الخطوة الأولى في العلاج هي تحديد السبب، لأن الحلول تختلف تماماً بحسبه. في بعض الحالات يكون السبب في الأسنان نفسها: ميل الأسنان العلوية إلى الأمام أو تزاحم يدفعها للخارج أو عادة مصّ إصبع أو دفع لسان. وفي حالات أخرى يكون السبب في علاقة الفكّين: فكّ علوي متقدّم أو فكّ سفلي متراجع. النوع الأول يُعالج بتحريك الأسنان، أما الثاني فيحتاج توجيه نمو الفك عند الطفل، أو جراحة فكّين مع تقويم عند البالغ الذي انتهى نموه.

الخيارات العلاجية تشمل التقويم الثابت أو المصفوفات الشفافة مع أو دون قلع، واستخدام مطاطات أو دعامات لضبط الإطباق، والأجهزة الوظيفية عند الأطفال في مرحلة النمو، والجراحة التقويمية للفكّين في الحالات الهيكلية عند البالغين. قرار القلع أو الجراحة ليس قراراً تجميلياً يُطلب، بل يُحدَّد بالفحص السريري والقياسات والصور الشعاعية، وقد تتعدّد الخطط المقبولة للحالة نفسها.

حدود العلاج يجب أن تُقال بصراحة: التقويم يحرّك الأسنان ولا يغيّر موضع الفك عند البالغ، لذا فإن الحالات الهيكلية الشديدة لا تُصحّح بالتقويم وحده وقد تحتاج جراحة. القشور والتيجان التجميلية لا تعالج بروزاً حقيقياً بل قد تفاقم المظهر. والنتيجة نفسها ليست نهائية بلا تثبيت: \*\*المثبِّت بعد انتهاء التقويم ليس اختيارياً؛ إهماله يعيد الأسنان إلى بروزها تدريجياً، وهذا أشيع سبب لفشل النتيجة.\*\*

## خطوات الإجراء

### 1. تحديد سبب البروز

يقيس الطبيب مقدار البروز الأفقي ويفحص علاقة الفكّين والشفتين وعادات الفم، ليفرّق بين بروز سببه الأسنان وآخر سببه هيكل الفك.

### 2. التصوير والقياسات التشخيصية

تُؤخذ صور شعاعية جانبية وشاملة وصور للفم ومسح للفكين، وتُحلّل القياسات لتقدير الفراغ المتوافر واتجاه النمو المتبقّي.

### 3. عرض الخطط البديلة واختيار واحدة

يشرح الطبيب الخيارات الممكنة بما فيها القلع أو التوجيه النمويّ أو الجراحة، مع مزايا كل خطة وحدودها والنتيجة المتوقّعة منها.

### 4. المرحلة العلاجية الفعلية

يُركّب التقويم أو تُسلَّم المصفوفات ويُتابع التحريك بجلسات منتظمة، وتُضاف المطاطات أو الأجهزة المساعدة عند الحاجة لضبط الإطباق.

### 5. التثبيت وحماية الأسنان الأمامية

يُركّب المثبِّت بعد إزالة الأجهزة، ويُنصح بواقٍ فموي عند ممارسة الرياضات التي تعرّض الأسنان الأمامية للرضّ.

## Procedure steps

### 1. Identifying the cause of the protrusion

The clinician measures the overjet and examines the jaw relationship, lip posture and oral habits to separate a dental protrusion from a skeletal one.

### 2. Imaging and diagnostic measurements

Lateral and panoramic radiographs, clinical photographs and arch scans are taken, and the measurements are analysed to estimate available space and remaining growth.

### 3. Presenting alternative plans and choosing one

The options are explained, including extraction, growth guidance or surgery, with the advantages, limits and expected outcome of each plan.

### 4. The active treatment phase

Braces are placed or aligners are delivered and movement is monitored at regular visits, with elastics or auxiliary devices added when needed to settle the bite.

### 5. Retention and protecting the front teeth

A retainer is fitted after the appliances are removed, and a mouthguard is advised for sports that put the front teeth at risk of impact.

## التحضير قبل الإجراء

أخبر الطبيب إن كان أي من أسنانك الأمامية قد تعرّض لرضّة سابقة أو تغيّر لونه أو سبق أن عُولج لبّه، فذلك يؤثّر في خطة التحريك. أخبره أيضاً عن ألم مفصل الفك أو طقطقته، وعن صعوبة إغلاق الشفتين أو التنفّس من الفم أو الشخير، وعن عادات مثل مصّ الإصبع أو عضّ القلم أو دفع اللسان لأن استمرارها يعيد البروز بعد العلاج. اذكر أي قشور تجميلية أو تيجان أو زراعات قائمة، وكل دواء منتظم وأي حالة تؤثّر في العظم أو اللثة. أنجز علاج التسوّس وتنظيف اللثة قبل البدء. وإن كنت بالغاً وسبب البروز هيكلي، اطلب شرحاً واضحاً للفرق بين ما يمكن للتقويم وحده أن يحقّقه وما يحتاج جراحة، وتجنّب أخذ القرار بناءً على صور نتائج مرضى آخرين.

## العناية بعد الإجراء

التزم بمواعيد الشدّ ولا تؤجّلها، فالتحريك المنقطع يطيل العلاج ويزيد خطر ارتداد الأسنان. إن وُصفت لك مطاطات، فالبسها بعدد الساعات المحدّد؛ فهي أكثر أجزاء العلاج اعتماداً على التزامك وأكثرها تأثيراً في تصحيح البروز. نظّف أسنانك بعد كل وجبة وتابع لثة الأسنان الأمامية خصوصاً، وتجنّب الأطعمة القاسية والعضّ المباشر على الفاكهة الصلبة. استخدم واقياً فموياً في الرياضات التي فيها تلامس، لأن السنّ البارز أكثر عرضة للكسر. \*\*راجع فوراً عند رضّة على سنّ أمامي أو تحرّكه أو تغيّر لونه، أو عند ألم نابض أو تورّم في اللثة أو خروج صديد.\*\* وبعد إزالة التقويم التزم بالمثبِّت كما وُصف تماماً، ولا تتوقّف عن لبسه لأن الأسنان تبدو مستقيمة، فالارتداد يبدأ بصمت قبل أن يُرى.

## About this service

Protruding front teeth means the upper front teeth sit further forward horizontally than the lower ones by more than the normal amount, a condition described clinically as an increased overjet. The issue is not only about appearance: prominent teeth are much more likely to fracture in any knock to the face, the lips may not close comfortably over them, and speech, biting and the health of the front gums can all be affected.

The first step in treatment is identifying the cause, because the solutions differ completely according to it. In some patients the problem lies in the teeth themselves: upper teeth tipped forward, crowding pushing them outward, or a habit such as thumb sucking or tongue thrusting. In others it lies in the relationship between the jaws, with an upper jaw set forward or a lower jaw set back. The first group is treated by moving teeth; the second needs jaw growth to be guided while the child is still growing, or combined orthodontic and jaw surgery in an adult whose growth is complete.

Treatment options include fixed braces or clear aligners, with or without extractions, elastics or anchorage devices to settle the bite, functional appliances in growing children, and orthognathic surgery combined with braces for skeletal cases in adults. The decision to extract teeth or to operate is not a cosmetic preference to be requested; it is determined by clinical examination, measurements and radiographs, and more than one acceptable plan may exist for the same case.

The limits should be stated frankly. Orthodontics moves teeth and does not reposition an adult jaw, so severe skeletal cases cannot be corrected by braces alone and may require surgery. Cosmetic veneers and crowns do not treat a genuine protrusion and can make the appearance worse. And the result itself is not permanent without retention: \*\*a retainer after braces is not optional; neglecting it lets the teeth drift forward again, which is the commonest reason a good result is lost.\*\*

## Preparation

Tell your orthodontist if any front tooth has been injured, changed colour or had root canal treatment, since this affects the movement plan. Mention jaw joint pain or clicking, difficulty closing the lips, mouth breathing or snoring, and habits such as thumb sucking, pen biting or tongue thrusting, because if they continue the protrusion returns after treatment. Declare any existing veneers, crowns or implants, all regular medication and any condition affecting bone or gums. Complete fillings and gum cleaning before treatment begins. If you are an adult and the cause is skeletal, ask for a clear explanation of what braces alone can achieve and what would need surgery, and avoid making the decision from photographs of other patients' results.

## Aftercare

Keep your adjustment appointments and do not postpone them, because interrupted tooth movement lengthens treatment and increases the risk of relapse. If elastics are prescribed, wear them for the exact number of hours you were told; they are the part of treatment that depends most on your commitment and that contributes most to correcting the protrusion. Brush after every meal and watch the gums around the front teeth in particular, and avoid hard foods and biting directly into firm fruit. Use a mouthguard for contact sports, since a protruding tooth fractures more easily. \*\*Seek care immediately after any blow to a front tooth, or if a tooth becomes mobile or changes colour, or with throbbing pain, gum swelling or pus.\*\* After the braces come off, wear your retainer exactly as prescribed and do not stop because the teeth look straight, as relapse begins silently before it becomes visible.
