## Procedure steps

### 1. Medical assessment and exclusion of organic causes

The child is examined and growth charted, while physical explanations for the avoidance are sought: reflux, food allergy, a swallowing problem, enlarged tonsils, painful dental decay, chronic constipation, or nutrient deficiencies measured by tests as indicated.

### 2. Psychological, behavioural and sensory assessment

A map of accepted and refused foods is drawn and the type of avoidance identified: sensory, fear of choking or vomiting, or low interest in food. Co-occurring anxiety, autism and hyperactivity are assessed, and the mealtime atmosphere and family behaviour around it are reviewed.

### 3. Graded intervention plan and parent coaching

A graded exposure plan is built, beginning with looking at, touching and smelling a food before tasting it, alongside changes to the mealtime environment and timing. Parents are coached to offer food without pressure, conditional rewards or negotiation, with small achievable goals.

### 4. Nutritional support and treating co-occurring problems

Nutritional support is arranged to protect growth while the behavioural work proceeds, any accompanying anxiety or depression is treated, and the child is referred for occupational therapy, swallowing or speech input when an oral sensory or motor problem is present.

### 5. Follow-up and progress measurement

Progress is measured by the number of newly accepted foods, the growth curve and the family's ability to share a meal. The plan is revised if progress stalls or regresses, and care is escalated if nutritional warning signs appear.

## About this service

Avoidant/restrictive food intake disorder — often described by families as extreme picky eating — is a severely limited eating pattern that genuinely affects a child's weight, growth, nutrition or social life, and is not driven by concern about body shape or weight, which is what distinguishes it from other eating disorders. The child is not being defiant. He avoids foods because of heightened sensitivity to texture, smell or colour, because of fear of choking or vomiting after a frightening experience, or because of a markedly low interest in eating at all.

This differs from the ordinary fussiness most preschool children pass through. Here the accepted list narrows over time instead of widening, may shrink to a very small number of items, whole food groups are refused, and consequences appear in weight, height, laboratory results, or in the family's ability to share a meal or visit relatives.

Treatment is teamwork rather than a prescription: a medical assessment to exclude organic causes, a nutritional assessment, graded behavioural intervention, parent coaching, and occupational therapy or swallowing training when indicated. Its limits should be stated plainly. Treatment will not make a child eat everything, does not happen suddenly, and never succeeds through pressure — forcing, threatening and comparing with siblings increase avoidance and entrench fear. Nutritional supplements support intake and buy time, but they do not treat the behaviour by themselves and do not replace the work of expanding the diet.

Parents should also know that seeing a child psychiatrist or specialist is not a stigma and not an admission of failed parenting; it is a medical step like any other. Delay is what fixes the problem in place: the longer avoidance continues, the narrower the window for accepting new foods becomes and the greater the accompanying anxiety. Eligibility for treatment, its type and its intensity are determined by clinical examination and by growth and nutritional assessment, not by reading or general advice.

## Preparation

Before the visit, record three full days of everything the child ate and drank in detail: the time, the place, who was present, how the food was offered, and exactly what happened at refusal. Bring the growth chart from the child's health book, any previous laboratory reports, and a list of medicines, supplements and vitamins. Tell the doctor about any choking or frightening vomiting episode, food allergy, reflux, dental pain, speech difficulty or language delay, and any previous diagnosis of autism or hyperactivity. If possible bring a short video of an ordinary meal at home, which is more informative than description. Do not use the appointment as a threat or present it as punishment; explain that it is to help him feel comfortable with food.

## Aftercare

Keep mealtimes regular and sessions short and time-limited, offer one new food alongside an accepted food without requiring a taste, and repeat the offer many times, because acceptance needs calm repetition rather than confrontation. Do not force, threaten or bargain over one bite, do not use a screen to slip food into the child's mouth, and do not comment on his eating in front of others. Stop counting mouthfuls in front of him, and record progress in writing, since change is slow and easily forgotten. \*\*Seek prompt review for weight loss, faltering growth or stalled weight gain\*\*; \*\*refusal of fluids or signs of dehydration such as reduced urine, dry mouth and lethargy\*\*; \*\*fainting or repeated dizziness\*\*; \*\*choking, repeated vomiting with meals or pain on swallowing\*\*; or \*\*any talk of self-harm, marked withdrawal or loss of pleasure in everything\*\*. If you are told this is merely a parenting issue, do not postpone assessment: delay entrenches avoidance and narrows the options.
