## Procedure steps

### 1. A precise history

You are asked to point to the pain with your finger rather than describe it in general terms, and about its onset, whether it followed an injury, a change of shoes or an increase in activity, its timing through the day, the distance that provokes it, and any numbness, tingling, swelling or warmth. Diabetes, gout, rheumatoid disease and arterial disease are asked about, along with your job and standing hours, since this narrows the diagnosis more than any test.

### 2. Clinical examination of both feet

Both feet are examined standing, sitting and while walking, the painful area is palpated precisely, and joint movement, Achilles flexibility and heel alignment are measured, while callus, pressure areas and skin and nail changes are noted. Pulses and sensation are tested, and these two examinations are indispensable in anyone with diabetes or a smoking history.

### 3. Imaging and tests when indicated

Weight-bearing X-rays are ordered when a fracture, arthritis or deformity is suspected, and MRI or ultrasound may be added for tendon and soft tissue problems and for an early stress fracture. Blood tests are requested where gout or inflammatory arthritis is suspected, Doppler ultrasound if a clot is possible, and perfusion studies if arterial disease is a concern. Imaging is not ordered for every ache without a reason.

### 4. A cause-directed treatment plan

The plan is built on the diagnosis: activity modification, footwear correction, a supportive or offloading insole, specific daily exercises, physiotherapy, local cooling, and control of weight, glucose or uric acid where relevant. The expected timeline is explained openly, because most of these conditions need weeks to months of consistency rather than days, and that realistic expectation is itself part of the treatment.

### 5. Follow-up and escalating care

A review visit is arranged to judge the response and adjust the plan, and if pain has not improved despite adherence, the diagnosis itself is reconsidered before treatment is escalated. Where appropriate, additional options such as shockwave therapy or a selected injection are discussed together with their limits and risks, and surgery is reserved for resistant cases or for a clear structural cause.

## About this service

Treating foot pain is a diagnostic and therapeutic pathway that begins by identifying the cause rather than simply silencing the symptom, because foot pain is not a diagnosis; it is a symptom shared by dozens of conditions whose treatments differ completely. The commonest include plantar fasciitis and heel pain, forefoot and metatarsal pain, an interdigital nerve neuroma, arthritis of the foot joints and a stiff big toe, Achilles tendon problems, stress fractures, gout, rheumatoid arthritis, diabetic neuropathy, and nail and skin conditions.

Diagnosis rests on the history and examination more than on any machine: the precise site of pain, whether it is worst on first standing in the morning or after activity, its relationship to footwear and walking distance, the presence of swelling, warmth or numbness, and how you load the foot when you walk. Both limbs are examined together, and pulses, sensation, heel alignment and Achilles flexibility are all assessed. Imaging is requested when indicated, and it is important to know that a plain X-ray can be entirely normal in an early stress fracture and in most soft tissue problems.

Some findings must not be postponed. Inability to bear weight after a twist or injury calls for imaging to exclude a fracture, because a common ankle sprain can conceal a fracture that, if missed, becomes a chronic problem. A single hot, red, very painful swollen joint with fever needs urgent assessment to exclude septic arthritis or acute gout. A painful, warm, swollen calf on one side requires exclusion of deep vein thrombosis, particularly if there is any breathlessness.

Treatment follows the cause: activity modification and relative rather than absolute rest, footwear correction, a supportive or offloading insole, a graded stretching and strengthening programme, physiotherapy, weight reduction where appropriate, and control of chronic disease. Selected injections have a limited role, are not the answer to every pain and should not be repeated indefinitely, and surgery is the exception rather than the rule. The limit of drug treatment is plain: a painkiller relieves pain without repairing its cause, and taking it long term without a diagnosis hides a problem that is getting worse. In a person with diabetes, the absence of pain is false reassurance, because a deep ulcer can form in a foot that complains of nothing at all.

## Preparation

Before your visit, pin down exactly where the pain is, when it began, what makes it worse and what relieves it, and note the distance you can walk before it appears. Bring the shoes you genuinely use for work, sport and home, because their condition and wear pattern are useful information. Bring any previous X-rays, MRI scans or reports and a complete list of your medicines, and tell the doctor if you have diabetes, gout, rheumatoid arthritis or arterial disease, or if you take blood thinners or steroids. Mention any injury or sprain and any sudden increase in activity or body weight over recent months, along with any treatment you have already tried and whether it helped. If possible, avoid taking a strong painkiller in the hours before the examination, because the assessment depends on localising the pain precisely.

## Aftercare

Stick to the daily exercise programme, as it is the single largest factor in the outcome, and do not stop at the first sign of improvement. Modify your activity rather than stopping movement altogether, since complete rest weakens muscle and prolongs the problem. Wear the prescribed shoes and insole whenever you are walking, including indoors, build your distance up gradually, and apply ice to the painful area after activity if advised. \*\*Seek medical review promptly if you cannot bear weight after an injury, if a single joint becomes hot, red and painful with fever or shivering, if one calf becomes painful, warm and swollen, especially with breathlessness, if numbness or weakness is increasing, or if pain wakes you from sleep.\*\* \*\*If you have diabetes, any wound, blister, crack or colour change in the foot must be seen the same day, even if it is completely painless.\*\*
