أما مثبِّطات إفراز الحمض فهي فعّالة في تهدئة الحرقة وشفاء التهاب المريء، لكن يجب أن يكون واضحاً ما تفعله وما لا تفعله: هي تُخفّف الحِدّة الحامضية للمُرتَجَع ولا تُشدّد صمّاماً ضعيفاً ولا تُصلح فتق الحجاب الحاجز ولا تمنع الارتداد من الحدوث. أي أنها \*\*تُخفي أعراضاً ولا تُعالج سبباً بشكل دائم\*\*. ولهذا يكون الاستخدام المزمن بلا تشخيص خطراً حقيقياً: الدواء يُسكت العرض بينما قد يستمرّ مرض أخطر بصمت — التهاب مريء متقدّم أو تغيّر في خلايا المريء أو ضيق أو قرحة أو ورم — فيتأخّر اكتشافه شهوراً أو سنوات. لذلك يُوصف الدواء بمدّة محدّدة وهدف واضح ومراجعة، لا كعلبة تُشترى بلا نهاية.

يصبح التشخيص إلزامياً في حالات محدّدة: استمرار الأعراض أو رجوعها سريعاً بعد كل محاولة إيقاف، أو بداية الأعراض في عمر متقدّم، أو وجود أي علامة إنذار. عندها يُطلب منظار للجزء العلوي من الجهاز الهضمي لرؤية المريء والمعدة مباشرة وأخذ خزعات إن لزم، وقد تُطلب فحوصات وظائف المريء مثل قياس الحموضة وقياس ضغط المريء لتأكيد أن الأعراض ارتداد فعلاً، خصوصاً قبل التفكير بأي تدخّل جراحي مضادّ للارتداد يُخصَّص لحالات مختارة يُقرّرها الفحص السريري والفحوصات لا رغبة المريض وحدها.

وتبقى نقطة سلامة لا يجوز إغفالها: ليست كل حرقة في الصدر ارتداداً. ألم القلب قد يتنكّر تماماً بصورة حرقة أو ثِقَل خلف عظم القصّ، خصوصاً إن ظهر مع المجهود أو رافقه تعرّق أو غثيان أو انتشار إلى الذراع أو الفكّ — وهذه حالة طارئة تُقيَّم كقلب حتى يُنفى ذلك.

## خطوات الإجراء

### 1. تقييم الأعراض واستبعاد المُشابهات

يسأل الطبيب عن نمط الحرقة وعلاقتها بالوجبات والاستلقاء ومدّتها، ويبحث عن علامات الإنذار، ويستبعد ما يشبه الارتداد مثل ألم القلب وقرحة المعدة واضطراب حركة المريء. هذه الخطوة هي ما يمنع علاج تشخيص خاطئ لسنوات.

### 2. تعديل الوزن وتوقيت الوجبات ووضعية النوم

This step is what prevents years of treating the wrong diagnosis.

### 2. Weight, meal timing and sleeping position

A practical plan is built: gradual weight loss, smaller meals, finishing food well before bedtime, raising the head of the bed, cutting your personal triggers and stopping smoking. These items are reviewed after some weeks to measure what has actually changed, rather than leaving medication solely responsible for the outcome.

### 3. Medication with a duration, a goal and a review

Acid suppression is prescribed at a dose and duration set by your physician, with a review appointment agreed in advance to judge the response and attempt gradual step-down. The aim is control with the least effective treatment, not years of refills without reassessment, because that delays detection of something more serious.

### 4. Endoscopy and oesophageal function testing when indicated

If symptoms persist, an alarm feature is present, or onset occurred at an older age, an upper endoscopy is performed to assess the oesophagus and stomach and take biopsies if needed. pH monitoring or oesophageal manometry may be added to confirm the diagnosis and define the abnormality before any surgical decision.

Gastro-oesophageal reflux is the return of acidic stomach content into the oesophagus because the valve between them, the lower oesophageal sphincter, fails to close properly or relaxes at the wrong time. The result includes familiar symptoms such as heartburn, an acid taste in the throat and repeated belching, and less obvious ones such as chronic cough, hoarseness, non-cardiac chest pain and early erosion of the back teeth. Treating reflux is not simply about silencing heartburn; it is a plan that addresses why the reflux happens.

The foundation on which everything else rests is reducing pressure inside the abdomen and changing meal timing: gradual weight loss for anyone carrying extra weight around the waist, finishing the last meal well before lying down, raising the head of the bed rather than piling up pillows, smaller meal volumes, less fat and fried food and caffeine along with whatever you personally notice triggers you, and stopping smoking. These are not polite add-on suggestions. They are part of the treatment itself, because how often reflux occurs is governed by abdominal pressure and how full the stomach is before you recline.

Acid-suppressing medication is genuinely effective at calming heartburn and healing oesophageal inflammation, but what it does and does not do must be clear: it reduces the acidity of the refluxed material. It does not tighten a weak sphincter, does not repair a hiatal hernia and does not stop reflux from happening. In other words, \*\*it masks symptoms rather than permanently correcting the cause\*\*. That is why long-term use without a diagnosis carries real risk: the drug quiets the symptom while a more serious problem may continue silently, whether advanced oesophagitis, a change in the oesophageal lining, a stricture, an ulcer or a tumour, and its discovery is delayed by months or years. For this reason medication is prescribed with a defined duration, a clear goal and a review date, not as an open-ended purchase.

Diagnostic testing becomes mandatory in specific situations: symptoms that persist or return quickly after every attempt to stop treatment, symptoms starting at an older age, or the presence of any alarm feature. An upper gastrointestinal endoscopy is then arranged to inspect the oesophagus and stomach directly and take biopsies if needed, and oesophageal function studies such as pH monitoring and manometry may be requested to confirm that the symptoms really are reflux, especially before considering any anti-reflux surgical intervention, which is reserved for selected patients chosen by clinical assessment and testing rather than by patient preference alone.

One safety point must never be skipped: not every burning sensation in the chest is reflux. Cardiac pain can disguise itself convincingly as heartburn or pressure behind the breastbone, particularly when it appears with exertion or comes with sweating, nausea, or spread to the arm or jaw. That is an emergency and must be assessed as a heart problem until proven otherwise.

## Preparation

Before your visit, note exactly when symptoms occur and how they relate to meals, sleep and exertion, along with every treatment you have tried, for how long, and the result; this information changes the whole plan. Tell your physician everything you take without exception, including painkillers, iron supplements and herbal products, since some irritate the oesophagus or imitate the symptoms. Mention any swallowing difficulty, weight loss, vomiting of blood, black stools or anaemia at the start of the conversation, not at the end. If an endoscopy is arranged, ask how long you must fast, which medicines are paused and who pauses them, and bring someone with you, because light sedation means you cannot drive. Never stop acid suppression before endoscopy on your own.

## Aftercare

Expect gradual rather than immediate improvement, and keep to the weight, timing and positioning changes even after the burning disappears, since abandoning them is the commonest reason symptoms return. Attend the agreed review so that stepping the medication down gradually can be assessed instead of stopping it abruptly, and do not extend any treatment for extra months on your own decision. \*\*Seek care immediately if you develop difficulty swallowing or a sense of food sticking, pain on swallowing, vomiting of blood or coffee-ground material, black tarry stools, unintended weight loss, repeated vomiting, or chest pain with sweating or spread to the arm or jaw.\*\* Do not treat heartburn that returns after every stop as normal and simply repeat the medication yourself; it is a signal that assessment is needed, not another box.
