## Procedure steps

### 1. Comprehensive assessment and excluding mimics

Intellectual functioning and adaptive behaviour are assessed with standardised tools from several sources, and hearing, vision, and language are checked before any conclusion. Pregnancy, birth, development, seizures, and illnesses are reviewed, the educational environment is examined to exclude deprivation, and blood tests or a medical or genetic consultation may be requested to look for a cause.

### 2. Determining the level of support, not labelling the person

Assessment findings are translated into a practical description of the support needed in each domain: communication, self-care, home living, social skills, safety, health, and work. What the person can do independently is written down, not only what he lacks, because deficit-only wording produces low expectations that limit his teaching.

### 3. Building a functional individual education plan

A few goals with real daily impact are chosen and written in measurable terms, spread across functional academics such as functional reading, money, and time, daily living skills, communication, and social and self-protection skills. Teaching methods, supports, the responsibilities of school and family, and review dates are all specified.

### 4. Teaching in small steps and transferring to real life

Each skill is broken into steps taught with prompting that fades towards independence, with reinforcement, enough repetition, and data recorded every session. Transfer of the skill to home, school, and shops is planned explicitly, and parents and teachers are trained in the same method to avoid contradictory teaching that confuses the learner.

### 5. Review and early planning for adulthood

Data are reviewed periodically to decide whether goals are mastered or need modification, and the plan is corrected if progress stalls once a medical or sensory cause has been excluded. Transition planning starts well before the end of schooling: vocational skills, mobility, money, health and puberty, protective skills, and the level of support needed in adult life.

## About this service

Intellectual disability is a developmental condition combining limitations in intellectual functioning with limitations in adaptive behaviour — that is, in daily living, communication, social, and practical skills — appearing during the developmental years rather than later in life. Special education here does not treat the condition in a medical sense; it builds functional skills and supports that make the person more independent and more able to participate at home, at school, and in the community.

Terminology matters. The older labels of mental retardation and mental handicap are now considered inappropriate, and the professional term is intellectual disability. A person is not described by a level but by the intensity of support he needs: intermittent, limited, extensive, or pervasive. This is not linguistic decoration, because the label determines how the person is treated and what is expected of him.

Diagnosis is never built on an intelligence test alone. Before any conclusion, conditions that mimic it must be excluded: hearing or vision loss, educational or language deprivation, a specific learning difficulty, anxiety or emotional disturbance, and medical problems affecting attention. A medical or genetic cause is also looked for where that search is useful, because knowing it may change health surveillance and help the family with genetic counselling. Coexisting conditions are common — epilepsy, attention disorders, autism spectrum disorder, thyroid problems, and hearing loss — and overlooking them lowers performance for no reason.

\*\*Early intervention changes the trajectory.\*\* Training in communication and self-care during the first years builds a foundation that is hard to recover later and reduces the entrenchment of difficult behaviours. \*\*The family and the school are part of the plan, not recipients of it,\*\* because a skill trained at the centre and not applied at home and in class does not hold.

\*\*Progress is measured against individual written goals for this person — dressing himself, using money for a simple purchase, or asking for help in a sentence — not by comparison with peers, class averages, or chronological age.\*\* One of the most important strands of the plan is teaching protective skills, privacy, and physical boundaries, because people with intellectual disability are at higher risk of exploitation and abuse; teaching them to say no and to tell a trusted person is part of education, not an add-on.

\*\*The limits are explicit: no programme raises intelligence, the goal is independence and quality of life rather than turning the person into someone else, and the programme neither cures the condition nor replaces medical follow-up of coexisting problems.\*\* \*\*Be wary of any programme, supplement, or device promising to cure intellectual disability, to raise intelligence, or to deliver fast results for large fees, or asking you to stop the doctor's treatment.\*\* Equally, be wary of underestimating the person, because assuming incapacity prevents learning that was possible. Eligibility and level of support are set by a specialist team assessment, not by an online test.

## Preparation

Bring pregnancy, birth, and developmental records if you have them, together with all medical reports, previous psychological and educational assessments, hearing and vision test results, and a full list of medicines with their timings. Before the appointment, write a precise description of what your child does on his own today: whether he eats, dresses, and uses the toilet independently, how he asks for what he needs, how many words he uses and who understands him, and how he behaves among other children. Note when he first sat, walked, and spoke, and whether he ever lost a skill he had mastered, because regression is a sign that requires medical assessment. Mention seizures, sleep, eating, constipation, and pain, and any family history of similar conditions. Record short videos at home, since they are more accurate than description. Do not coach your child on assessment answers. And discuss your expectations openly: the aim is greater independence in small steps, and no programme raises intelligence.

## Aftercare

Apply the training steps at home exactly as written, use everyday situations for practice rather than a separate session, and do not complete tasks on the person's behalf, because excessive help creates a dependence that is hard to undo later. Maintain a stable routine, regular sleep, and good nutrition, and keep up medical checks for coexisting conditions such as vision, hearing, thyroid, and seizures, since these quietly lower performance. Teach protective skills, privacy, and body boundaries, and teach him to tell a trusted person, while keeping oversight of all his environments because the risk of exploitation is higher. Keep notes, attend reviews, compare his progress with his written goals rather than with peers, and expect slow, uneven gains. \*\*Seek review immediately if he loses a skill he had mastered or clearly declines in performance, has a seizure, shows a sudden behaviour change with no clear cause since it may be hidden pain such as toothache, earache, or constipation, escalates self-injury or aggression, refuses food and loses weight, or shows any sign of harm, neglect, or exploitation.\*\* \*\*Be wary of any product or programme promising higher intelligence or a fast cure.\*\*
