients eventually need the band removed or converted to another operation. It also depends heavily on regular adjustment visits and strict eating discipline, and it carries its own complications: band slippage, dilatation of the pouch or oesophagus, erosion of the band into the stomach, and problems with the port or its tubing.

What the operation does not do: it does not burn localised fat and does not tighten loose skin, it does not treat binge or emotional eating without psychological and behavioural support, it is not suitable for severe reflux, a large hiatal hernia or oesophageal motility disorders, and it does not replace changes in diet and activity. Soft, calorie-dense items such as ice cream and juices pass the band easily, so weight can fail to fall even when the device is working perfectly.

Candidacy is determined by multidisciplinary assessment: a bariatric surgeon, a dietitian, a psychological evaluation and sometimes endocrinology and gastroenterology input, with body mass index calculated, associated diseases reviewed and an upper endoscopy performed. Only clinical assessment decides whether this operation rather than another suits you — never a comparison with other people's experiences.

## Preparation

Tell the team about every medicine and supplement, especially blood thinners, contraceptive pills, diabetes drugs and heartburn medicines, and never change a dose yourself — your doctor adjusts diabetes treatment around fasting. Mention drug allergies, previous abdominal surgery and any bad experience with anaesthesia. Follow the preparatory diet your dietitian prescribes if one is requested, since it shrinks the liver and makes laparoscopic work easier. Stop smoking before and after surgery, as it increases respiratory and ulcer complications. Complete the endoscopy, blood tests and the psychological and nutritional assessments and bring the reports. Follow the anaesthetist's fasting instructions. Arrange a companion and one to two weeks off work, and know your adjustment schedule in advance, because it is an essential part of the treatment and not optional.

## Aftercare

Follow the staged diet your dietitian sets: clear fluids, then full fluids, puree, soft food and finally solids over several weeks. Take very small bites, chew thoroughly, eat slowly and stop at the first sense of fullness, and do not drink with meals — drink about half an hour before or after. Avoid fizzy drinks, straws, fresh bread and dry meat in the early stages, and avoid liquid calories, which slip past the band without producing fullness. Take prescribed vitamins and minerals and keep up with follow-up blood tests. Walk and move early to lower the risk of clots, increase exercise gradually with your surgeon's approval, and never miss a band adjustment appointment. \*\*Seek care immediately for: inability to pass food or persistent vomiting after every swallow, severe abdominal or chest pain, fever, rapid heartbeat or breathlessness, swelling, pain or redness at the port site, or vomiting blood or black stools.\*\*
