## Procedure steps

### 1. Clinical examination and red-flag screening

The therapist asks how the pain started, how it behaves by day and night and what aggravates it, then examines movement, leg muscle strength, reflexes, sensation and nerve-tension tests. The most important part of this step is screening out what needs a doctor and imaging first: pain after significant trauma, advancing neurological symptoms, or features suggesting infection, tumour or a fragility fracture.

### 2. Pain relief and activity modification, not rest

Activities that flare the pain are temporarily reduced while everyday movement continues, and heat, manual therapy, relieving positions or electrical stimulation may be used for symptom control. You are told plainly that days in bed delay recovery and weaken muscle, and that short, frequent walks are among the best ways to start.

### 3. Restoring mobility and flexibility

Graded mobility exercises for the lumbar spine and pelvis, stretching for the hamstrings, hip flexors and iliopsoas where they have shortened, and improved hip and thoracic movement, since stiffness there raises the load on the low back. The directions that ease your symptoms are identified and built upon, repeated daily in small doses.

### 4. Building endurance and strength

The most important phase: endurance work for the deep and extensor trunk muscles, strengthening for the glutes and thighs, then functional exercises that mimic lifting, bending and standing up. The dose is advanced weekly, judged by symptoms the following day, and this capacity does more to prevent recurrence than any device or belt.

### 5. Education, return to work and prevention

Training in lifting and sitting technique, workstation setup and movement breaks, plus addressing the fear of movement that prolongs chronic pain, followed by a weekly maintenance plan after discharge, because the benefit fades if exercise stops. No session count is promised: the plan is estimated initially and reassessed according to your response.

## About this service

Physical therapy for back pain is a non-surgical treatment combining prescribed therapeutic exercise, patient education, manual therapy and activity modification, built on a clinical examination that identifies the source of the symptoms and screens out what exercise cannot treat. It is used in acute and chronic low back pain, in pain referred into the leg, in upper back pain related to posture and desk work, and after selected spinal operations with the surgeon's agreement.

Treatment begins with an assessment that separates mechanical pain from conditions needing a doctor first, then a plan graded week by week: mobility work for the spine, endurance training for the trunk and pelvic muscles, strengthening for the glutes and thighs, stretching for what has shortened, and retraining of everyday movement patterns such as bending, lifting, prolonged sitting and driving. Important education is added about back pain itself: that it is common, that most episodes settle, and that graded movement helps more than over-protection.

Results are cumulative and are not created by the session: your adherence to home exercise is the single biggest factor in improvement and in preventing recurrence, and patients who continue the programme after the pain has gone are the least likely to return with a new episode. Two myths need correcting here. The first is that \*\*physiotherapy is just massage\*\* — in reality massage is a temporary comfort measure that neither builds capacity nor prevents recurrence, while the backbone of treatment is prescribed exercise with a dose, a resistance and a progression. The second is that \*\*pain proves benefit\*\* — in truth mild soreness that settles by the next day is acceptable, while sharp escalating pain signals an excessive dose that needs adjusting rather than enduring. Devices such as electrical stimulation, therapeutic ultrasound, laser and traction have a supporting role in settling a phase, and none of them replaces exercise.

The limits are explicit. \*\*Exercise does not push a herniated disc back into place and does not repair the degenerative changes visible on a scan\*\*; what improves is symptoms, function and tolerance. \*\*Bed rest delays recovery and weakens muscle\*\* and is advised only for a few hours in very severe pain. A back belt is a temporary aid and should not be worn long term. Duration is estimated initially in weeks and reassessed at every review, and suitability for each exercise is decided by clinical examination rather than by reading.

## Preparation

Tell the therapist when and how the pain started, whether it travels into the leg or foot, whether it wakes you at night, what makes it better or worse, and whether it followed a fall, an accident or heavy lifting. Declare previous spinal surgery, osteoporosis, cancer, inflammatory arthritis or long-term steroid use, your medications including blood thinners, and any unintentional weight loss or fever accompanying the pain. Bring X-rays or MRI scans if you have them, but do not rely on them alone; the clinical examination decides the plan. Wear comfortable clothing that allows movement and sports shoes, and eat something light beforehand. If you develop numbness around the saddle area, difficulty passing urine or progressive leg weakness, go to the emergency department rather than waiting for your physiotherapy appointment.

## Aftercare

Do your home programme daily at the prescribed dose, and continue it weekly after the pain has gone, because that is what prevents the next episode. Keep moving through your day: avoid sitting for more than 30 to 45 minutes at a stretch, take frequent short walks, split loads between both hands and keep objects close to your body when lifting, and review your chair and desk setup with your therapist. Expect mild muscle soreness after the first sessions that settles within a day or two; pain that increases and spreads further down the leg should be reported so the exercises can be adjusted rather than endured. Seek medical care immediately if you develop: \*\*numbness around the saddle or groin area, or difficulty or incontinence of urine or stool with leg weakness — an emergency needing urgent assessment\*\*, \*\*progressive neurological weakness or a dropping foot\*\*, \*\*persistent night pain that wakes you, with fever, weight loss or a history of cancer\*\*, \*\*back pain after a fall or major accident — imaging before continuing physiotherapy\*\*.
