## Procedure steps

### 1. Planning and workup

Radiographs and CT are reviewed to size the defect and assess the surrounding bone, blood tests including inflammatory markers, HbA1c and vitamin D are requested, and hidden infection is excluded clinically and sometimes by sampling. Graft type and fixation method are decided on this basis.

### 2. Anaesthesia, sterility and prophylaxis

Surgery is performed under general or regional anaesthesia according to the site and your condition, prophylactic antibiotic is given at a defined interval before the incision, and the limb is prepared and draped. A tourniquet may be used to reduce bleeding in limb surgery.

### 3. Site preparation and graft harvest

The bone is exposed, fibrous and dead tissue is removed and the bone ends are freshened until they bleed, because a graft cannot survive on a dead surface. If autologous bone is used it is harvested from the iliac crest, the marrow cavity or another donor site chosen by the surgeon; otherwise donor bone or a synthetic substitute is prepared.

### 4. Graft placement and fixation

The graft is packed firmly into the defect in contact with living bone on all sides, then the bone is stabilised with the appropriate device — plates and screws, an intramedullary nail or an external fixator — because stability is a precondition for the graft to turn into bone. The wound is closed in layers, with a drain if required.

### 5. Rehabilitation and union follow-up

A graded weight-bearing plan is set by the surgeon, movement of adjacent joints starts early to limit stiffness, and follow-up radiographs are taken every few weeks to look for signs of union. Union is measured in months rather than weeks, and any delay prompts reassessment of fixation, smoking, nutrition and infection.

## About this service

Bone grafting is a procedure in which bone tissue is added to a deficient or poorly healing part of the skeleton, using either the patient's own bone, processed and sterilised donor bone from a bone bank, or synthetic substitutes such as calcium phosphate compounds and bioactive ceramics. The purpose is for the graft to act as a scaffold that the patient's own cells grow into and are stimulated by, so that the body gradually replaces it with living bone.

It is used in many situations: a fracture that has failed to unite or has united in a deformed position, a bone void left after removal of a cyst or benign tumour, fusion of a joint or of vertebrae, filling bone loss around a joint replacement during revision surgery, and reconstruction of bone lost after injury or after an infection has been brought under control. The type of graft is chosen according to the size and site of the defect and the mechanical support required: autologous bone is the biological benchmark but is limited in quantity and leaves pain at the harvest site, while donor bone provides larger volumes but incorporates more slowly.

Certain limits must be understood before consent. \*\*A graft is not a glue that fixes bone instantly\*\* — it is a scaffold that needs months to become bone, and it is the mechanical fixation with plates, screws, rods or an external frame that carries the limb during that time. Grafting does not succeed in the presence of active infection, inadequate fixation or continuing movement at the fracture site, or poor blood supply. Tobacco and nicotine in every form are among the strongest causes of failed union, and controlling diabetes, vitamin D and nutritional status is part of the treatment rather than general advice.

The procedure carries risks: wound infection, chronic pain at an autologous harvest site, failure of the graft to incorporate or its resorption, haematoma, and injury to a nerve or vessel depending on the site, along with a very small risk of disease transmission with donor bone, which is subject to strict screening and processing. Suitability and graft type are determined by clinical examination and imaging rather than by reading, and they are reassessed during surgery according to what is found at the site.

## Preparation

Stop smoking, waterpipe and nicotine substitutes several weeks before surgery and stay off them afterwards; this is the single strongest factor you control for bone healing. Get your diabetes under control and share your glucose readings, and list all your medicines, especially blood thinners, corticosteroids, immune-modulating drugs and osteoporosis treatments, stopping nothing on your own. Tell your surgeon about any skin, dental or urinary infection close to the operation date, and about any previous surgery or infection in the same bone. Follow fasting instructions, arrange help at home for the first two weeks, prepare crutches or a walker and a shower seat, remove slippery rugs and set up a sleeping place on a single floor.

## Aftercare

Elevate the limb and use cold packs as instructed, take your prescribed painkillers, and do not start anti-inflammatory drugs on your own, as some may affect bone healing — ask your surgeon first. Do not put weight on the limb before you are explicitly cleared, move the adjacent joints and your digits as taught, and keep the wound clean and dry until the dressing review. Avoid nicotine completely, keep well hydrated with adequate protein intake, and attend your imaging and physiotherapy appointments. \*\*Seek care immediately for fever with redness, warmth or discharge from the wound, pain that increases after having improved, numbness, pallor, coldness or inability to move the digits, calf pain with swelling, or sudden breathlessness or chest pain.\*\*
