## Procedure steps

### 1. Initial assessment and baseline

The nurse assesses level of consciousness, speech, movement in each limb, swallowing as graded by the specialist, and the state of skin, continence and mood, and measures vital signs. A written baseline is recorded, because the value of home follow-up lies in detecting change: any new decline from that baseline means urgent contact rather than waiting.

### 2. Positioning, movement and contracture prevention

The affected limb is placed in a correct position supported by pillows to reduce swelling and pain, range-of-motion exercises are carried out as the physiotherapist taught, and the patient is transferred to a chair and to standing progressively as permitted. The aims are to prevent joint contracture, muscle tightening and shoulder subluxation, and to reduce venous clots through movement.

### 3. Safe feeding and swallowing safety

The patient is fed only at the consistency the specialist prescribed, sitting upright, in small unhurried mouthfuls, with the mouth cleared before the next one, and the head kept elevated after eating. Coughing, a wet-sounding voice or gurgling during a meal are signs of aspiration: feeding stops and the swallowing assessment is repeated.

### 4. Skin, continence and medication care

A repositioning schedule is applied and the skin inspected daily over the tailbone, heels and hips, with prompt cleaning after any episode of wetness. Bowel and bladder patterns are tracked and unnecessary urinary catheters avoided, since they increase infection. Secondary-prevention medicines are given at the times the physician wrote, with nothing added and nothing stopped.

### 5. Communication, rehabilitation and documentation

The patient is addressed in short clear sentences with enough time to answer, gestures and pictures are used where speech is impaired, and nobody talks about them as if they were absent. Physiotherapy, occupational therapy and speech therapy appointments are tracked, signs of post-stroke depression are watched for and referred on, and every assessment is documented in a record that stays with the patient.

## About this service

Nursing care after a stroke begins once the patient is stable and discharged from hospital. Its aims are to protect the functions that remain, to prevent the complications that send patients back to hospital, and to support the rehabilitation programme. It includes daily assessment of consciousness, movement, swallowing and skin, correct positioning and range-of-motion exercises, safe feeding, continence and skin care, and follow-up of adherence to the secondary-prevention medicines the physician prescribed.

The limits deserve stating clearly: \*\*nursing alone does not restore lost function\*\*. Rehabilitation — physiotherapy, occupational therapy and speech and swallowing therapy — is the engine of recovery, while nursing creates the conditions for it and protects the patient in between. Nor does home care replace medical follow-up of blood pressure, diabetes, lipids and heart rhythm disorders, since these are what prevent a second stroke. A licensed nurse \*\*does not diagnose and does not change any dose\*\*, and never stops an anticoagulant or an antihypertensive. The service is delivered \*\*on a physician's order with a written care plan\*\*.

Some things save lives if they are known in advance. Any sudden weakness of the face, arm or leg, slurred or lost speech, loss of balance or sudden confusion \*\*means immediate emergency care, because time is brain\*\*: it does not wait for the morning or for the nurse's next visit. While a stroke is suspected, \*\*give the person nothing by mouth, no water, no medicine and no food\*\*, and do not give aspirin on your own initiative, because a proportion of strokes are haemorrhagic and a blood thinner can make them worse. That decision belongs to the physician after imaging.

Aspiration into the lungs is among the leading causes of death after a stroke, so no patient is fed before their swallowing has been assessed and the appropriate consistency of food and fluid prescribed. Because mobility is limited, pressure injury is a genuine risk and \*\*prevention is the treatment\*\*: a repositioning schedule, daily skin inspection and adequate nutrition. Where a catheter or feeding tube is present, \*\*hygiene and sterile technique are what prevent the most dangerous complication, sepsis\*\*. And because Jordan's home-care market includes unqualified providers, \*\*verify the licence of the nurse and of the centre or company\*\*.

## Preparation

Bring the hospital report, the discharge sheet and imaging report if available, the medicines in their original packaging, the swallowing assessment with the prescribed food and fluid consistency, and the physiotherapy plan. Tell the nurse the patient's level of awareness and their ability to speak, swallow and move, their chronic conditions such as high blood pressure, diabetes and heart rhythm disorders, any anticoagulant they take, and their drug allergies. Arrange the home to reduce falls: remove slippery rugs and obstacles, add night lighting, fit a grab rail and a stable seat in the bathroom, provide a bed reachable from both sides, and extra pillows for positioning. Ask the physician about a suitable support surface if mobility is very limited. \*\*Know in advance the emergency number and the nearest hospital with a stroke service.\*\* \*\*Verify the licence of the nurse and of the centre or company\*\*, and ask for a written care plan.

## Aftercare

Keep to the repositioning schedule and inspect the skin daily over the tailbone, heels, hips and shoulders. Feed the patient only at the prescribed consistency, sitting upright, in small mouthfuls, keep the head elevated after eating, and clean the mouth at least twice a day. Continue range-of-motion exercises and physiotherapy and speech therapy appointments, because consistency is what makes the difference. Give medicines on time and never stop an anticoagulant or blood-pressure medicine because the patient seems better, and attend follow-up for blood pressure, glucose and heart rhythm. Watch mood, since post-stroke depression is common and treatable. Call emergency services immediately for: \*\*new sudden weakness of the face, arm or leg, slurred or lost speech, loss of balance or sudden confusion, because time is brain\*\*, \*\*any change in level of consciousness\*\*, \*\*difficulty breathing or severe coughing with meals\*\*, \*\*high fever\*\*, \*\*bleeding from a catheter or tube site\*\*, \*\*a seizure\*\*, \*\*a pressure sore that is widening or draining pus\*\*, \*\*pain or swelling in one leg\*\*. Do not wait for the next visit, and give nothing by mouth when a new stroke is suspected.
