## Procedure steps

### 1. Reviewing the oncology plan and assessing symptoms

The nurse begins by reading the treatment plan, the cycle dates and the effects expected at each stage, then assesses symptoms: fever, fatigue, nausea, mouth ulcers, diarrhoea or constipation, appetite, weight, pain and its site and severity, and sleep and mood. A baseline is recorded against which any deterioration between visits can be measured.

### 2. Central line or port care

Hands are cleaned and sterile supplies used, the line site is inspected for redness, swelling, discharge or pain, and the dressing is changed and the line flushed strictly according to the physician's order. Connectors are not opened without need and no tip touches a non-sterile surface, because the line is a direct route into the bloodstream.

### 3. Symptom management in coordination with the team

Nausea, vomiting, mouth condition, bowel function and fatigue are followed according to the physician's plan: mouth care several times a day with a gentle rinse, small frequent meals, fluid monitoring and weight tracking. Any new or worsening symptom is reported to the treating team to decide on, and the nurse neither adjusts a medicine nor adds a treatment independently.

### 4. Infection prevention and safe handling

Patient and family are taught hand hygiene, avoiding crowds during low-immunity periods, food safety, and skin and mouth care. Precautions for handling body fluids in the days after chemotherapy are followed as the team directs, and gloves are used for cleaning. In these patients a fever is never dismissed, however innocent its apparent cause.

### 5. Psychological support, documentation and referral

The nurse listens to the patient and family and explains what to expect without making promises, helps organise appointments, medicines and transport, and watches for signs of depression and anxiety to refer on. Every assessment is documented in a record that stays with the patient, so the oncologist receives a true picture of what happens between cycles.

## About this service

Nursing care for cancer patients is a supportive service that runs alongside the oncology team's plan, never in place of it. It covers symptom follow-up between treatment cycles, care of a central venous catheter or implanted port, monitoring the effects of chemotherapy, radiotherapy or immunotherapy, nutritional support and mouth care, infection prevention, help with mobility and hygiene during periods of weakness, and psychological support for the patient and family. The aim is practical: to keep the patient able to complete treatment with as few crises as possible.

The limits are explicit: \*\*nursing does not treat cancer, does not diagnose, does not change a dose and does not stop or postpone a treatment protocol\*\*. Those are decisions for the oncologist. At home the service is delivered \*\*on a physician's order with a written care plan\*\* stating what is monitored, what must be reported at once, and when a visit is cancelled in favour of emergency care.

The most dangerous thing for the patient and family to know is this: \*\*a high fever after chemotherapy is an emergency\*\*, because the immune defences may be low and no obvious infection may be visible, and a delay of hours can mean bloodstream infection. Do not wait for the next visit and do not give a fever-reducing medicine to mask the sign before contacting the treating team. In the same way, \*\*hygiene and sterile technique in central-line care are what prevent the most dangerous complication, sepsis\*\*, since the line is a direct route into the circulation.

Several harmful myths need correcting. Starving the patient or severe fasting does not kill cancer; it accelerates loss of muscle mass, weakens tolerance of treatment and delays cycles. Herbal products and supplements are not inert: some interact with cancer medicines or affect the liver, so they should be taken only with the treating team's approval. Chronic cancer pain is not something to be simply endured; it is managed medically on a plan the physician sets. And because Jordan's home-care market includes unqualified providers, \*\*verify the licence of the nurse and of the centre or company\*\* before contracting.

## Preparation

Bring your oncologist's report, the treatment plan, the schedule of cycles and your most recent laboratory results, along with a list of everything you take, including supplements and herbal products, in their original packaging, since some interact with cancer treatment. Tell the nurse the type of central line or implanted port you have and when it was placed, any drug allergy, your chronic conditions, when your last cycle was and when the next is due. Record your temperature, weight, food and fluid intake and any new symptoms before the visit, because this information changes decisions. Prepare a clean, well-ventilated space at home, a working thermometer and an appointments log. \*\*Ask your team in advance: which number do I call for a high fever at night, and which emergency department do I go to.\*\* \*\*Verify the licence of the nurse and of the centre or company\*\* before contracting, and ask for a written care plan.

## Aftercare

Take and record your temperature daily, and more often in the days following a cycle. Care for your mouth several times a day with a soft brush and a gentle rinse, drink fluids as your physician allows, and eat small frequent protein-rich meals: better nutrition helps you tolerate treatment and does not feed the disease. Wash your hands often, avoid crowded places and people with infections during low-immunity periods, and take no herb or supplement without your team's approval. Do not endure pain in silence; report it so it can be treated. Go to the emergency department immediately for: \*\*high fever, chills or shivering after chemotherapy, which is an emergency that does not wait\*\*, \*\*a change in level of consciousness or confusion\*\*, \*\*difficulty breathing or chest pain\*\*, \*\*bleeding that will not stop, sudden bruising, or blood in urine or stool\*\*, \*\*persistent vomiting or diarrhoea that prevents drinking\*\*, \*\*bleeding, redness, pus or pain at the line or any tube site\*\*, \*\*a pressure sore that is widening or draining pus\*\*. All of these mean \*\*urgent contact or emergency care, not waiting for the next visit\*\*.
