### 2. تقدير الخطر القلبي الوعائي

يجمع الطبيب النتيجة مع العمر والتدخين وضغط الدم والسكري والوزن والتاريخ العائلي، ويفحص القلب والنبض في الأطراف ويسأل عن ألم الصدر أو ألم الساق عند المشي. هذا التقدير هو ما يحدّد شدّة العلاج ومدى الحاجة إلى دواء، لا الرقم منفرداً، وقد يُطلب تصوير أو تخطيط قلب عند وجود أعراض.

This risk estimate, not the number in isolation, determines how intensively to treat and whether medication is needed; imaging or an electrocardiogram may be added when symptoms are present.

High cholesterol means an excess of fats circulating in the blood, chiefly low-density lipoprotein cholesterol, which deposits in the artery wall and narrows it slowly over years. The condition is usually silent: there is no pain and no symptom to announce it, and it is detected by a blood lipid profile. Feeling well is therefore no reassurance, and waiting for a symptom to appear is not a strategy.

Treating high cholesterol is not about chasing a number on a laboratory report; it is about reducing the chance of heart attack, stroke and blocked limb arteries. The doctor therefore starts by estimating total risk: age, smoking, blood pressure, diabetes, weight and waist circumference, physical activity, kidney disease, and a family history of early cardiovascular disease. Two people with an identical number may be managed differently, one needing medication early and the other lifestyle change with monitoring; eligibility for treatment is decided by clinical assessment and risk estimation, not by the result alone.
