يُطرح هذا الخيار غالباً على من لا يصلح لليزر: الدرجات العالية جداً، أو القرنية الرقيقة أو غير المنتظمة، أو الجفاف الشديد، أو حين يكون النسيج الذي سيُزيله الليزر أكثر من الآمن. لكن القرار يحتاج قياسات دقيقة لا يمكن تقديرها من الخارج: عمق الغرفة الأمامية، تعداد الخلايا البطانية للقرنية، المسافة بين زوايا العين، حجم الحدقة، وفحص شبكية موسّع. وإن كانت الغرفة ضيّقة أو الخلايا البطانية قليلة فالإجراء غير مناسب مهما كانت الدرجة.

حدود الإجراء يجب أن تكون واضحة قبل الموافقة: العدسة تصحّح الانكسار فقط، فهي لا تعالج ضعف النظر القريب بعد الأربعين ولا تمنع الحاجة لنظارة قراءة لاحقاً، ولا تعالج الشبكية ولا الجلوكوما، ولا تُوقف طول العين ولا خطر تمزّق الشبكية عند شديدي قصر النظر — وهؤلاء يحتاجون فحص قاع عين دورياً مدى الحياة. كما أنها لا تمنع تكوّن الساد لاحقاً، وعندها تُزال العدسة المزروعة أثناء عملية الساد وتُحسب عدسة جديدة.

ولأنها جراحة داخل العين فمخاطرها من نوع آخر غير مخاطر الليزر السطحي: ارتفاع ضغط العين خاصة في الساعات الأولى، تفاعل التهابي، تلامس العدسة مع العدسة الطبيعية أو نقص الخلايا البطانية على المدى الطويل، توهّج وهالات ليلية عند بعض المرضى، والتهاب داخل العين وهو نادر لكنه خطير. لذلك تُقاس ضغط العين في نفس اليوم، وتُتابع العين بمواعيد منتظمة لسنوات لمراقبة صفاء العدسة الطبيعية وسلامة القرنية.

## خطوات الإجراء

### 1. قياسات تحديد الترشيح ومقاس العدسة

تُقاس درجة النظر بعد توسيع الحدقة، وعمق الغرفة الأمامية، وأبعاد الجزء الأمامي بالتصوير، وتعداد الخلايا البطانية، وطول محور العين، ويُفحص قاع العين. من هذه الأرقام تُختار قوّة العدسة ومقاسها — وهي خطوة لا تقبل التقريب.

### 2. التحضير والتخدير

تُستخدم قطرات لتوسيع الحدقة أو تضييقها بحسب تصميم العدسة، وقطرات مخدّرة موضعية مع تسكين وريدي خفيف عند الحاجة. يُعقَّم محيط العين، وقد تُجرى قبل العملية أو خلالها فتحة صغيرة في القزحية في بعض التصاميم لتسهيل تصريف السائل.

### 3. الشقّ وحماية الأنسجة

يُعمل شقّ صغير عند حدود القرنية، وتُحقن مادة لزجة شفّافة تحافظ على شكل الغرفة الأمامية وتحمي بطانة القرنية والقزحية أثناء إدخال العدسة. الشقّ صغير ولا يحتاج خيوطاً في معظم الحالات.

### 4. إدخال العدسة وضبط موضعها

تُدخل العدسة مطويّة عبر حاقن رقيق فتنفتح داخل العين، ثم تُوضع أطرافها في موضعها الصحيح — أمام العدسة الطبيعية أو مثبَّتة بالقزحية — ويُضبط محورها إن كانت تصحّح انحرافاً، ثم تُشفط المادة اللزجة كاملة لتقليل خطر ارتفاع الضغط.

### 5. قياس الضغط والمتابعة الطويلة

يُقاس ضغط العين بعد ساعات من العملية ثم في اليوم التالي، وتُعطى خطّة قطرات مضادّة للالتهاب والعدوى. العين الأخرى تُجرى عادة في جلسة قريبة منفصلة. المتابعة تستمرّ سنوياً لمراقبة الضغط وصفاء العدسة الطبيعية وتعداد الخلايا البطانية.

## Procedure steps

### 1. Sizing and candidacy measurements

Dilated refraction, anterior chamber depth, anterior-segment imaging, endothelial cell count and axial length are measured, and the retina is examined. Lens power and size are selected from these figures — a step that leaves no room for approximation.

### 2. Preparation and anaesthesia

Drops dilate or constrict the pupil depending on the lens design, with topical anaesthesia and light intravenous sedation if required. The eye area is sterilised, and with some designs a small iris opening is created before or during surgery to keep fluid drainage open.

### 3. Incision and tissue protection

A small incision is made at the corneal margin and a clear viscoelastic is injected to maintain the shape of the anterior chamber and protect the corneal endothelium and iris while the lens is introduced. The incision is small and usually needs no sutures.

### 4. Lens insertion and positioning

The folded lens is introduced through a fine injector, unfolds inside the eye, and its haptics are settled in position — in front of the natural lens or fixated to the iris. Its axis is aligned if it corrects astigmatism, then the viscoelastic is fully aspirated to reduce the risk of a pressure spike.

### 5. Pressure check and long-term follow-up

Intraocular pressure is checked a few hours after surgery and again the next day, and anti-inflammatory and antibiotic drops are prescribed. The second eye is usually treated at a separate nearby session. Yearly follow-up continues to monitor pressure, the clarity of the natural lens and the endothelial cell count.

أوقف العدسات اللاصقة قبل القياسات وقبل العملية بالمدّة التي يحدّدها طبيبك حتى تعود القرنية إلى شكلها الطبيعي. أخبر الطبيب بكل أدويتك، وخصوصاً مضادّات التخثّر وأدوية البروستاتا من نوع حاصرات ألفا لأنها تؤثّر في سلوك الحدقة، وبأي التهاب داخل العين سابق أو ارتفاع في ضغط العين أو عملية عين قديمة أو إصابة. اذكر أمراضك المزمنة والسكري وأمراض المناعة، وأي حساسية دوائية. اتبع تعليمات الصيام إن كان هناك تسكين وريدي، ولا تضع مكياجاً أو عطراً على الوجه يوم العملية. أحضر مرافقاً يقود بك ويبقى معك أوّل ليلة، وجهّز في البيت مكاناً هادئاً ومسكّناً موصوفاً وقائمة أرقام الطوارئ الخاصّة بالمركز.

## العناية بعد الإجراء

التزم بمواعيد القطرات المضادّة للعدوى والالتهاب بدقّة، ولا تفرك العين ولا تضغط عليها، والبس الواقي أثناء النوم إن طُلب منك. تجنّب الانحناء لحمل أوزان ثقيلة والشدّ والسباحة والساونا في الأيام الأولى، ولا تضع مكياج عيون قبل إذن الطبيب. حضور مراجعة قياس الضغط في نفس اليوم أو اليوم التالي ليس اختيارياً، لأن أخطر ما يحدث مبكراً هو ارتفاع ضغط العين. \*\*راجع الطوارئ فوراً عند ألم شديد في العين أو صداع مع غثيان وقيء ورؤية هالات ملوّنة (قد يعني ارتفاعاً حادّاً في ضغط العين)، أو هبوط مفاجئ في الرؤية، أو احمرار شديد مع إفراز وضبابية متزايدة (شبهة التهاب داخل العين)، أو ظهور وميض ونقاط متحرّكة كثيفة أو ظلّ كستارة في مجال البصر (شبهة انفصال شبكية).\*\*

## About this service

Implantable lens surgery places a permanent medical lens inside the eye — in front of the natural lens or fixated to the iris — to correct myopia, hyperopia or astigmatism without removing corneal tissue. It is not cataract surgery: the natural lens stays in place, preserving the young eye's ability to focus at near, and the implanted lens can be removed or exchanged if needed.

The option is most often raised for eyes that are unsuitable for laser treatment: very high prescriptions, thin or irregular corneas, significant dryness, or cases where the amount of tissue the laser would have to remove exceeds what is safe. The decision, however, depends on measurements that cannot be estimated from the outside: anterior chamber depth, corneal endothelial cell count, the internal distance across the eye, pupil size, and a dilated retinal examination. If the chamber is shallow or the endothelial count is low, the procedure is not appropriate regardless of the prescription.

The limits must be clear before consent. The lens corrects refraction only. It does not treat the loss of near focus that begins after the age of forty, so reading glasses may still be needed later. It does not treat retinal disease or glaucoma, and it does not shorten the eye or remove the lifelong retinal-tear risk that comes with high myopia — those patients still need regular dilated retinal checks. It also does not prevent a cataract from forming later; if that happens, the implanted lens is removed during cataract surgery and a new lens power is calculated.

Because this is intraocular surgery, its risks differ in kind from surface laser risks: a pressure rise in the first hours, inflammation, contact with the natural lens or endothelial cell loss over the long term, night-time glare and halos in some patients, and endophthalmitis, which is rare but serious. For that reason intraocular pressure is measured on the same day, and the eye is monitored for years to check the clarity of the natural lens and the health of the cornea.

## Preparation

Stop contact lenses before the measurements and before surgery for the period your doctor specifies, so the cornea returns to its natural shape. Report all your medications, especially blood thinners and alpha-blocker prostate drugs, which affect pupil behaviour, and mention any previous intraocular inflammation, raised eye pressure, earlier eye surgery or injury. List chronic conditions such as diabetes, autoimmune disease and any drug allergy. Follow fasting instructions if sedation is planned, and avoid make-up and perfume on the day. Bring someone to drive you and stay with you the first night, and prepare a quiet space at home, your prescribed painkiller and the centre's emergency numbers.

## Aftercare

Follow the antibiotic and anti-inflammatory drop schedule precisely, do not rub or press the eye, and wear the protective shield at night if instructed. Avoid bending to lift heavy weights, straining, swimming and saunas in the first days, and use no eye make-up until your doctor allows it. The pressure check on the same day or the next morning is not optional, because the most dangerous early event is a rise in intraocular pressure. \*\*Go to emergency care immediately if you have severe eye pain, or headache with nausea and vomiting and coloured halos around lights (possible acute pressure rise), a sudden drop in vision, marked redness with discharge and increasing haze (possible intraocular infection), or a shower of new floaters, flashes or a curtain-like shadow in your field of vision (possible retinal detachment).\*\*
