## Procedure steps

### 1. Comprehensive assessment and pain typing

The doctor asks where the pain is, how it feels, what worsens and relieves it, how you sleep, how your mood and daily function are affected, and every medicine you have tried with its dose and result. Movement, strength, sensation and reflexes are examined, warning features are sought, and imaging or blood tests are requested only when they would change the plan, not as a routine for everyone.

### 2. Written functional goals

Measurable goals are agreed with you - sitting for an hour, walking ten minutes, returning to part-time work, five hours of unbroken sleep - and your starting point is recorded. Such goals make progress visible when the pain score itself barely moves, and they protect against hasty decisions like escalating doses in pursuit of a complete relief that is not realistic.

### 3. Physiotherapy, graded activity and sleep

An exercise programme starts and is graded slowly to build endurance without flare-ups, with manual or thermal therapy where useful and correction of work and sleep posture. Sleep is treated in its own right because poor sleep genuinely raises pain intensity, pain-management, breathing and cognitive behavioural techniques are added, and it is explained that a small temporary rise in pain with exercise is not harm.

### 4. Medicines matched to the mechanism

Medication is selected by pain type: drugs for neuropathic pain differ entirely from those for nociceptive pain, and topical agents or muscle relaxants have a place in specific situations. Your other conditions and drug interactions are reviewed, each medicine is given a defined trial period and a clear target, and anything that has not delivered benefit is stopped rather than stacked on top of the rest.

### 5. Targeted interventions and review

In selected cases guided injections, a diagnostic nerve block or radiofrequency treatment are offered, aiming at temporary relief that enables rehabilitation rather than permanent cure, and usually performed under imaging guidance. Regular reviews measure the functional goals and adjust the plan, and complex cases are referred to a multidisciplinary pain team.

## About this service

Chronic pain treatment is a multi-strand medical programme for pain that persists beyond three months, or beyond the time the original injury should have needed to heal. It starts with careful assessment to define the type of pain: nociceptive pain arising from a joint, muscle or bone; neuropathic pain that burns with numbness and tingling; centrally mediated pain in which the nervous system's own processing of pain is amplified; or a combination of these. Classifying the pain is not an academic detail, because it determines which medicines can help, what kind of physiotherapy is appropriate, and whether any interventional procedure is worth considering.

A typical plan combines physiotherapy and graded activity that is built up slowly rather than stopped at the first discomfort, patient education about how pain works, sleep restoration because poor sleep genuinely increases pain intensity, treatment of coexisting anxiety and low mood, medicines chosen by pain mechanism rather than by severity alone, and targeted interventions such as perineural injections, nerve blocks or radiofrequency treatment in selected cases. The most successful programmes measure progress by function - the distance you walk, the stairs you climb, the hours you work, whether you sleep through the night - and not by a pain score alone.

The limits deserve plain speaking. The realistic goal is \*\*less pain and a better ability to live your life, not zero pain\*\*. Opioid painkillers are not a long-term solution for chronic non-cancer pain: their benefit fades over time and they carry risks of tolerance, dependence and increased nervous system sensitivity to pain, so they are used with great caution and monitoring and are never a measure of how good the treatment is. Injections and nerve blocks provide \*\*temporary relief that opens a window for rehabilitation\*\*; they do not cure the cause and do not replace exercise. Complete rest and prolonged lying down worsen joint stiffness and muscle weakness, and therefore increase pain in the longer run.

It is equally important to know that MRI scans frequently show degenerative changes that are also present in people with no pain at all, so a scan does not automatically explain your symptoms; decisions rest on the fit between imaging and clinical examination, never on imaging alone. On the other hand, some features must not be postponed: progressive weakness, loss of bladder or bowel control, fever, unexplained weight loss, or relentless night pain that wakes you. Suitability for any medicine or procedure is decided by clinical examination, not by reading, and you should never change your own medication regimen.

## Preparation

Bring a written list of everything you take: painkillers of all kinds, nerve pain medicines, topical products, supplements and herbal remedies, with doses, how long you used them, and what helped and what did not. Such a list saves months of trial and error. Bring previous reports and imaging on a disc or app rather than phone photographs alone, plus any physiotherapy or procedure reports. Before the visit, keep a short one-week diary: when the pain rises and eases, how much you sleep, and which activities you have given up. Mention chronic illnesses, pregnancy or possible pregnancy and drug allergies, and be open about anxiety, low mood or previous opioid use - honesty here improves the plan rather than weakening your case.

## Aftercare

Keep doing your exercises on the good days and, in a lighter version, on the difficult ones too, because stopping is the commonest reason people lose the gains they built. Do not raise or stop any medicine on your own, especially nerve pain drugs and strong analgesics that need gradual adjustment, and avoid collecting painkillers from the same family from more than one doctor. Track your functional goals rather than the pain score alone, protect your sleep, limit late nights and excess caffeine, and stop smoking, which worsens chronic pain. Report side effects such as dizziness, drowsiness, constipation or poor concentration instead of enduring them silently.

Seek care immediately with \*\*progressive weakness in a leg or arm\*\*, \*\*loss of bladder or bowel control or numbness in the saddle area\*\*, \*\*fever with back or neck pain\*\*, unexplained weight loss or night pain that wakes you every night, chest pain or breathlessness, or a sudden change in speech or awareness.
