ويُختار نوع العدسة قبل الجراحة بقياسات دقيقة للعين: عدسة أحادية البؤرة تعطي وضوحاً ممتازاً على مسافة واحدة مع الحاجة لنظارة قراءة عادة، أو عدسة متعدّدة البؤر أو ممتدّة العمق تقلّل الاعتماد على النظارة لكنها لا تناسب كل عين وقد تسبّب هالات وتوهّجاً ليلياً، أو عدسة تصحّح الاستجماتزم. اختيار العدسة يُحدَّد بالفحص السريري وقياسات القرنية والشبكية وطبيعة عمل المريض، لا بالإعلانات.

ولحدود الجراحة أهمية مساوية لفوائدها: العدسة الصناعية تحلّ مشكلة العدسة المعتمة فقط، ولا تعالج أمراض الشبكية أو العصب البصري أو الزَرَق أو التنكّس الشبكي المرتبط بالعمر؛ فإذا كان ضعف النظر ناتجاً عن هذه الأمراض فالتحسّن سيكون محدوداً بقدرها. والساد لا يعود بعد إزالته، لكن الكبسولة الخلفية قد تتعكّر مع الوقت فيرجع الضباب تدريجياً، ويُعالَج ذلك بجلسة ليزر قصيرة في العيادة لفتح الكبسولة بلا دخول للعين. واستخدام الليزر في تحضير الجراحة خيار متاح لا ضرورة، والقرار الأهم يبقى في دقة القياسات ومهارة التنفيذ.

## خطوات الإجراء

### 1. الفحص والقياسات واختيار العدسة

يُفحص النظر وضغط العين والقرنية والشبكية بعد توسيع البؤبؤ، ويُقاس طول العين وانحناء القرنية بجهاز البيومتري لحساب قوة العدسة المزروعة. تُراجع الأمراض العامة والأدوية، ويُناقش نوع العدسة وتوقّعاتها الواقعية بلا مبالغة.

### 2. التحضير يوم العملية

تُوضع قطرات لتوسيع البؤبؤ وقطرات مخدّرة، وقد يُضاف تخدير موضعي حول العين وتسكين خفيف. تُنظَّف المنطقة ويُثبَّت غطاء معقّم، ويُطلب من المريض النظر إلى ضوء المجهر والبقاء ساكناً وإخبار الطبيب إن احتاج شيئاً.

### 3. الشقّ وتفتيت العدسة بالفاكو

يُصنع شقّ صغير جدّاً في القرنية وتُفتح الكبسولة الأمامية بدقّة، ثم يُدخل مجس الفاكو الذي يفتّت العدسة المعتمة بالموجات فوق الصوتية ويشفط بقاياها، مع الحفاظ على الكبسولة الخلفية سليمة لتحمل العدسة الجديدة.

### 4. زرع العدسة الصناعية

تُطوى العدسة المرنة وتُدخل عبر الشقّ نفسه ثم تُفتح وتُوضع في مركز الكبسولة، وتُضبط زاويتها بدقة عند العدسات المصحّحة للاستجماتزم. يُشطف الجل الواقي ويُتأكَّد من إحكام الشقّ ذاتياً بلا خياطة عادة.

### 5. المتابعة القريبة وبرنامج القطرات

يُفحص المريض في اليوم نفسه أو التالي لقياس النظر وضغط العين وسلامة القرنية، ويُشرح جدول القطرات المضادة للالتهاب والعدوى. تُحدَّد مراجعات متقاربة أول أسبوعين، ثم تُوصف النظارة النهائية بعد استقرار النظر.

## Procedure steps

### 1. Examination, measurements and lens selection

Vision, eye pressure, cornea and retina are examined after pupil dilation, and axial length and corneal curvature are measured by biometry to calculate the power of the implanted lens. General medical conditions and medicines are reviewed, and the lens type is discussed with realistic expectations rather than promises.

### 2. Preparation on the day of surgery

Dilating and anaesthetic drops are instilled, and a local block around the eye with light sedation may be added. The area is cleaned and a sterile drape applied, and the patient is asked to look at the microscope light, stay still, and tell the surgeon if anything is needed.

### 3. The incision and phacoemulsification

A very small corneal incision is created and the anterior capsule is opened precisely, then the phaco probe is introduced to emulsify the clouded lens with ultrasound and aspirate the fragments, while keeping the posterior capsule intact to support the new lens.

### 4. Implanting the intraocular lens

The foldable lens is inserted through the same incision, unfolded and centred within the capsule, with precise rotational alignment for lenses that correct astigmatism. The protective gel is washed out and the incision is confirmed to seal itself, usually without sutures.

### 5. Early follow-up and the drop schedule

The patient is reviewed the same or the next day to check vision, eye pressure and corneal clarity, and the schedule of anti-inflammatory and antibiotic drops is explained. Close reviews follow during the first two weeks, and final spectacles are prescribed once vision has stabilised.

## العناية بعد الإجراء

لا تفرك عينك ولا تضغط عليها إطلاقاً، والبس الغطاء الواقي للنوم في الأيام الأولى كما يوصي الطبيب. استخدم القطرات بمواعيدها بدقّة واغسل يديك قبل كل قطرة ولا تلمس رأس العلبة للعين. تجنّب دخول الماء والصابون والشامبو إلى العين، وامتنع عن السباحة والبخار والغبار والدخان، ولا ترفع أوزاناً ثقيلة ولا تنحن برأسك إلى الأسفل أول أسبوعين. النظر يتحسّن تدريجياً ويكون ضبابياً في البداية وقد ترى ألواناً أزهى وهالات حول الأضواء ليلاً، والنظارة النهائية تُوصف بعد استقرار العين. لا تقُد سيارتك حتى يسمح لك الطبيب. \*\*راجع الطوارئ فوراً عند: ألم شديد في العين لا يهدأ بالمسكّن المعتاد، تدهور مفاجئ في النظر بعد تحسّن، احمرار متزايد مع إفراز صديدي، غثيان وقيء مع ألم عين وصداع، أو ظهور وميض وذبابات كثيفة مفاجئة أو ظلّ كالستارة يزحف على مجال الرؤية.\*\*

## About this service

Phacoemulsification with intraocular lens implantation is the standard operation for cataract today: the clouded natural lens is broken up by ultrasound energy through a very small corneal incision, its fragments are gently aspirated, and a clear artificial lens is implanted inside the original lens capsule. It is usually performed under local or topical anaesthesia with the patient awake, needs no sutures in most cases because the incision seals itself, and the patient goes home the same day.

One point deserves to be stated plainly: \*\*cataract cannot be treated with eye drops, medicines, vitamins or a change of spectacles, and surgery is the only proven treatment\*\*. The timing, however, is not decided by a number on a machine but by how much the loss of vision affects daily life: reading, night driving, recognising faces, work, the risk of falling, and the ability to stay independent. Earlier surgery may be advised in specific situations, such as a dense cataract that prevents examination of the retina or raises eye pressure.

The type of lens is chosen before surgery using precise ocular measurements. A monofocal lens gives excellent clarity at one distance and usually still requires reading glasses; a multifocal or extended depth of focus lens reduces dependence on glasses but does not suit every eye and can produce halos and night-time glare; a toric lens corrects astigmatism. Which lens fits you is determined by clinical examination, corneal and retinal measurements and the nature of your work, not by advertising.

The limits of surgery matter as much as its benefits. An artificial lens solves the problem of a clouded lens only; it does not treat retinal disease, optic nerve damage, glaucoma or age-related macular degeneration, so when vision loss comes from those conditions the improvement will be limited accordingly. A cataract does not come back once removed, yet the posterior capsule may cloud over time and blur can return gradually; this is treated with a short laser session in the clinic that opens the capsule without entering the eye. Laser assistance during the operation itself is an available option rather than a necessity, and the decisive factors remain the accuracy of the measurements and the quality of the surgery.

## Preparation

Tell your eye surgeon about every medicine you take, especially blood thinners and anticoagulants and alpha-blocker prostate medicines, which affect how the pupil behaves during surgery, and stop nothing without your treating doctor's decision. Bring diabetes and blood pressure under control before the appointment, and report any infection of the eye, eyelid, teeth or urinary tract, as well as any previous eye surgery or contact lens use. Ask about fasting times if intravenous sedation is planned. Stop wearing contact lenses as directed, and use no eye make-up on the day before. Bring a list of your current drops, and arrange someone to drive you home, since vision will be blurred afterwards. Prepare your home in advance: light the hallways, remove trip hazards, and postpone heavy work and travel.

## Aftercare

Never rub or press on the eye, and wear the protective shield at night during the first days as advised. Use your drops exactly on schedule, wash your hands before each one, and do not let the bottle tip touch the eye. Keep water, soap and shampoo out of the eye, avoid swimming, steam, dust and smoke, and do not lift heavy weights or bend your head down during the first two weeks. Vision improves gradually and is blurred at first; colours may look brighter and halos may appear around lights at night, and final spectacles are prescribed once the eye has settled. Do not drive until your surgeon allows it. \*\*Seek emergency care immediately for severe eye pain that ordinary painkillers do not relieve, a sudden drop in vision after an initial improvement, increasing redness with purulent discharge, nausea and vomiting with eye pain and headache, or the sudden onset of flashes, a shower of floaters, or a curtain-like shadow moving across your field of vision.\*\*
