## Procedure steps

### 1. Examination and testing flexibility

The clinician examines the toe while trying to straighten it by hand, to establish whether the deformity is flexible or rigid, a distinction that changes the whole treatment plan. The site of callus, the state of the skin, the presence of a neighbouring bunion, muscle strength and foot sensation are all assessed, and weight-bearing X-rays are then obtained to evaluate the metatarsophalangeal and toe joints.

### 2. Footwear change and local protection

The first practical step is a shoe with a high, wide toe box that does not press on the top of the toe, avoiding high heels and shoes that are too short. A silicone pad or protective sleeve is added over the toe, spacers are used where toes crowd each other, and callus is reduced by a trained professional rather than cut at home, especially if you have diabetes.

### 3. Insoles and foot exercises

An insole is prescribed to redistribute load away from the painful metatarsal head, together with exercises to extend the toe, strengthen the intrinsic muscles of the sole and stretch the Achilles tendon, since a tight calf increases pressure on the forefoot. In a flexible deformity, daily adherence over weeks is what determines the response, not a single session.

### 4. Deciding on surgery and choosing the procedure

If disabling pain persists, ulceration recurs or the deformity becomes rigid, the appropriate procedure is discussed: tendon lengthening or transfer for a flexible toe, or joint resection or fusion for a rigid one, with correction of an accompanying bunion in the same session when present, since leaving it is a frequent reason for recurrence.

### 5. Surgery and recovery

Surgery is usually performed under local anaesthesia or an ankle block while you are awake, and you go home the same day in a special postoperative shoe. A wire may be left protruding from the toe for some weeks to hold the new position and is then removed in clinic. Swelling lasts weeks to months, the toe stays stiff for a while, and return to normal footwear is gradual.

## About this service

A hammer toe is a fixed bending at one of the joints of a toe, so that it buckles upwards like a hammer instead of lying flat on the ground. The problem lies not in the toe alone but in the balance of tendons and forces acting across the forefoot, which is why it is so often accompanied by pain under the metatarsal head beneath the toe, a callus or corn on top of the joint where the shoe rubs, and sometimes skin ulceration in people who have lost protective sensation.

The most important distinction in treatment is between a flexible toe, which can still be straightened by hand, and a rigid toe that has set in its bent position and will not straighten. A flexible deformity responds to footwear change, exercises and protective padding, whereas a rigid one cannot be straightened by any non-surgical means. Common contributing factors include tight, short shoes and high heels that crowd the toes forward, a bunion that pushes the neighbouring toe out of line, muscle imbalance, nerve disorders including diabetic neuropathy, and rheumatoid arthritis.

Treatment always begins with non-surgical options: a shoe with a wide, deep toe box that does not touch the top of the toe, a silicone pad or protective sleeve to stop friction, an insole to offload the metatarsal head, stretching and strengthening exercises for the small muscles of the foot, and professional reduction of callus. The limits are explicit: these measures relieve pain and prevent skin breakdown, but they do not straighten a rigid toe and they do not halt progression while the cause continues.

When these measures fail, or the deformity is fixed and the pain interferes with walking, surgery is discussed. Options include tendon lengthening or transfer for a flexible toe, and removing part of the joint or fusing it for a rigid toe, held with a temporary wire or an internal implant. You should know that after surgery the toe may remain shorter, less mobile or permanently stiff, and that the deformity can recur if an accompanying bunion is not corrected or if you go back to the footwear that caused the problem. Which procedure suits you is determined by clinical examination and weight-bearing X-rays, not by reading.

## Preparation

Bring the shoes you actually wear every day, along with any previous foot X-rays or reports. Tell your doctor if you have diabetes, neuropathy, poor circulation or rheumatoid arthritis, and whether you take blood thinners, steroids or immunosuppressive drugs, but never stop a medication on your own. Describe what you have already tried in the way of pads, insoles or exercises and whether it helped, and mention any past wound or ulcer over the top of the toe. If surgery is planned, follow any fasting instructions given, do not apply creams to the foot on the day of the procedure, trim your nails and wash your feet thoroughly beforehand, and arrange for someone to drive you home, since you will leave in a postoperative shoe and driving is not advised. Prepare a place at home where you can keep the foot elevated.

## Aftercare

Keep the foot elevated above heart level for most of the first days after surgery, wear the prescribed postoperative shoe and do not return to your usual footwear before you are allowed to. Keep the dressing clean and dry, do not wet a fixation wire if one is in place, and do not force the toe to move. Expect swelling that lasts weeks and sometimes months, and stiffness of the toe that improves slowly. \*\*Seek review promptly for fever or shivering, foul-smelling or yellow discharge from the wound, spreading redness, pain that increases after having been settling, or numbness, pallor or blue discolouration of the toe.\*\* \*\*If you have diabetes, any wound or colour change in the foot must be seen the same day rather than waiting for an appointment.\*\* Once healed, do not go back to tight shoes or high heels, as this is the leading reason deformity returns.
