إعادة التغذية بعد نقص شديد ليست مجرد زيادة طعام: تحتاج تدرّجاً ومراقبة طبية لأملاح الدم في الأيام الأولى، لأن التسرّع قد يسبّب اضطراباً خطيراً في الأملاح والقلب. أما الجانب النفسي فيعمل على أنماط التفكير حول الوزن والطعام، وعلى سلوكيات التعويض والتقيّؤ والإفراط في الرياضة، وعلى إشراك الأسرة — وهو أساسي خصوصاً مع المراهقين.

وهناك حدود وخرافات يجب تصحيحها بوضوح: اضطراب الأكل ليس اختياراً ولا رغبة في الرشاقة ولا قلّة إرادة، ولا يُعالج بالتوبيخ أو الحمية أو مراقبة الطعام في البيت. ولا يشترط أن يكون الشخص نحيلاً جداً ليكون مصاباً — فالاضطراب يصيب كل الأوزان ويصيب الذكور أيضاً. والتعافي لا يُقاس بالوزن وحده بل بعلاقة الشخص بالطعام وجسده وعودته إلى حياته. ولا يوجد دواء يُعالج اضطراب الأكل بمفرده.

## خطوات الإجراء

### 1. تقييم شامل نفسي وجسدي وتغذوي

يُوثَّق نمط الأكل وسلوكيات التقييد أو الشراهة أو التعويض، وتُقاس العلامات الحيوية والوزن والطول، ويُجرى فحص سريري وتحاليل للأملاح ووظائف الكلى والكبد وصورة الدم وتخطيط قلب، ويُسأل عن الدورة الشهرية والإغماء والدوار.

### 2. تحديد مستوى الرعاية المناسب

يُقرَّر بحسب الخطورة: متابعة في العيادة، أو برنامج نهاري مكثّف، أو إدخال إلى المستشفى عند عدم استقرار العلامات الحيوية أو نقص الأملاح أو نقص وزن سريع أو خطر انتحاري. هذا القرار طبي بحت ولا يُترك لرغبة الشخص أو لتقدير الأسرة وحدها.

### 3. الاستقرار الطبي وإعادة التغذية المراقبة

تُصحَّح الأملاح والسوائل، ثم تُبنى خطة تغذية تُزاد تدريجياً بإشراف أخصائي مع مراقبة أملاح الدم والنبض في الأيام الأولى تجنّباً لمضاعفات إعادة التغذية. تُعالج أيضاً آثار جسدية شائعة مثل تسوّس الأسنان وارتجاع المعدة والإمساك.

### 4. العلاج النفسي وإشراك الأسرة

يعمل العلاج على الخوف من الزيادة وصورة الجسم وقواعد الطعام الصارمة وسلوكيات التقيّؤ والإفراط في الرياضة، ويُعالج القلق أو الاكتئاب المصاحب. تُدرَّب الأسرة على دعم الوجبات بلا مراقبة عدائية ولا تعليق على الشكل والوزن، وهو محور أساسي في علاج المراهقين.

### 5. الوقاية من الانتكاس والمتابعة الطويلة

تُحدَّد محرّضات الانتكاس الشخصية مثل الضغط والمقارنة على وسائل التواصل والحميات، وتُكتب خطة نكسة واضحة. تستمر المتابعة أشهراً بعد استقرار الوزن والسلوك، لأن التعافي النفسي غالباً يتأخّر عن التعافي الجسدي.

## Procedure steps

### 1. Comprehensive psychological, medical and nutritional assessment

Eating patterns and behaviours of restriction, bingeing or compensation are documented, vital signs, weight and height are measured, and physical examination is carried out with tests for electrolytes, kidney and liver function, a blood count and an ECG, plus questions about periods, fainting and dizziness.

### 2. Deciding the appropriate level of care

This is decided by severity: outpatient follow-up, an intensive day programme, or hospital admission when vital signs are unstable, electrolytes are low, weight loss is rapid or suicide risk is present. It is a strictly medical decision and is not left to preference or to the family's judgement alone.

### 3. Medical stabilisation and monitored refeeding

Electrolytes and fluids are corrected, then a nutrition plan is built and increased gradually under a dietitian's supervision, with monitoring of blood salts and pulse in the first days to avoid refeeding complications. Common physical effects such as dental erosion, reflux and constipation are treated as well.

### 4. Psychological treatment and family involvement

Therapy addresses fear of weight gain, body image, rigid food rules, vomiting behaviours and excessive exercise, and treats any co-occurring anxiety or depression. Families are coached to support meals without hostile surveillance and without comments about shape or weight — a central element in treating adolescents.

### 5. Relapse prevention and long-term follow-up

Individual relapse triggers such as stress, social media comparison and dieting are identified and a clear setback plan is written. Follow-up continues for months after weight and behaviour have stabilised, because psychological recovery usually lags behind physical recovery.

## About this service

Treatment for eating disorders is a team-based plan, because these conditions are psychological and physical at the same time: eating patterns and a person's relationship with their body and weight change in ways that genuinely threaten health. The group includes anorexia nervosa, bulimia nervosa, binge eating disorder and avoidant/restrictive food intake disorder.

The team usually brings together a psychiatrist, a dietitian and a physician for the medical side, sometimes with a dentist or paediatrician. The reason for this range is simple: psychotherapy alone cannot correct electrolyte disturbance or an irregular heartbeat, and nutrition alone cannot correct body image and fear of eating. Both sides are treated in parallel.

The first decision in the plan is the level of care: outpatient follow-up, an intensive day programme, or hospital admission when vital signs are unstable, weight loss is rapid or suicide risk is high. Suitability is determined by clinical examination, laboratory tests and an ECG — never by the number on the scale alone.

Refeeding after severe restriction is not simply eating more. It requires a gradual approach and medical monitoring of blood electrolytes in the first days, because moving too fast can cause a dangerous disturbance of salts and heart rhythm. The psychological side works on thinking patterns about weight and food, on compensatory behaviours such as vomiting and excessive exercise, and on involving the family, which is essential particularly with adolescents.

Several limits and myths need correcting plainly. An eating disorder is not a choice, not a wish to be slim and not a lack of willpower, and it is not treated by scolding, dieting or policing food at home. A person does not have to be extremely thin to be affected — these disorders occur at every body weight and affect males too. Recovery is not measured by weight alone but by a person's relationship with food and their body and their return to life. And no medication treats an eating disorder on its own.

## Preparation

Come to the appointment with an honest picture of the past week: what you actually ate, whether restriction or binge episodes occurred, whether vomiting, laxatives, diuretics, excessive exercise or slimming products were used and how often, together with a list of all supplements and herbal products taken. Note your weight if you know it and any recent change, plus any fainting, dizziness, palpitations, muscle weakness, hair loss or loss of periods. Bring previous tests and reports. If the person affected is an adolescent, a parent should attend, because treatment needs the family as a partner. Before the visit, agree at home to suspend all talk about weight, shape and dieting — that conversation makes treatment harder from day one.

## Aftercare

Stick to the meal plan and its timing even on days when you feel no hunger or when anxiety rises, and keep both the nutrition appointments and the medical and psychological follow-up rather than only one of them. Get rid of the home scale if it leads you to weigh yourself daily, reduce exposure to dieting and body-focused accounts, and ask your family to stop commenting on your shape or your eating. \*\*Seek help immediately for fainting or severe dizziness, palpitations or a fast or slow pulse, chest pain, muscle cramps, marked muscle weakness, vomiting blood, severe abdominal pain, low body temperature or confusion, or rapid weight loss over a few days\*\* — these can signal an electrolyte or cardiac disturbance and are an emergency. \*\*If thoughts of harming yourself or of suicide arise, get urgent help straight away: call emergency services, go to the nearest emergency department, or tell someone you trust right now. Do not wait for your next appointment.\*\* Do not stop any medication prescribed for co-occurring anxiety or depression on your own; discontinuation follows a gradual medical plan, and psychotherapy and medication are not rivals.
