## Procedure steps

### 1. Preparation and fasting

Six to eight hours of fasting is usually requested so the gallbladder and bile ducts can be assessed, and you may be asked to drink water and not empty your bladder if the bladder and pelvis are to be examined. Regular medicines are taken as usual unless you are told otherwise.

### 2. Positioning and applying the gel

You lie on the couch with the abdomen exposed and slightly cool coupling gel is applied to eliminate air between probe and skin. There are no needles, no contrast and no sedation; the usual sensation is light pressure that may increase a little over a tender area.

### 3. Systematic scanning of the organs

The operator sweeps the probe over the abdomen in a set order to assess liver, gallbladder, ducts, pancreas, spleen, kidneys, bladder and vessels, taking measurements and still images for the report whenever something merits documenting.

### 4. Repositioning, breath holding and Doppler

You may be asked to take a deep breath and hold it, or to turn onto your side, which brings the liver and gallbladder down and moves gas away so the view improves. Doppler is added when blood flow in vessels or organs needs to be assessed.

### 5. The report and linking it to the clinical picture

The radiologist issues a report of findings and measurements, which the requesting doctor then reads alongside your symptoms, examination and blood tests to decide the next step. Do not base a treatment decision on the report alone, and do not compare it with someone else's.

## About this service

An abdominal ultrasound is an imaging test that uses high-frequency sound waves to display the abdominal organs on screen in real time. A radiologist moves a probe over the skin after applying coupling gel; the waves reflect differently from different tissues and are converted into a moving image showing the liver, gallbladder, bile ducts, pancreas, spleen, kidneys, bladder and major vessels, with the option of assessing blood flow speed and direction using Doppler.

Two features matter most. The examination is \*\*safe and involves no ionising radiation\*\*, so it can be repeated freely and is used in pregnancy, in children and in newborns; and it is a live study performed with the patient in front of the operator, who can change your position, ask you to hold your breath, and press on the painful spot to understand it. It is commonly requested for abdominal pain, gallbladder disease and stones, raised liver enzymes, jaundice, suspected kidney stones or obstruction, follow-up of a cyst, fatty liver or an enlarged spleen, and for assessment after abdominal injury or during monitoring of a chronic condition.

The test also has genuine limits you should know. \*\*An ultrasound report is read together with the clinical examination and laboratory results, never on its own.\*\* A report describing mild fatty liver or a simple kidney cyst is extremely common and does not necessarily mean disease that needs treatment, and the reverse also happens: symptoms may be significant while the scan looks entirely normal. Image quality is affected by bowel gas and by the thickness of the abdominal wall, and some structures such as the pancreas and parts of the bowel may not be seen clearly enough. Ultrasound \*\*cannot see inside the stomach and colon\*\*, so it is not a substitute for endoscopy, and it \*\*does not diagnose cancer by itself\*\*: any suspicious mass needs more detailed imaging and then a biopsy, since tissue diagnosis is the basis of any treatment decision.

Preparation matters because the result depends on it. Fasting keeps the gallbladder full so it can be properly assessed, a full bladder allows the pelvis and bladder to be evaluated, and a lot of bowel gas can hide exactly what is being looked for. The test needs no needles, no contrast and no sedation, leaves no mark, and you can return to work and to eating immediately afterwards.

Interpretation remains the responsibility of the doctor who requested the scan. That is who links the images to your symptoms and blood results and decides whether further testing, a repeat scan after an interval, or nothing at all is required. No treatment decision should be built on an imaging report in isolation, and certainly not by comparing your report with someone else's.

## Preparation

Ask when booking what preparation is required, since it differs with what is being examined. For a standard liver and gallbladder study you are usually asked to fast for six to eight hours from food and drink apart from water and medicines, and to avoid fizzy drinks, chewing gum and smoking beforehand because they introduce air that blocks the view. If the bladder or pelvis is to be assessed, drink water and do not pass urine before the scan, exactly as your instructions state. Take your regular medicines as usual, and tell the department if you have diabetes and use insulin so a suitable time can be arranged. Mention pregnancy: the scan is safe and uses no radiation, but the information is useful context. Mention any wounds, dressings on the abdomen or previous surgery. Bring your previous reports and images and the request form from your doctor, and wear loose clothing that is easy to lift.

## Aftercare

There are no restrictions after an abdominal ultrasound: wipe the gel off your skin and return immediately to eating, drinking, work and normal activity, with no recovery period and no monitoring needed. If you fasted since the morning, have a balanced meal afterwards, particularly if you have diabetes. The important next step is to take the report and images to the doctor who requested them rather than reading it alone and searching the terms online; phrases such as mild fatty liver, simple cyst or renal sand are very common and their meaning depends on your symptoms and blood results. Keep a copy of the report and the disc or images, as they are the baseline for comparison in any future scan. \*\*Go to the emergency department immediately for\*\* \*\*sudden severe abdominal pain, or pain that will not settle with a rigid abdomen\*\*, \*\*fever with shivering, or yellowing of the eyes and skin\*\*, persistent vomiting that prevents drinking, vomiting blood or black stools, \*\*no urine output, or urine that is tea coloured or bloody\*\*, or marked dizziness with pallor and a racing pulse.
