وعن الإشعاع نطمئنك بصدق: الجرعة في التصوير البانوراميّ منخفضة، ولا يُطلب التصوير عندنا كإجراء روتيني بلا داعٍ، بل يُبرَّر بسؤال تشخيصي واضح لا تُجيب عنه العين ولا الفحص السريري وحده. ويُقلّل التعرّض أكثر باستخدام الدرع الواقي والتصويب الدقيق واختيار أصغر مجال يفي بالغرض، وبمراجعة الصور السابقة قبل تكرار أي تصوير. أخبر الطبيب إن كنتِ حاملاً أو تشكّين بالحمل، فحينها يُوازن الطبيب بين ضرورة الصورة وتوقيتها ويُؤجّلها إن أمكن التأجيل.

ومن حدودها المهمّة: البانوراما مسح شامل لكن قليل التفصيل، فهي لا تكشف التسوّس المبكر بين الأسنان بدقّة — وهذا عمل الصور الصغيرة داخل الفم — ولا تعطي أبعاداً ثلاثية موثوقة لتخطيط زرعة أو لتحديد علاقة الجذر بالعصب بدقّة، وهنا يُطلب التصوير المقطعي المخروطي. كما تظهر فيها تشويهات وتضخيم وظلال متراكبة قد تُقرأ خطأً إن أُخذت بمعزل عن الفحص السريري. لذلك يبقى قرار طلب الصورة وتفسيرها للطبيب بعد فحصك مباشرة.

## خطوات الإجراء

### 1. تحديد الاستطباب ومراجعة الصور السابقة

يحدّد الطبيب السؤال التشخيصي الذي تجيب عنه الصورة، ويراجع أي صور سابقة قريبة لتفادي تكرار غير ضروري، ويسأل عن الحمل أو احتماله، ثم يشرح لك سبب الطلب وماذا سيُبحث فيه بالتحديد.

### 2. خلع المعادن ووضع الدرع الواقي

تُخلع الأقراط والحلق ومشابك الشعر والقلائد والنظارات وأطقم الأسنان المتحرّكة وأي حَلَق في اللسان أو الشفة، لأنها تُنتج ظلالاً تحجب التفاصيل؛ ثم يُوضع الدرع الواقي بحسب ما تعتمده العيادة.

### 3. وضع الرأس بدقّة

تقف أو تجلس أمام الجهاز، وتعضّ قضيباً بلاستيكياً صغيراً بين الأسنان الأمامية، ويُسند الجبين والذقن، ويُصوّب الفنّي أشعّة الضبط الضوئية لتوازي الفكّ ومنتصف الوجه، فدقّة الوضع هي أهمّ عامل في جودة الصورة.

### 4. التقاط الصورة أثناء دوران الجهاز

يُطلب منك إغلاق الشفتين وإلصاق اللسان بسقف الحنك والبقاء ساكناً تماماً وأنت تتنفّس بهدوء، بينما يدور الذراع حول الرأس ثوانيَ قليلة. الحركة أثناء الدورة أكثر أسباب إعادة التصوير.

### 5. مراجعة الجودة والقراءة والخطة

تُراجع جودة الصورة فوراً، ثم يقرأها الطبيب مع الفحص السريري ويشرح لك ما ظهر فيها، ويحدّد إن كانت كافية أم تحتاج صوراً صغيرة داخل الفم أو تصويراً مقطعياً مخروطياً، ثم يبني خطة العلاج.

## Procedure steps

### 1. Establishing the indication and reviewing previous images

The dentist defines the diagnostic question the image will answer, reviews any recent previous radiographs to avoid unnecessary repetition, asks about pregnancy or its possibility, and explains why the image is requested and exactly what will be looked for.

### 2. Removing metal and applying the shield

Earrings, piercings, hair clips, necklaces, spectacles, removable dentures and any tongue or lip jewellery are removed, since they cast shadows that obscure detail. A protective shield is then applied according to the clinic's protocol.

### 3. Precise head positioning

You stand or sit at the machine and bite a small plastic rod between the front teeth while the forehead and chin are supported. The operator aligns the light guides with the jaw and facial midline, since positioning accuracy is the single biggest factor in image quality.

### 4. Capturing the image as the machine rotates

You are asked to close the lips, press the tongue to the palate, breathe quietly and stay completely still while the arm rotates around your head for a few seconds. Movement during the rotation is the commonest reason an image has to be repeated.

### 5. Quality check, reporting and treatment plan

Image quality is checked immediately, then the dentist reads it alongside the clinical examination, explains the findings, decides whether it is sufficient or whether small intraoral films or cone beam imaging are needed, and builds the treatment plan.

## التحضير قبل الإجراء

أخبر الطبيب إن كنتِ حاملاً أو تشكّين بالحمل أو مرضعة، فهذا يغيّر قراره في التوقيت وفي بدائل التصوير. أخبره أيضاً بأي صورة شعاعية للأسنان أُخذت لك حديثاً وأحضرها إن كانت بحوزتك، لأن الصورة الموجودة قد تُغني عن أخرى. انزع قبل التصوير كل المعادن من الرأس والرقبة: الأقراط والحلق ومشابك الشعر والقلائد والنظارات، وانزع طقم الأسنان المتحرّك وأدوات التقويم القابلة للإزالة. اربط الشعر الطويل خارج منطقة التصوير. لا يلزم صيام ولا توقّف عن أي دواء. إن كان المريض طفلاً صغيراً أو يخاف الأجهزة فاطلب دخول أحد مرافقيه معه لطمأنته مع درع واقٍ، واشرح للطفل أن الجهاز يدور ولا يلمسه ولا يُوجعه.

## العناية بعد الإجراء

لا تحتاج الصورة البانورامية إلى أي عناية بعدها ولا فترة نقاهة، وتعود إلى نشاطك وطعامك وأدويتك فوراً، ولا تحتاج راحة ولا سوائل خاصة. ونصحّح خرافة شائعة: لا يبقى إشعاع في الجسم بعد التصوير الشعاعي، فالأشعّة السينية لا تتراكم في الأنسجة كما يظنّ البعض ولا تنتقل منك إلى أحد، والرضاعة الطبيعية لا تتوقّف بسببه. احصل على نسخة رقمية من الصورة واحتفظ بها مع تاريخها، لأن إبرازها في أي عيادة أخرى قد يوفّر عليك تصويراً جديداً. اطلب من الطبيب أن يشرح لك ما ظهر بلغة واضحة، ولا تعتمد قراءة الصورة وحدها لأي قرار. راجع بلا انتظار عند: \*\*تورّم في الوجه ينتشر إلى العين أو الرقبة\*\*، \*\*صعوبة في البلع أو التنفّس أو فتح الفم\*\*، \*\*حرارة وقشعريرة مع ألم فكّ أو سنّ\*\*، \*\*ألم شديد متزايد لا يستجيب\*\*. هذه علامات خمج فموي منتشر ولا تنتظر موعداً.

## About this service

A dental panoramic radiograph (OPG) is a single X-ray image that shows both jaws in one view: all erupted and unerupted teeth, the jaw bone, the jaw joints, the lower part of the sinuses, and the relationship between tooth roots and nerve canals. It is taken with a machine that rotates slowly around the head while the patient stays still, producing a flattened strip image of a curved region.

It is requested when small intraoral films are not enough: assessing a wisdom tooth before extraction, following the development of a child's dentition and any extra or missing teeth, orthodontic planning, evaluating jaw fractures, looking for cysts or bone changes, and an initial survey before planning implants or dentures. Its real value is that it gives the big picture quickly and at a low radiation dose.

About radiation, an honest reassurance: the dose in panoramic imaging is low, and the image is not taken as a routine ritual. It is justified by a clear diagnostic question that neither the eye nor the clinical examination alone can answer. Exposure is reduced further by using a protective shield, careful positioning, selecting the smallest field that answers the question, and reviewing previous images before repeating any radiograph. Tell your dentist if you are pregnant or think you might be, so the necessity and timing can be weighed and the image postponed if postponement is reasonable.

Its limits matter. A panoramic film is a broad survey but low in fine detail: it does not reliably show early decay between teeth — that is the job of small intraoral films — and it does not provide trustworthy three-dimensional measurements for implant planning or for defining the exact relationship of a root to a nerve, where cone beam imaging is requested instead. It also contains distortion, magnification and overlapping shadows that can be misread if interpreted apart from the clinical examination. For that reason both the decision to take the image and its interpretation stay with the dentist who has examined you.

## Preparation

Tell the dentist if you are pregnant, might be pregnant, or are breastfeeding, since this changes decisions about timing and alternatives. Mention any dental radiograph taken recently and bring it if you have it, because an existing image may make another unnecessary. Before the exposure, remove all metal from the head and neck: earrings, piercings, hair clips, necklaces and spectacles, and take out removable dentures and removable orthodontic appliances. Tie long hair away from the imaging area. No fasting is required and no medicine needs to be stopped. If the patient is a young child or is frightened by equipment, ask whether an accompanying adult may stay with a protective shield, and explain to the child that the machine turns around the head without touching or hurting him.

## Aftercare

A panoramic radiograph needs no aftercare and no recovery period: you return to your normal activity, food and medicines immediately, with no need for rest or special fluids. One common myth deserves correction: no radiation stays in the body after an X-ray. X-rays do not accumulate in tissue as some people believe, nothing is transmitted from you to anyone else, and breastfeeding does not need to be interrupted because of it. Obtain a digital copy of the image and keep it with its date, since showing it at another clinic may save you a repeat exposure. Ask the dentist to explain the findings in plain language, and never base a decision on the image alone. \*\*Seek care without delay if facial swelling spreads towards the eye or neck\*\*, \*\*if swallowing, breathing or opening the mouth becomes difficult\*\*, \*\*if fever and chills accompany jaw or tooth pain\*\*, or \*\*if pain becomes severe and progressive\*\*. These indicate spreading oral infection and must not wait for an appointment.
