\*\*وأخطر ما يحدث في هذه الحالات أن تُقدَّر قدرة الشخص من إعاقته الأظهر وحدها.\*\* فيُنظَر إلى الطفل الذي لا يمشي ولا يتكلّم فيُفترَض أنه لا يفهم، أو يُشخَّص بإعاقة ذهنية طفل مشكلته الحقيقية أنه لا يسمع أصلاً ما يُقال له. \*\*ولذلك فإن فحص السمع والبصر ليس اختيارياً في هذه الحالات بل إلزامي ويُعاد دورياً\*\*، لأن بعض الحالات تتدهوّر تدريجياً ولأن ضعفاً حسّياً غير مكتشف يُفسَّر خطأً على أنه عجز في الفهم، ويترك الشخص سنوات بلا تعليم كان ممكناً.

والمبدأ العملي الثاني أن \*\*التواصل أولاً ثم كل شيء آخر\*\*: يجب أن يمتلك الشخص وسيلة موثوقة يفهمها محيطه للإجابة بنعم ولا وطلب حاجاته والاعتراض، سواء كانت إشارة أو لمساً أو لوحة أو جهازاً أو إشارة لمسية للصمّ العُمي. وبلا هذه الوسيلة يكون كل هدف أكاديمي أو سلوكي مبنيّاً على الرمل، ويصبح السلوك الصعب هو لغة الشخص الوحيدة.

والمبدأ الثالث أن الخطة \*\*واحدة ومتكاملة لا خطط متفرّقة متناقضة\*\*. تعدّد المختصّين في هذه الحالات نعمة إن اجتمعوا على خطة واحدة، ونقمة إن كتب كل واحد أهدافه وحده فتتضارب التوصيات وتُستهلَك الأسرة في مواعيد بلا اتّجاه. ولذلك يُنسَّق العمل بين التربية الخاصة والنطق والعلاج الوظيفي والطبيعي والطبيب في وثيقة واحدة بأهداف مرتّبة بالأولوية.

\*\*والتدخّل المبكر يغيّر المسار\*\* في هذه الحالات أكثر من غيرها لأن الفجوات تتراكم من قناتين لا من قناة واحدة. \*\*والأسرة والمدرسة جزء من الخطة لا متلقّون لها\*\*، والأسرة هي مَن يعرف إشارات الشخص الخاصة قبل أي مختصّ ويجب أن تُوثَّق منها. \*\*والتقدّم يُقاس بأهداف فردية مكتوبة لهذا الشخص — كزيادة عدد الطلبات التي يعبّر عنها بوسيلته، أو إنجاز خطوة إضافية في مهمّة بمساعدة أقلّ — لا بمقارنته بغيره ولا بأقرانه في العمر.\*\*

\*\*وحدود الخدمة صريحة: البرنامج لا يشفي أي من الإعاقتين ولا يُعيد وظيفة حسّية أو حركية مفقودة ولا يُغني عن المتابعة الطبية والعلاجية، والتقدّم أبطأ ويحتاج دعماً طويل الأمد.\*\* \*\*واحذر من أي برنامج أو جهاز أو مكمّل يَعِد بشفاء أو بنتائج سريعة أو بإصلاح السمع والبصر أو الحركة مقابل مبالغ كبيرة، أو يطلب إيقاف علاج الطبيب، أو يزعم أن مختصّاً واحداً يكفي لكل شيء.\*\* والترشيح ومستوى الدعم يُحدَّدان بتقييم فريق متخصّص لا بالقراءة على الإنترنت.

## خطوات الإجراء

### 1. الفحص الحسّي الإلزامي قبل أي حكم

يبدأ العمل بفحص سمع وبصر متخصّص حتى لو بدا الطفل مستجيباً، لأن ضعفاً جزئياً غير مكتشف يُفسَّر خطأً على أنه عجز في الفهم. وتُراجَع تقارير الطبيب والحالات المصاحبة، ويُعاد الفحص دورياً لأن بعض الحالات تتدهوّر تدريجياً ويجب التقاط التغيّر مبكراً.

### 2. تقييم بوسائل لا تعتمد على القناة المصابة

يُقيَّم الفهم والقدرات بوسائل تتجاوز الإعاقة: لمسياً للصمّ العُمي، وبصرياً لمَن لا يسمع، وبإجابات لا تحتاج كلاماً ولا كتابة لمَن لا يتحكّم في حركته. ويُوثَّق من الأسرة قاموس الإشارات الخاصة التي يستخدمها الشخص فعلاً، لأنها الأساس الذي يُبنى عليه أي تواصل رسمي.

### 3. بناء نظام تواصل موثوق أولاً

يُختار نظام تواصل يناسب القناة السليمة ويُدرَّب عليه الشخص ومحيطه معاً: إشارات لمسية، أو أشياء مرجعية، أو صور مكبّرة عالية التضادّ، أو جهاز تواصل بمفاتيح كبيرة أو مسح ضوئي. ويُبدأ من طلب حاجات حقيقية يهتمّ بها الشخص، ويُثبَّت النظام في كل بيئاته لا في غرفة واحدة.

### 4. خطة واحدة متكاملة بأولويات واضحة

يجتمع الفريق — تربية خاصة ونطق وعلاج وظيفي وطبيعي وطبيب — على وثيقة واحدة بأهداف قليلة مرتّبة بالأولوية، وتُوزَّع الأدوار ومواعيد المراجعة. وتُحدَّد الوضعيات الآمنة وتعديلات البيئة والإضاءة والصوت وأدوات الوصول، وتُكتَب خطة طوارئ إن كانت هناك نوبات أو مشاكل بلع.

### 5. تدريب المحيط والمراجعة الدورية

يُدرَّب الأهل والمعلّمون ومقدّمو الرعاية على النظام والوضعيات والمناولة بالطريقة نفسها، لأن التناقض بين البيئات يوقف التقدّم. وتُسجَّل البيانات وتُراجَع دورياً على أهداف الشخص المكتوبة، وتُعدَّل الوسائل مع النموّ ومع أي تغيّر حسّي أو صحّي، ويُخطَّط للانتقال إلى مرحلة البلوغ مبكراً.

## Procedure steps

### 1. Mandatory sensory testing before any conclusion

Work begins with specialist hearing and vision testing even if the child appears responsive, because an undetected partial loss is misread as a failure of understanding. Medical reports and coexisting conditions are reviewed, and testing is repeated periodically, since some conditions deteriorate gradually and the change must be caught early.

### 2. Assessment through channels that are not impaired

Understanding and ability are assessed through channels that bypass the impairment: tactile for the deafblind, visual for those who cannot hear, and response modes that need neither speech nor handwriting for those who cannot control movement. The family's dictionary of the individual signals the person already uses is documented, because it is the foundation on which any formal communication is built.

### 3. Building a reliable communication system first

A communication system suited to the intact channel is chosen, and the person and those around him are trained in it together: tactile signs, objects of reference, enlarged high-contrast pictures, or a device with large keys or scanning access. It starts from requesting real things the person cares about, and the system is established in all his environments rather than one room.

### 4. One integrated plan with clear priorities

The team — special education, speech therapy, occupational therapy, physiotherapy, and medicine — agrees on a single document with a few goals ranked by priority, with roles and review dates assigned. Safe positioning, environmental adjustments to lighting and sound, and access tools are specified, and an emergency plan is written if there are seizures or swallowing problems.

### 5. Training the environment and periodic review

Parents, teachers, and caregivers are trained in the same system, positioning, and handling, because inconsistency between environments halts progress. Data are recorded and reviewed periodically against the person's written goals, equipment is adjusted with growth and after any sensory or health change, and transition to adulthood is planned early.

## العناية بعد الإجراء

استخدم نظام التواصل نفسه في كل مكان وفي كل موقف، ودرّب كل مَن يتعامل مع الشخص عليه، فوسيلة يعرفها شخص واحد ليست وسيلة تواصل. طبّق الوضعيات الآمنة والمناولة الصحيحة كما دُرّبت، وافحص الجلد يومياً فوق مناطق الضغط، وراقب الأكل والشرب فالشرقة المتكرّرة ليست أمراً عادياً. حافظ على مواعيد المتابعة الطبية والعلاجية وأعد فحص السمع والبصر بالمدّة التي حدّدها الفريق، لأن أي تدهوّر حسّي يُبطل خطّة قائمة على قناة لم تعد تعمل. سجّل ملاحظاتك اليومية القصيرة واحملها إلى المراجعة، ولا تقارن تقدّمه بغيره بل بأهدافه المكتوبة، واطلب دعماً للأسرة فالإنهاك حقيقي. \*\*راجع فوراً عند فقدان مهارة كان يتقنها، أو تراجع واضح في السمع أو البصر، أو نوبة تشنّج، أو سعال وشرقة متكرّرة أثناء الطعام أو التهاب صدري متكرّر، أو احمرار جلدي لا يزول أو تقرّح، أو تغيّر سلوكي مفاجئ بلا سبب لأنه قد يكون ألماً خفيّاً، أو رفض الطعام وفقدان وزن، أو أي علامة إيذاء أو إهمال.\*\*

## About this service

Dual disability means two or more disabilities in the same person: intellectual disability together with a motor impairment, hearing loss together with visual loss, a sensory impairment together with a developmental disorder, or a motor impairment together with a communication disorder. The basic rule is that their impact is not the arithmetic sum of two conditions but an interaction that makes assessment and teaching harder: a child who cannot see well cannot compensate for hearing loss by lip-reading, and a child who cannot control his hands cannot use a manual sign language.

\*\*The most dangerous thing that happens in these cases is that the person's ability is judged from his most visible disability alone.\*\* A child who does not walk and does not speak is assumed not to understand, or a child whose real problem is that he cannot hear what is said to him is diagnosed with intellectual disability. \*\*Hearing and vision testing is therefore not optional in these cases but mandatory, and it is repeated periodically,\*\* because some conditions deteriorate gradually and because an undetected sensory loss is misread as a failure of understanding, leaving the person for years without education that was entirely possible.

The second practical principle is \*\*communication first, everything else after.\*\* The person must have a reliable means, understood by those around him, to answer yes and no, to request what he needs, and to object — whether a gesture, touch cue, board, device, or tactile signing for the deafblind. Without it, every academic or behavioural goal is built on sand, and difficult behaviour becomes the person's only language.

The third principle is that the plan is \*\*one integrated document, not several conflicting plans.\*\* Having many specialists is a blessing when they agree on a single plan and a curse when each writes his own goals, so recommendations clash and the family is consumed by appointments with no direction. Special education, speech therapy, occupational therapy, physiotherapy, and medicine are therefore coordinated in one document with goals ranked by priority.

\*\*Early intervention changes the trajectory\*\* here more than anywhere, because gaps accumulate through two channels rather than one. \*\*The family and the school are part of the plan, not recipients of it,\*\* and the family knows the person's individual signals before any specialist does, so those signals must be documented from them. \*\*Progress is measured against individual written goals for this person — more requests expressed through his communication method, or one more step of a task completed with less help — not by comparison with others or with same-age peers.\*\*

\*\*The limits are explicit: the programme cures neither disability, does not restore lost sensory or motor function, does not replace medical and therapeutic follow-up, and progress is slower and needs long-term support.\*\* \*\*Be wary of any programme, device, or supplement promising a cure, fast results, or the repair of hearing, vision, or movement for large fees, asking you to stop the doctor's treatment, or claiming that one specialist is enough for everything.\*\* Eligibility and level of support are determined by a specialist team assessment, not by reading online.

## Preparation

Bring all previous medical, therapy, and educational reports, especially hearing and vision test results with their dates, and if there are no recent tests, request them explicitly before any educational assessment. Provide a complete list of medicines and their timings, and mention seizures, swallowing, breathing, sleep, and pain problems. Before the appointment, write your child's dictionary: every sign, sound, or movement he uses and what it means in your family, how he says yes and no, how he expresses refusal, pain, and pleasure, and which family members understand him. Record short videos of him in different situations, because they are more accurate than any description. Describe what he can actually do today in sitting, eating, transfers, and self-care, and what calms him and what distresses him in light, sound, or touch. List every specialist following him and their written recommendations, so they can be merged into one plan. And discuss your expectations openly: the aim is more communication and independence in small steps, not a cure.

## Aftercare

Use the same communication system everywhere and in every situation, and train everyone who interacts with the person, because a method only one person understands is not communication. Apply the safe positions and correct handling as you were trained, inspect the skin daily over pressure areas, and watch eating and drinking, since repeated choking is not normal. Keep medical and therapy appointments and repeat hearing and vision testing at the interval the team specified, because any sensory deterioration invalidates a plan built on a channel that no longer works. Keep short daily notes and bring them to reviews, compare his progress with his written goals rather than with other people, and ask for family support, because caregiver exhaustion is real. \*\*Seek review immediately if he loses a skill he had mastered, if hearing or vision clearly declines, if a seizure occurs, if there is repeated coughing or choking during meals or recurrent chest infection, skin redness that does not fade or an ulcer, a sudden behaviour change with no clear cause since it may be hidden pain, food refusal with weight loss, or any sign of harm or neglect.\*\*
