## Procedure steps

### 1. Examination and measuring the excess

The surgeon examines the arm both raised and relaxed, pinches the skin to estimate what can be removed, distinguishes skin excess from fat excess, and decides whether liposuction alone will do or excision is required. The armpit and chest wall are checked because laxity often continues there, and the expected scar line on the inner arm is shown to you frankly before any decision is made.

### 2. Medical work-up and risk assessment

Blood tests, clotting studies and blood sugar are checked, and your medications are reviewed so that blood thinners and supplements that increase bleeding can be paused in coordination with your treating doctor. You are asked about previous arm swelling or lymph node removal, because either changes the surgical plan. Weight and weight stability, smoking and coexisting illnesses are discussed openly, and stopping smoking before the date is required.

### 3. Marking the scar line

Markings are drawn with you standing and the arm elevated, because skin redistributes when you lie down. The line is placed in the inner groove of the arm to be least visible, or confined to the armpit crease for minor laxity, or extended onto the chest wall for extensive laxity. The chosen line is agreed with you, and informed consent is signed after the complications have been explained.

### 4. Liposuction, excision and tightening

Limited liposuction is often done first to reduce the fat layer and let the skin redrape, then the excess skin is carefully excised while protecting the lymphatic channels and the sensory skin nerves on the inner arm. Deep tissue is anchored with sutures that carry tension away from the skin to limit scar widening, and symmetry with the other arm is checked before closure.

### 5. Closure, dressings and follow-up

The wound is closed in layers with fine sutures, a compression sleeve is fitted to limit swelling and fluid collection, and a small drain is sometimes used. Most people go home the same day. Appointments are set for suture removal and for reviewing swelling and the scar over about a year, with written instructions on warning signs and on gradually resuming elbow movement.

## About this service

An arm lift, or brachioplasty, is surgery that removes excess sagging skin from the inner upper arm - the loose curtain of tissue between the armpit and the elbow - and re-anchors the tissue beneath it, with limited liposuction when needed to smooth the borders. It is usually sought after substantial weight loss or when skin elasticity has been lost with age, so that the skin keeps hanging even though the fat has gone.

\*\*An arm lift is not a treatment for obesity.\*\* It is a shaping procedure for people whose weight has already stabilised, not a way to lose weight and not a substitute for doing so, and it does not treat the medical consequences of excess weight. Anyone operated on before their weight settles loses the result: later weight loss leaves fresh laxity beside the scar, and weight gain stretches the sutured skin and puts the wound and scar at risk of widening. A stable weight for six to twelve months is advised before surgery.

The limits are explicit. Surgery removes skin and fat; it does not strengthen the arm muscle or improve its function, so a toned appearance comes from resistance training, not from the operation. Non-surgical devices of any kind do not repair genuine skin laxity in the upper arm - skin that can be pinched and does not spring back needs to be excised. Cellulite and surface irregularity are never permanently cured and may persist after the lift. Liposuction on its own is enough only for people whose skin elasticity is good and whose excess is fat rather than skin.

Candidacy is decided by clinical examination. The surgeon measures how much excess skin there is and how elastic it is, and where the laxity ends - whether it stops mid-arm or extends onto the chest wall and armpit. Your weight and its stability, smoking, and coexisting conditions such as diabetes, thyroid disease and clotting disorders are discussed, along with any history of arm lymphoedema or previous breast or axillary surgery. \*\*Smoking impairs the blood supply to the skin and causes wounds to fail\*\*, and increases the risk of the wound opening near the elbow where tension is highest.

The scar is a fact that cannot be hidden, and it is the most visible of all body-contouring scars because it lies on the inner arm and shows when you raise your hand or wear sleeveless clothing. It usually runs from the armpit to the elbow, may be extended onto the chest wall when laxity continues there, and can sometimes be confined to the armpit crease when the excess is minor. Colour fades over about a year but the scar remains, and the arm is one of the sites most prone to scar widening or thickening.

## Preparation

Tell your surgeon about every medication and supplement you take, especially blood thinners, aspirin, steroids and herbal products, and never stop a long-term medication on your own. Stop smoking, shisha and nicotine substitutes for the period your surgeon specifies, because smoking impairs blood supply and causes wound failure, particularly near the elbow. Stabilise your weight and do not have surgery while you are still dieting or actively losing weight. Report any previous arm swelling or breast or armpit surgery. Fast for the period you are told and arrange someone to drive you home. At home, prepare loose front-opening clothes so you do not have to raise your arms, pillows to elevate the arms while sleeping, and postpone any work that involves lifting weights or reaching overhead for several weeks.

## Aftercare

Elevate your arms on pillows for the first days to limit swelling, wear the compression sleeve for the period prescribed, and move the elbow gently from day one without pulling on the wound. Do not lift weights or reach forcefully overhead for four to six weeks, do not massage the wound, and keep dressings clean and dry. Expect swelling of the forearm and hand and numbness along the inner arm, both settling gradually over weeks to months. Protect the scar from the sun for a full year, and keep your weight stable, because the result is lost when it changes.

Seek care immediately for: \*\*shortness of breath or chest pain, which may mean a pulmonary embolism, the most dangerous complication\*\* - \*\*leg pain with swelling and warmth\*\* - \*\*fever, purulent discharge or a foul smell from the wound\*\* - \*\*sudden increasing swelling of the arm, wound separation, or edges turning blue or black\*\*. Do not wait for your scheduled appointment if any of these appear.
