يُلجأ إليه عندما يفشل الترويض السلوكي أو يكون غير واقعي: أطفال صغار جداً لديهم تسوّس واسع في أسنان كثيرة، خوف شديد يمنع أي تدبير في العيادة، أطفال ذوو احتياجات خاصة أو صعوبة في التواصل أو حركة لا يمكن السيطرة عليها، منعكس تقيّؤ شديد، التهاب واسع يجعل التخدير الموضعي غير كافٍ، أو حالات تحتاج جلسات عديدة سيكون تكرارها أشدّ إرهاقاً للطفل نفسياً وجسدياً. القرار مشترك بين طبيب أسنان الأطفال وطبيب التخدير والأهل، ويُبنى على تقييم طبي وسنّي كامل لا على الرغبة في اختصار الوقت.

ما يُنجَز في الجلسة يُخطَّط مسبقاً بالكامل: حشوات، بتر لبّ، تلبيسات، خلع للأسنان غير القابلة للإنقاذ، حافظات مسافة، تنظيف وفلورايد وسدّ شقوق. ولأن الطفل نائم يمكن العمل بجودة عالية بحاجز مطاطي وبلا استعجال، فيُقلّل ذلك من الحلول المؤقّتة.

مخاطر التخدير العام عند الأطفال الأصحّاء منخفضة في مكان مجهّز وبفريق مختصّ، لكنها ليست صفراً، وأشيع ما يحدث بسيط وعابر: غثيان أو قيء، بحّة أو التهاب حلق، ارتجاف وبرد، دوار وبكاء عند الاستفاقة، وألم موضع العلاج. المضاعفات التنفّسية أو الدورانية نادرة وتُدار في هذا الإعداد بالذات، ولهذا يُشترط تقييم ما قبل التخدير والصيام الدقيق.

وحدود الإجراء تُقال بوضوح: التخدير العام يجعل العلاج ممكناً ولا يعالج سبب التسوّس. الطفل الذي يعود إلى الزجاجة الليلية والسكّر المتكرّر وبلا تفريش بإشراف بالغ يعود إليه التسوّس في الأسنان التي لم تُعالَج وحول ما عُولج. كما أن الجلسة لا تُغني عن تدريب الطفل تدريجياً على زيارات العيادة العادية بعدها، ولا يجوز اعتبارها اختصاراً دائماً لكل زيارة أسنان.

## خطوات الإجراء

### 1. التقييم السنّي ووضع خطة كاملة

يُفحص الفم كلّه وتُؤخذ الصور اللازمة لتُوضع خطة نهائية لكل سنّ قبل يوم العملية، مع بدائل جاهزة إن تبيّن داخل الغرفة أن سنّاً غير قابلة للترميم، وتُشرح الخطة وتكاليفها الزمنية للأهل ويُوقَّع الرضا المستنير.

### 2. تقييم ما قبل التخدير

يقابل طبيب التخدير الطفل وأهله لمراجعة الوزن والتاريخ المرضي والربو والحساسية والأدوية والتشنّجات وأي عملية سابقة أو مشكلة عائلية مع التخدير، ويُفحص التنفّس والقلب واللوزتان، وتُطلب تحاليل عند الحاجة فقط.

### 3. الصيام والتحضير في يوم العملية

يُلتزم بتعليمات الصيام التي يحدّدها فريق التخدير، وهي عادة توقّف الطعام الصلب والحليب قبل نحو ست ساعات والسوائل الصافية قبل نحو ساعتين. يُسجَّل الطفل، ويُقاس وزنه وحرارته، ويُعطى تمهيد مهدّئ عند الحاجة قبل الدخول.

### 4. التنويم وإنجاز العلاجات في جلسة واحدة

يُنوَّم الطفل ويُؤمَّن مجرى الهواء وتُراقَب علاماته الحيوية باستمرار، ثم تُنجَز كل الإجراءات المخطّطة بالترتيب مع عزل جيّد وتخدير موضعي مساعد لتقليل الألم بعد الاستفاقة، وتُوثَّق الحالة النهائية بالصور.

### 5. الإفاقة والمراقبة والخروج

يُراقَب الطفل في غرفة الإفاقة حتى يستعيد وعيه ومنعكساته ويشرب بأمان، ويُدار الألم والغثيان. يُخرَج في اليوم نفسه غالباً بتعليمات مكتوبة وموعد متابعة، مع خطة وقاية وجدول مراجعات كل 3 إلى 6 أشهر.

## Procedure steps

### 1. Dental assessment and a complete plan

The whole mouth is examined and the necessary radiographs are taken so that a definitive plan for every tooth is set before the day of surgery, with ready alternatives if a tooth turns out to be unrestorable in theatre. The plan is explained to the parents and informed consent is obtained.

### 2. Pre-anaesthetic assessment

The anaesthetist meets the child and parents to review weight, medical history, asthma, allergies, medications, seizures, previous operations, and any family problem with anaesthesia, examines the chest, heart, and tonsils, and requests blood tests only when indicated.

### 3. Fasting and preparation on the day

The fasting instructions set by the anaesthetic team are followed, typically stopping solids and milk about six hours before and clear fluids about two hours before. The child is admitted, weight and temperature are recorded, and premedication is given if needed before going in.

### 4. Anaesthesia and completing treatment in one session

The child is anaesthetised, the airway is secured, and vital signs are monitored continuously. All planned procedures are then completed in order with good isolation and supplementary local anaesthesia to reduce pain after waking, and the final result is documented with images.

### 5. Recovery, monitoring, and discharge

The child is monitored in recovery until consciousness and reflexes return and fluids are tolerated safely, with pain and nausea managed. Discharge is usually the same day with written instructions and a follow-up appointment, plus a prevention plan and reviews every 3 to 6 months.

## التحضير قبل الإجراء

التزم بتعليمات الصيام كما يحدّدها فريق التخدير حرفياً، ولا تعطِ طفلك حلوى أو علكاً أو رشفة عصير خارج المسموح لأن ذلك قد يؤجّل العملية لسلامته. أخبر الفريق قبل الموعد بأي كحّة أو زُكام أو حرارة أو نزلة صدرية في الأيام السابقة، فقد يُنصح بالتأجيل. اذكر الوزن الحالي، وكل الأدوية والمكمّلات وأدوية الربو، والحساسية من أي دواء أو لاصق أو مطاط، وأي تشنّجات أو مشاكل قلبية أو تنفّسية أو شخير شديد وتوقّف تنفّس أثناء النوم، وأي عملية سابقة تحت تخدير أو مشكلة تخدير في العائلة. أحضر تقارير طفلك وصوره الشعاعية، وألبسه ثياباً واسعة مريحة، وأزل الأساور والحلي وطلاء الأظافر. أحضر ملابس تغيير ولعبته المفضّلة، ورتّب حضور بالغَين إن أمكن أحدهما يجلس مع الطفل والآخر ينهي الإجراءات، ولا يقود الطفل أي وسيلة ولا يعود إلى الحضانة في اليوم نفسه.

## العناية بعد الإجراء

توقّع أن يكون طفلك ناعساً أو منزعجاً أو باكياً بعض الساعات، وهذا طبيعي بعد التخدير؛ احمله واطمئنه ولا تتركه بلا مراقبة ولا ينام على وجهه بعمق دون ملاحظة. ابدأ بسوائل صافية بكمّيات صغيرة ثم أطعمة طرية باردة، وتجنّب الحارّ والقاسي واللاصق في اليوم الأول، ولا تستخدم الشلمونة إن أُجري خلع. أعطِ المسكّن بالجرعة الموصوفة لوزن طفلك في مواعيدها لأن انتظار الألم يجعله أصعب، ولا تضف مسكّناً آخر من نفسك. قد تظهر بحّة أو التهاب حلق أو حرارة منخفضة عابرة أو كدمة على اليد من الوريد، وهذا شائع ويزول. امنع الجري والملاعب والحمّام بلا مساعدة في اليوم الأول، وامنع الحضانة والمدرسة يومَه. أعِد التفريش اللطيف بإشراف بالغ من اليوم التالي، وابدأ فوراً بخطة الوقاية وإيقاف الزجاجة الليلية. \*\*راجع الطوارئ فوراً عند صعوبة تنفّس أو صفير أو ازرقاق، أو نزف فموي لا يتوقّف بالضغط بشاش عشر دقائق، أو قيء متكرّر يمنع الشرب، أو حرارة عالية مع خمول شديد، أو عدم استفاقة كاملة أو ارتباك مستمر، أو تورّم في الوجه أو الرقبة أو صعوبة بلع.\*\*

## About this service

Dental treatment of children under general anaesthesia means completing all necessary dental work in a single session in a properly equipped operating theatre, while the child is fully asleep under the care of an anaesthetist with continuous monitoring of breathing, heart, and oxygen levels. It is not a type of treatment in itself but a way of making necessary treatment possible and safe.

It is chosen when behavioural management fails or is unrealistic: very young children with extensive decay in many teeth, severe fear that prevents any chairside care, children with special needs, communication difficulties, or uncontrollable movement, a strong gag reflex, widespread infection that makes local anaesthesia insufficient, or cases needing so many appointments that repeating them would be more distressing physically and emotionally. The decision is shared between the paediatric dentist, the anaesthetist, and the parents, and is based on a full medical and dental assessment rather than on a wish to save time.

Everything to be done is planned in advance: fillings, pulp treatment, crowns, extraction of teeth that cannot be saved, space maintainers, cleaning, fluoride, and fissure sealants. Because the child is asleep, the work can be done to a high standard with rubber dam isolation and without rushing, which reduces reliance on temporary solutions.

The risks of general anaesthesia in healthy children are low in an equipped facility with a specialist team, but they are not zero. The most common effects are minor and short-lived: nausea or vomiting, hoarseness or sore throat, shivering and feeling cold, dizziness and crying on waking, and soreness at the treated sites. Respiratory or circulatory complications are rare and are precisely what this setting is prepared to manage, which is why pre-anaesthetic assessment and strict fasting are required.

The limits are stated plainly. General anaesthesia makes treatment possible; it does not treat the cause of decay. A child who returns to a night-time bottle, frequent sugar, and brushing without adult supervision will develop new decay in untreated teeth and around what was restored. The session also does not replace gradually training the child to accept ordinary clinic visits afterwards, and it should not be regarded as a permanent shortcut for every dental appointment.

## Preparation

Follow the fasting instructions from the anaesthetic team exactly, and do not give your child sweets, gum, or a sip of juice outside what is allowed, as this may postpone the procedure for their safety. Tell the team before the date about any cough, cold, fever, or chest infection in the preceding days, since postponement may be advised. Report the current weight, all medications and supplements including asthma inhalers, allergies to any drug, plaster, or latex, any seizures, heart or breathing problems, heavy snoring or pauses in breathing during sleep, and any previous anaesthetic or family anaesthetic problem. Bring reports and radiographs, dress the child in loose comfortable clothes, and remove jewellery and nail polish. Bring a change of clothes and a favourite toy, and if possible arrange for two adults to attend, one to stay with the child and one to complete paperwork. The child should not return to nursery on the same day.

## Aftercare

Expect your child to be drowsy, unsettled, or tearful for some hours; this is normal after anaesthesia. Hold and reassure them, do not leave them unsupervised, and do not let them sleep face-down without observation. Start with small amounts of clear fluids, then cool soft food, avoiding hot, hard, and sticky items on the first day, and do not use a straw if a tooth was extracted. Give pain relief at the dose prescribed for your child's weight and on schedule, because waiting until pain builds makes it harder to control, and do not add another painkiller yourself. Hoarseness, a sore throat, a mild transient temperature, or a bruise on the hand from the drip are common and settle. Avoid running, playgrounds, and unsupervised bathing on the first day, and keep the child off nursery or school that day. Resume gentle supervised brushing from the next day, and start the prevention plan and stopping night-time bottles immediately. \*\*Go to emergency care at once for breathing difficulty, wheeze or blue discolouration, oral bleeding that does not stop after ten minutes of gauze pressure, repeated vomiting preventing drinking, high fever with marked lethargy, incomplete waking or ongoing confusion, or swelling of the face or neck with difficulty swallowing.\*\*
