تُستخدم مصادر عدة: عظم من المريض نفسه من موضع مجاور أو من الفك، أو مواد عظمية مُعالَجة من مصدر بشري أو حيواني مرخّصة، أو مواد صناعية، وغالباً مع غشاء يفصل ويحمي المنطقة أثناء التكوّن. وفي الفك العلوي الخلفي قد يُرفع أرضية الجيب الأنفي لإتاحة ارتفاع كافٍ. اختيار المادة قرار طبي يعتمد على حجم العيب وموقعه لا على تفضيل تجاري.

والحدود مهمة: زراعة العظم لا تعالج التهاب اللثة النشط ولا التسوّس، ويجب علاجهما قبلها. ولا تضمن نجاح الزرعة بذاتها، ولا تنمّي عظماً حول زرعة فاشلة أو ملتهبة بلا معالجة السبب. ولا تُعطي نتيجة فورية: العظم يحتاج وقتاً ليتكوّن ويتماسك، والتعجّل بوضع الزرعة قبل ذلك يهدّد الاثنين. والتدخين وسوء التحكّم بالسكري من أقوى أسباب فشل الزراعة.

توقّع تورّماً وكدمات لأيام، وتحسّساً في المنطقة، وحاجة لالتزام دقيق بالنظافة والغسول. ومن المضاعفات المحتملة العدوى، وانكشاف الغشاء، وفقدان جزء من المادة المزروعة، وتنميل مؤقت. اطلب أن يُشرح لك احتمال كل ذلك في حالتك، وخطة البديل إن لم يتكوّن العظم كما هو مأمول.

## خطوات الإجراء

### 1. تقييم العظم بالصورة المقطعية

تُقاس أبعاد العظم المتوفّر وموقع الجيب الأنفي والعصب بصورة مقطعية ثلاثية الأبعاد، ويُقرّر هل النقص يحتاج إضافة بسيطة مع الزرعة أم عملية مستقلّة تسبقها بأشهر.

### 2. تهيئة الفم قبل الإجراء

تُعالج اللثة الملتهبة ويُنظّف الجير ويُعالج أي تسوّس أو التهاب حول جذر مجاور، لأن الزراعة في فم غير سليم من أكثر أسباب الفشل والعدوى.

### 3. الوصول إلى منطقة النقص

بتخدير موضعي أو مع تهدئة، تُرفع اللثة بعناية لكشف العظم، ويُنظّف الموضع من أي نسيج ليفي، وتُحضّر أرضية العظم لتشجيع الأوعية الدموية على النموّ داخل المادة المضافة.

### 4. إضافة المادة العظمية والغشاء

تُوضع المادة العظمية بالحجم المخطّط وقد تُثبَّت، ويُغطّى الموضع بغشاء يحميه ويمنع الأنسجة الرخوة من احتلال المساحة، وفي الفك العلوي الخلفي قد يُرفع الجيب الأنفي بلطف قبل ذلك.

### 5. الإغلاق ومهلة التكوّن

تُخاط اللثة بإغلاق محكم بلا شدّ، وتُعطى تعليمات صارمة للنظافة، ثم يُنتظر عادة من أربعة إلى تسعة أشهر بحسب حجم الإضافة قبل تقييم العظم بصورة جديدة ووضع الزرعة.

## Procedure steps

### 1. Assessing the bone with a 3D scan

The available bone dimensions and the position of the sinus and nerve are measured on a three dimensional scan, and it is decided whether the deficiency needs a small simultaneous graft or a separate operation months before the implant.

### 2. Preparing the mouth beforehand

Inflamed gums are treated, calculus is removed and any decay or infection around an adjacent root is dealt with, because grafting into an unhealthy mouth is one of the commonest causes of failure and infection.

### 3. Accessing the deficient site

Under local anaesthesia, with sedation if needed, the gum is carefully raised to expose the bone, fibrous tissue is cleared, and the bone bed is prepared so blood vessels can grow into the added material.

### 4. Placing the graft and membrane

The graft material is placed to the planned volume and may be fixed in position, then covered by a membrane that protects the site and stops soft tissue from invading the space. In the upper back jaw, the sinus floor may be gently lifted first.

### 5. Closure and the consolidation period

The gum is closed tightly but without tension, strict hygiene instructions are given, and a wait of roughly four to nine months follows, depending on the graft size, before the bone is reassessed on a new scan and the implant is placed.

## التحضير قبل الإجراء

أخبر الطبيب بكل أدويتك، وخصّ بالذكر مسيّلات الدم وأدوية هشاشة العظام سواء كانت حبوباً أو حقناً لأنها تؤثّر مباشرة على شفاء عظم الفك، والكورتيزون ومثبّطات المناعة والعلاج الكيماوي أو الإشعاعي على منطقة الرأس والرقبة إن سبق لك ذلك. اضبط سكر الدم قبل الإجراء إن كنت مصاباً بالسكري، وأخبره إن كنتِ حاملاً أو مرضعاً. أوقف التدخين قبل الإجراء بأسابيع واستمرّ في الامتناع بعده، فهو من أقوى مسبّبات فشل الزراعة. أكمل تنظيف اللثة والجير قبل الموعد، وأحضر صورك السابقة، والتزم بالصيام إن كانت هناك تهدئة، ورتّب من يوصّلك، وجهّز طعاماً طريّاً في المنزل.

## العناية بعد الإجراء

لا تشدّ الشفة أو الخدّ لترى الجرح ولا تتحسّسه بلسانك، فأي شدّ على الخياطة قد يفتح الموضع ويكشف الغشاء. ضع الكمادات الباردة في الساعات الأولى ونم برأس مرفوع، وتناول طعاماً طريّاً بارداً وابتعد عن الطعام الصلب والحبوب والمكسّرات حتى يُسمح لك، ولا تمضمض بقوة ولا تستخدم الشفّاطة. استخدم الغسول الموصوف بدقّة، ونظّف بقية الأسنان بلطف، وامتنع عن التدخين تماماً، وتجنّب نفخ الأنف بقوة إن كان الإجراء قريباً من الجيب الأنفي. \*\*راجع فوراً عند تورّم يزيد بعد اليوم الثالث مع حرارة، أو خروج قيح أو رائحة كريهة، أو انفتاح الخياطة أو ظهور الغشاء، أو نزيف لا يتوقف بالضغط، أو تنميل مستمرّ في الشفة أو الذقن، أو خروج سوائل أو هواء من الأنف عند نفخه.\*\*

## About this service

A jaw bone graft adds bone tissue or a bone substitute to an area where the jaw bone is deficient, so that the ridge regains enough volume, width and height to support a dental implant or to repair a defect left by an extraction, an infection, a cyst or an injury.

Jaw bone typically shrinks after teeth have been missing for years, because the ridge no longer carries a root. It is also lost after advanced gum disease, cyst removal, a difficult extraction or trauma. The deficiency is measured on a three dimensional scan rather than by eye, because the width of the bone and its proximity to the maxillary sinus or the lower jaw nerve decide which type of grafting is even possible.

Several sources are used: the patient's own bone taken from a nearby site or the jaw, processed and licensed bone material of human or animal origin, or synthetic materials, frequently combined with a membrane that separates and protects the site while new bone forms. In the upper back jaw, the sinus floor may be lifted to create sufficient height. The choice of material is a clinical decision based on the size and location of the defect, not a commercial preference.

The limits matter. Grafting does not treat active gum disease or decay, and both must be controlled first. It does not by itself guarantee that an implant will succeed, and it will not rebuild bone around a failing or infected implant unless the cause is addressed. It also gives no instant result: new bone needs time to form and consolidate, and placing an implant before that jeopardises both. Smoking and poorly controlled diabetes are among the strongest causes of graft failure.

Expect swelling and bruising for some days, tenderness at the site, and a need for meticulous hygiene and rinsing. Possible complications include infection, membrane exposure, partial loss of the grafted material and temporary numbness. Ask for these risks to be explained for your case, along with the fallback plan if the bone does not form as hoped.

## Preparation

Tell your surgeon about all your medicines, particularly blood thinners and bone density drugs whether tablets or injections, since these directly affect jaw bone healing, as well as steroids, immunosuppressants, and any previous chemotherapy or radiotherapy to the head and neck. If you have diabetes, bring your blood sugar under control beforehand, and mention pregnancy or breastfeeding. Stop smoking weeks before and stay off it afterwards, as it is among the strongest causes of graft failure. Complete gum and scaling treatment before the appointment, bring previous images, follow fasting rules if sedation is planned, arrange a driver, and prepare soft food at home.

## Aftercare

Do not pull your lip or cheek to inspect the wound and do not touch it with your tongue, because any tension on the sutures can open the site and expose the membrane. Use cold packs during the first hours, sleep with your head raised, eat soft cool food and avoid hard items, grains and nuts until cleared. Do not rinse forcefully or use a straw. Use the prescribed rinse exactly as directed, clean the other teeth gently, avoid smoking completely, and do not blow your nose forcefully if the graft was near the sinus. \*\*Seek care immediately if swelling increases after day three with fever, if there is pus or a foul smell, if the sutures open or the membrane becomes visible, if bleeding will not stop with pressure, if numbness of the lip or chin persists, or if fluid or air passes from the nose when you blow it.\*\*
