وللعلاج ثلاثة مسارات تُختار بالتشارك بينك وبين طبيبك: أدوية مضادّة للغدة تُخفّض الإنتاج وتُعطى لشهور مع احتمال انتكاس بعد إيقافها في داء غريفز؛ أو اليود المشعّ الذي يقلّص النسيج المفرط النشاط تدريجياً على أسابيع إلى أشهر وقد يحتاج جرعة ثانية، ولا يُعطى في الحمل والرضاعة ويُنصح بتأجيل الحمل بعده مدّة يحدّدها الفريق؛ أو الجراحة التي تُستطبّ عند الغدة الكبيرة الضاغطة أو العقيدة المشتبهة أو فشل الخيارين الآخرين. وتُضاف عادة أدوية تضبط سرعة النبض والرجفة سريعاً حتى يبدأ العلاج الأساسي بالعمل.

ومن حدود العلاج التي يجب معرفتها: لا الأدوية ولا اليود المشعّ يعالج جحوظ العين في داء غريفز — تلك مشكلة منفصلة لها تدبيرها، ويسوء وضعها بالتدخين تحديداً فالإقلاع جزء من العلاج. واليود المشعّ والجراحة يُتوقّع أن يتبعهما خمول دائم يحتاج تعويضاً هرمونياً مدى الحياة، وهذا نتيجة متوقّعة لا فشل. و\*\*لا تُخفّض الأعشاب ولا الحجامة ولا حذف الملح نشاط الغدة\*\*، وإيقاف الدواء من تلقاء نفسك عند التحسّن يعيد الفرط وقد يُمهّد لعاصفة درقية وهي حالة طارئة. والترشيح لكل مسار يُحدَّد بالفحص السريري والفحوص لا بالقراءة.

## خطوات الإجراء

### 1. تأكيد التشخيص وتحديد السبب

تُقاس هرمونات الغدة والهرمون المحفّز لها، وتُطلب أجسام مضادّة وسونار للغدة، وقد يُطلب مسح نووي للتمييز بين داء غريفز والعقيدة المفرطة والتهاب الغدة. هذه الخطوة ليست تفصيلاً: المسار العلاجي يتغيّر كلياً بحسب السبب.

### 2. ضبط الأعراض سريعاً

تُعطى أدوية تُهدّئ تسارع النبض والرجفة والتوتّر ما لم يوجد مانع طبّي، وتُصحَّح العوامل المفاقمة: مصادر اليود الزائدة، ومنبّهات كالكافيين، والتدخين خاصّة مع إصابة العين. وتُعالج مضاعفات مثل الرجفان الأذيني إن وُجدت.

### 3. اختيار المسار العلاجي

يُقارَن بين الأدوية المضادّة للغدة واليود المشعّ والجراحة بحسب عمرك وحجم الغدة ووجود عقيدة مشتبهة وإصابة العين والحمل أو التخطيط له وأمراضك المصاحبة ورغبتك بعد شرح ما بعد كل خيار. ويُذكر صريحاً أن اليود والجراحة يتبعهما خمول متوقّع.

### 4. المتابعة المخبرية وتعليم الإنذار

تُعاد فحوص الغدة كل عدّة أسابيع في البداية ثم تتباعد مع الاستقرار، وتُعدَّل الخطّة على النتائج لا على الإحساس. وتُعلَّم علامات تستوجب فحص دم عاجلاً مثل التهاب الحلق مع الحرارة، أو اليرقان وألم أعلى البطن، مع تعليمات صريحة بعدم إيقاف الدواء ذاتياً.

### 5. الرصد على المدى الطويل

بعد اليود المشعّ أو الجراحة يُراقب ظهور الخمول ويُبدأ التعويض الهرموني ويُعدَّل دورياً مدى الحياة. وبعد إيقاف الأدوية في داء غريفز تُتابع الفحوص لكشف الانتكاس أو الخمول المتأخّر، وتُتابع العينان وكثافة العظام والقلب عند من تأثّرت عندهم.

## Procedure steps

### 1. Confirming the diagnosis and its cause

Thyroid hormones and thyroid-stimulating hormone are measured, antibodies and thyroid ultrasound are requested, and a nuclear scan may be needed to distinguish Graves' disease from an overactive nodule or thyroiditis. This step is not a detail: the entire treatment path depends on the cause.

### 2. Rapid symptom control

Medication is given to settle a racing pulse, tremor and anxiety unless contraindicated, and aggravating factors are corrected: excess iodine sources, stimulants such as caffeine, and smoking, especially where the eyes are involved. Complications such as atrial fibrillation are treated if present.

### 3. Selecting the definitive treatment

Antithyroid drugs, radioactive iodine and surgery are compared according to your age, gland size, any suspicious nodule, eye involvement, pregnancy or plans for it, coexisting illness and your own preference after the aftermath of each option is explained. It is stated plainly that iodine and surgery are followed by expected underactivity.

### 4. Laboratory follow-up and warning signs

Thyroid tests are repeated every few weeks at first and then spaced out as things stabilise, with the plan adjusted to results rather than to how you feel. You are taught the signs that require urgent blood testing, such as sore throat with fever, or jaundice with upper abdominal pain, and instructed never to stop medication on your own.

### 5. Long-term monitoring

After radioactive iodine or surgery, the emergence of underactivity is monitored and hormone replacement is started and adjusted lifelong. After antithyroid drugs are stopped in Graves' disease, testing continues to detect relapse or delayed underactivity, and the eyes, bone density and heart are followed in those affected.

## العناية بعد الإجراء

خذ دواءك في نفس الوقت يومياً ولا توقفه ولا تعدّل مقداره من تلقاء نفسك حتى لو زالت الأعراض، والتزم بمواعيد فحوص الدم لأنها هي التي تقود العلاج. سجّل وزنك ونبضك ودوّن أي أعراض جديدة، وأقلع عن التدخين تماماً خصوصاً إن تأثّرت عيناك، واستخدم قطرات مرطّبة ونظارة شمسية عند جفاف العين. وإن أخذت اليود المشعّ فالتزم بتعليمات الوقاية الإشعاعية التي يعطيها الفريق مدّة محدّدة. \*\*راجع الطوارئ فوراً عند حرارة عالية مع تخبّط أو ارتباك ونبض شديد السرعة وقيء (شبهة عاصفة درقية)، أو التهاب حلق مع حرارة أو تقرّحات فم، أو اصفرار العين والبشرة وبول غامق وألم أعلى البطن، أو ألم صدر أو ضيق نفس، أو ألم شديد في العين أو ازدواج رؤية أو تراجع في البصر.\*\*

## About this service

Treating an overactive thyroid means managing a state in which the gland produces more hormone than the body needs, so bodily functions accelerate: palpitations and a fast pulse, weight loss despite a good appetite, hand tremor, sweating and heat intolerance, anxiety and poor sleep, more frequent bowel movements, thigh muscle weakness and menstrual disturbance. The first step is not medication but identifying the cause, because treatment differs entirely according to it.

The commonest causes are Graves' disease, an autoimmune condition that may involve the eyes, followed by one or more overactive nodules, and thyroiditis, which produces a temporary excess. That last point matters: \*\*hyperthyroidism caused by thyroiditis is not treated with antithyroid drugs\*\*, because it is a transient phase managed symptomatically and often followed by a temporary underactive phase. Other causes include taking more thyroid hormone than needed, or an iodine load from contrast dye or certain medicines. Assessment therefore includes thyroid-stimulating hormone and free hormone levels, antibody testing, thyroid ultrasound and, when needed, a nuclear scan.

Three treatment paths are chosen jointly with your doctor. Antithyroid medication reduces hormone production and is given for months, with a real chance of relapse after stopping in Graves' disease. Radioactive iodine shrinks overactive tissue gradually over weeks to months and may require a second dose; it is not given in pregnancy or breastfeeding, and pregnancy is postponed afterwards for a period your team specifies. Surgery is indicated for a large compressive gland, a suspicious nodule, or when the other options fail or are unsuitable. Medication to slow the pulse and reduce tremor is usually added early, while the main treatment takes effect.

Some limits should be known in advance. Neither medication nor radioactive iodine treats the eye disease of Graves' disease, which is a separate problem with its own management and is made distinctly worse by smoking, so stopping smoking is part of treatment rather than advice. Radioactive iodine and surgery are both expected to be followed by permanent underactivity requiring lifelong hormone replacement, which is an anticipated outcome and not a failure. And \*\*herbs, cupping and cutting out salt do not lower thyroid activity\*\*. Stopping your medicine on your own once you feel better allows the excess to return and can open the door to thyroid storm, a medical emergency. Suitability for each path is determined by clinical assessment and testing rather than by reading.

## Preparation

Bring all your previous thyroid results with their dates and a list of your medicines and supplements, mentioning specifically anything containing iodine, herbal thyroid products or biotin — biotin interferes with thyroid assays and must be stopped before the blood draw for the period your laboratory specifies. Tell your doctor if you have recently had a contrast-enhanced scan, since an iodine load affects both tests and any nuclear scan. Record your pulse and weight at home for a week beforehand if you can. Tell your doctor if you are pregnant, breastfeeding or planning pregnancy soon, as this fundamentally changes the options. Fasting is not usually needed for thyroid tests, but ask if other tests are requested alongside them.

## Aftercare

Take your medicine at the same time each day and never stop it or change the amount yourself, even if symptoms have gone, and keep every blood-test appointment, because the results steer the treatment. Record your weight and pulse and note any new symptom, stop smoking completely — especially if your eyes are affected — and use lubricating drops and sunglasses for dry eyes. If you receive radioactive iodine, follow the radiation-safety instructions your team gives for the stated period. \*\*Go to emergency care immediately for high fever with agitation or confusion, a very fast pulse and vomiting (possible thyroid storm), sore throat with fever or mouth ulcers, yellowing of the eyes or skin with dark urine and upper abdominal pain, chest pain or breathlessness, or severe eye pain, double vision or falling vision.\*\*
