## Procedure steps

### 1. Assessment and confirming the cause

A clinical examination of the feet and hands covering light touch, vibration and temperature sensation, tendon reflexes, foot pulses, and the state of the skin, nails and pressure points. Blood tests (HbA1c, vitamin B12, thyroid and kidney function) are ordered, and nerve conduction studies may be added to define the pattern and severity and to rule out a focal nerve entrapment.

### 2. Glucose control and associated risk factors

The diabetes regimen is reviewed and an individualised HbA1c target is set according to age and comorbidities, alongside blood pressure and lipid control, smoking cessation, reduced alcohol and weight management. This is the only pillar that slows nerve damage itself, although its effect on symptoms appears over months rather than days.

### 3. A plan for neuropathic pain control

Medicines used specifically for nerve pain are prescribed and titrated gradually under supervision, with monitoring for drowsiness, dizziness, swelling and kidney function, and a switch to an alternative if the response is poor. Topical options may be added, and sleep disturbance and low mood are treated because both raise perceived pain intensity.

### 4. Rehabilitation, balance and fall prevention

A physiotherapy programme strengthens the calf and ankle and trains balance, because loss of sensation increases the risk of tripping, falling and fractures. Footwear is assessed and custom insoles prescribed where there is deformity or a pressure area, and the home is adapted with night lighting, removal of loose rugs and grab rails in the bathroom.

### 5. Foot protection programme and follow-up

The patient is trained to inspect both feet daily with a mirror, to have nails and callus managed by a professional rather than with blades or caustic products, and to seek same-day care for any wound. Follow-up is scheduled every three to six months to review medication and symptoms, with a comprehensive foot examination at least once a year.

## About this service

Diabetic peripheral neuropathy is progressive damage to the peripheral nerves. It usually begins symmetrically in the toes and creeps upward over years of elevated blood glucose and impaired flow in the tiny vessels that feed the nerve. Treatment is not a single tablet: it combines glucose control to halt progression, targeted management of neuropathic pain, and a foot-protection programme that prevents ulceration and amputation.

Patients typically describe numbness, burning, or electric-shock pain that worsens at night. More dangerous is the silent form, in which sensation is lost and a wound forms and becomes infected without ever being felt. Assessment is mainly clinical: light touch, vibration and temperature sensation, reflexes, foot pulses, skin condition and pressure points. Blood tests or nerve conduction studies may be requested to exclude conditions that look identical, including vitamin B12 deficiency, thyroid disease, alcohol use, certain medications and autoimmune disease. This step matters, because some cases blamed on diabetes have another, treatable cause.

The first and most important pillar is control of blood glucose, blood pressure and lipids, along with stopping smoking, because this is what genuinely slows nerve damage. Pain is treated with medicines used specifically for nerve pain, prescribed by a physician and increased gradually to an acceptable response while side effects and kidney function are monitored. Ordinary painkillers help little with this type of pain. Physiotherapy addresses balance and strength, and poor sleep or low mood are treated because both amplify the perception of pain.

The honest limits: no medicine currently rebuilds destroyed nerve fibres. The realistic goals are to stop progression, reduce pain and protect the feet, not to restore lost sensation. Vitamins help only where a true deficiency is documented, and marketed supplements or devices do not replace glucose control. The absence of pain is not recovery either; an insensate foot is the one most likely to ulcerate. Daily foot inspection and appropriate footwear prevent the most serious complication. Eligibility for any medication or rehabilitation plan is decided by clinical examination and a review of your medications and kidney function.

## Preparation

Bring a list of every medicine and supplement you take, your glucose log, your most recent HbA1c, and any vitamin B12, thyroid and kidney results you have. Tell the doctor if you drink alcohol, are receiving chemotherapy, have kidney or liver disease, or take medicines for sleep or mood, because all of these change the choice and dose adjustment of nerve-pain medication. Wear socks and shoes that are easy to remove, since examining both feet is an essential part of the visit, and do not apply creams or dyes to your feet beforehand. Photograph any wound, crack or colour change you noticed even if it has gone, and bring previous nerve conduction reports. Do not stop any medication on your own, and bring a companion if you have dizziness or unsteadiness.

## Aftercare

Expect pain to improve over weeks rather than days, and never increase the dose yourself; report drowsiness, dizziness, swelling or confusion, particularly in older patients. Inspect both feet daily with a mirror, including between the toes and the sole, and never walk barefoot, even indoors. Test water temperature with your hand or elbow rather than your foot, cut nails straight across, and avoid blades, callus-burning products and hot packs. \*\*Seek care immediately if\*\* you develop any wound, ulcer, swelling, redness or odour in the foot, a blackened toe, sudden pain with a cold pale foot, fever with chills, rapidly progressing weakness, or numbness climbing into the legs and hands within days.
