التوقيت مهم لفهم مسار العلاج: التحسّن غالباً تدريجي على مدى أسابيع، ويسبق تحسّنُ النوم والطاقة عادةً تحسّنَ المزاج والاهتمام. ولذلك تُقاس الاستجابة بمقاييس أعراض في مواعيد المتابعة، وتُراجع الخطة إن لم يظهر تقدّم كافٍ بعد مدّة معقولة: يُعدَّل المقدار، أو يُغيَّر الخيار، أو يُضاف علاج نفسي، أو يُعاد النظر في التشخيص نفسه.

وعندما لا تستجيب الحالة لخطة كافية، تُطرح خيارات متقدّمة كالتحفيز المغناطيسي عبر الجمجمة، وفي الحالات الشديدة جداً أو الخطرة يُطرح العلاج بالصدمات الكهربائية. الترشيح لأي منها يُحدَّد بالفحص السريري لا بالقراءة.

ولحدود العلاج أهمية توازي فوائده: هو لا يجعل الحياة بلا حزن — فالحزن على فقد حقيقي جزء من الإنسان لا مرض. والدواء لا يحلّ ضغطاً مالياً ولا علاقة مؤذية ولا عملاً مستنزفاً، لكنه قد يعيد لك القدرة على التعامل معها. ولا يوجد تحليل دم يُشخّص الاكتئاب. ومن الخرافات المؤذية أن الاكتئاب ضعف إيمان أو قلّة إرادة، وأن عبارة شدّ حيلك خطة علاج — وهي ليست كذلك.

## خطوات الإجراء

### 1. التقييم وتقدير الشدة والخطر

مقابلة سريرية توثّق الأعراض ومدّتها وأثرها على النوم والعمل والعلاقات، مع تقييم صريح لأفكار إيذاء النفس، وسؤال عن أي فترة سابقة ارتفع فيها النشاط وقلّت الحاجة إلى النوم لتمييز الاكتئاب أحادي القطب عن ثنائي القطب.

### 2. استبعاد الأسباب الجسدية

قد تُطلب تحاليل مثل وظائف الغدة الدرقية وصورة الدم ومخزون الحديد وفيتامينات محددة، ويُسأل عن انقطاع النفس أثناء النوم وأمراض مزمنة وأدوية قد تُخفّض المزاج. تصحيح سبب جسدي يُغيّر أحياناً الخطة كلها.

### 3. بناء خطة مشتركة

تُشرح لك الخيارات: علاج نفسي منظّم، أو خطة دوائية، أو كلاهما، مع المتوقع من كل خيار ومدّته وآثاره الجانبية. تُضاف تدخّلات سلوكية عملية: جدول نشاط تدريجي، وتنظيم نوم، وحركة يومية، وتقليل الكحول، وإعادة تواصل اجتماعي.

### 4. المتابعة وقياس الاستجابة

تكون المواعيد متقاربة في البداية لتقييم التقدّم والآثار الجانبية وسلامتك. يُستخدم مقياس أعراض ثابت للمقارنة بدل الاعتماد على الشعور العام، وإن لم يظهر تحسّن كافٍ بعد مدّة معقولة تُعدَّل الخطة بدلاً من انتظار مفتوح.

### 5. الاستمرار والوقاية من الانتكاس

بعد التحسّن تُستكمل مرحلة استمرار لعدّة أشهر لتثبيت النتيجة، مع خطة وقاية: مهارات مكتسبة من العلاج النفسي، وعلامات إنذار مبكّر مكتوبة، ونشاط ونوم منظّمان، ثم إيقاف دوائي تدريجي مخطَّط عند الاتفاق. وعند عدم الاستجابة تُطرح الخيارات المتقدّمة.

## Procedure steps

### 1. Assessment of severity and risk

A clinical interview documents symptoms, their duration and their effect on sleep, work and relationships, with explicit assessment of thoughts of self-harm and questions about any earlier period of high activity and reduced need for sleep, in order to distinguish unipolar from bipolar depression.

### 2. Excluding physical contributors

Tests such as thyroid function, a blood count, iron stores and specific vitamins may be requested, with questions about sleep apnoea, chronic illness and medicines that can lower mood. Correcting a physical contributor sometimes changes the whole plan.

### 3. Building a shared plan

The options are explained: structured psychotherapy, a medication plan, or both, along with what each can achieve, how long it takes and its side effects. Practical behavioural elements are added: a graded activity schedule, sleep regulation, daily movement, cutting alcohol and re-establishing social contact.

### 4. Follow-up and measuring response

Appointments are close together at first to review progress, side effects and safety. A consistent symptom scale is used for comparison instead of relying on a general impression, and if improvement is insufficient after a reasonable period the plan is changed rather than waited on indefinitely.

### 5. Continuation and relapse prevention

After improvement, a continuation phase of several months consolidates the result alongside a prevention plan: skills gained in therapy, written early warning signs, and regulated activity and sleep, followed by a planned gradual withdrawal of medication when agreed. If response is inadequate, advanced options are discussed.

اكتب قبل الموعد متى بدأت الأعراض وكيف تغيّرت، وكيف تؤثّر على نومك وشهيتك وتركيزك وعملك وعلاقاتك، وما الذي يزيدها أو يخفّفها. أحضر قائمة كاملة بكل ما تتناوله من أدوية ومكمّلات وأعشاب، وتقاريرك وتحاليلك السابقة، واذكر ما جرّبته من علاج سابق ونتيجته والمدّة التي استمرّ فيها. أخبر الطبيب عن الكحول والمواد والمنبّهات، وعن التاريخ العائلي لأي اضطراب مزاجي، وعن أي فترة كنت فيها أقلّ حاجة إلى النوم ومع ذلك نشيطاً جداً. وإن مرّت بك أفكار عن إيذاء نفسك فقُلها في الموعد بصراحة — قولها هو ما يجعل الخطة آمنة، ولا يعني إدخالاً إلى المستشفى بالضرورة. وإن كان الكلام صعباً فاكتبه على ورقة وأعطها للطبيب.

## العناية بعد الإجراء

التزم بالخطة كما اتُّفق عليها، واحرص على أساسيات لا تبدو علاجاً وهي كذلك: مواعيد نوم ثابتة، وحركة يومية ولو قصيرة، وخروج إلى ضوء النهار، وتواصل مع شخص واحد على الأقل، وتقليل الكحول والسهر. سجّل مزاجك ونومك باختصار كل يوم فهذا يجعل المتابعة أدقّ من الذاكرة. \*\*راجع فوراً عند تدهور سريع في المزاج، أو توقف عن الأكل والشرب، أو عجز عن النهوض والعمل، أو ظهور شكّ شديد أو سماع أصوات، أو تصاعد في القلق والتهيّج والأرق في أول أسابيع بدء العلاج أو تغييره، أو ارتفاع مفاجئ في النشاط مع قلّة الحاجة إلى النوم.\*\* \*\*وإذا مرّت بك أفكار عن إيذاء نفسك أو عن الانتحار فاطلب مساعدة عاجلة فوراً: اتصل بالطوارئ أو اذهب إلى أقرب قسم طوارئ أو أخبر شخصاً تثق به الآن، ولا تنتظر موعدك القادم — ولا تبقَ وحدك في تلك الساعات.\*\* ولا تُوقف الدواء عند أول تحسّن؛ مرحلة الاستمرار هي ما يمنع الانتكاس، وأي إيقاف يكون بخطة طبية تدريجية لا فجأة. والعلاج النفسي والدوائي ليسا متنافسين.

## About this service

Treatment for depression is a structured plan built after clinical assessment. Its aim is to return mood, sleep, energy and the ability to function to a workable level, and then to maintain that improvement long enough to reduce the chance of symptoms returning. Depression here is not passing sadness but a cluster of persistent symptoms that disrupt daily life.

The plan is chosen according to severity, your own preference and what has been tried before. In mild to moderate presentations, structured psychotherapy with behavioural changes may be enough: a gradual return to activity and movement, sleep regulation, light exposure, and rebuilding routine and social contact. In moderate to severe presentations, medication set and titrated by a clinician is usually added. The two are not rivals, and combining them is often better than either alone.

Timing matters for understanding the course of treatment. Improvement is usually gradual over weeks, and sleep and energy typically improve before mood and interest do. Response is therefore measured with symptom rating scales at follow-up visits, and the plan is reviewed if there is not enough progress after a reasonable period: adjusting the amount, switching the option, adding psychotherapy, or reconsidering the diagnosis itself.

When a condition does not respond to an adequate plan, advanced options such as transcranial magnetic stimulation are discussed, and in very severe or high-risk situations electroconvulsive therapy is considered. Suitability for any of these is determined by clinical assessment, not by reading about them.

The limits matter as much as the benefits. Treatment does not make life free of sadness — grief over a real loss is part of being human, not an illness. Medication does not resolve financial pressure, a harmful relationship or an exhausting job, though it may restore your capacity to deal with them. No blood test diagnoses depression. And two harmful myths deserve naming: depression is not weak faith or a lack of willpower, and telling someone to pull themselves together is not a treatment plan.

## Preparation

Before the appointment, write down when symptoms started and how they have changed, how they affect sleep, appetite, concentration, work and relationships, and what makes them better or worse. Bring a full list of everything you take, including supplements and herbal products, along with previous reports and tests, and note any earlier treatment, its result and how long it lasted. Tell the clinician about alcohol, substances and stimulants, about any family history of mood disorder, and about any period when you needed less sleep yet felt highly energetic. If thoughts of harming yourself have occurred, say so plainly at the visit — saying it is what makes the plan safe, and it does not automatically mean admission to hospital. If speaking about it is hard, write it on a piece of paper and hand it over.

## Aftercare

Follow the plan as agreed and hold on to the basics that do not look like treatment but are: consistent sleep times, daily movement however brief, getting out into daylight, contact with at least one person, and cutting back on alcohol and late nights. Keep a short daily note of mood and sleep, since this makes follow-up more accurate than memory. \*\*Seek help immediately for a rapid decline in mood, stopping eating and drinking, inability to get up or work, the appearance of intense suspiciousness or hearing voices, a surge of anxiety, agitation and insomnia in the first weeks after starting or changing treatment, or a sudden rise in activity with little need for sleep.\*\* \*\*If thoughts of harming yourself or of suicide arise, get urgent help straight away: call emergency services, go to the nearest emergency department, or tell someone you trust right now. Do not wait for your next appointment, and do not stay alone during those hours.\*\* Do not stop medication at the first sign of improvement; the continuation phase is what prevents relapse, and any discontinuation follows a gradual medical plan rather than happening abruptly. Psychotherapy and medication are not rivals.
