يبدأ التقييم باستبعاد أسباب قابلة للتصحيح: فحص السمع إلزامي قبل الحكم على تأخّر الكلام، وفحص النظر، وتقييم فقر الدم ونقص فيتامين د وخلل الغدة الدرقية، ومراجعة تاريخ الحمل والولادة ونقص الأكسجين واليرقان والتشنّجات، والسؤال عن اللغات في البيت ووقت الشاشة. وقد يُطلَب تقييم لطيف التوحّد أو فحوص جينية أو استقلابية أو تصوير للدماغ إذا وجّهت العلامات إلى ذلك، وليست هذه فحوصاً روتينية لكل طفل.

## خطوات الإجراء

### 1. قياس المعالم وتوثيق الخط الأساسي

يُراجع تاريخ الطفل من الحمل والولادة حتى اليوم، وتُقاس مهاراته الحالية في الحركة واللغة والتواصل والمهارات الدقيقة والاعتمادية بأدوات قياس معتمدة، مع تسجيل الطول والوزن ومحيط الرأس. يُوثَّق ما يستطيعه الطفل الآن بدقّة ليكون مرجعاً تُقاس عليه أي تغيّرات لاحقة.

### 2. استبعاد الأسباب الطبية والسمعية والبصرية

يُطلَب فحص سمع حديث قبل أي حكم على تأخّر الكلام، وفحص للنظر، وتحاليل لفقر الدم والغدة الدرقية وفيتامين د بحسب الحالة. يُفحص الطفل عصبياً لتقييم قوّة العضلات والمنعكسات، ويُسأل عن التشنّجات والشخير وانقطاع النفس أثناء النوم لأنها تؤثّر في الانتباه والنمو.

### 3. التقييم المتخصّص متعدّد المجالات

يُحوَّل الطفل بحسب مجال التأخّر إلى أخصائي نطق ولغة، أو علاج وظيفي، أو علاج طبيعي، أو تقييم نفسي وتطوّري، وقد يُقيَّم لطيف التوحّد أو الإدراك عند الحاجة. الهدف تحديد نوع التأخّر ودرجته وسببه المحتمل، لا مجرد وصف جلسات عامة.

### 4. خطة تدخّل مبكر بأهداف وتدريب الأسرة

تُكتب خطة بأهداف وظيفية قابلة للقياس ومدّة زمنية، وتُحدَّد نوع الجلسات وعددها، ويُدرَّب الوالدان على تمارين يومية قصيرة داخل الروتين: اللعب المتبادل، تسمية الأشياء، انتظار دور الطفل، القراءة، وتقليل وقت الشاشة. البيت والحضانة شريكان في التنفيذ لا مجرد ناقلين للطفل.

### 5. إعادة القياس وتعديل الخطة

## Procedure steps

### 1. Measuring milestones and recording a baseline

The child's history from pregnancy and birth to the present is reviewed, and current skills in movement, language, communication, fine motor and self-care are measured with recognised screening tools, along with height, weight and head circumference. Exactly what the child can do now is documented as the baseline against which later change is judged.

### 2. Ruling out medical, hearing and vision causes

An up-to-date hearing test is requested before any conclusion about speech delay, together with a vision check and, where indicated, tests for anaemia, thyroid function and vitamin D. A neurological examination assesses muscle tone and reflexes, and the parents are asked about seizures, snoring and sleep apnoea, since these affect attention and development.

### 3. Multidisciplinary specialist evaluation

Depending on the affected domain the child is referred to a speech and language therapist, an occupational therapist, a physiotherapist or a developmental and psychological assessment, with autism-spectrum or cognitive evaluation added when needed. The aim is to define the type, degree and likely cause of the delay rather than simply prescribing generic sessions.

### 4. Early-intervention plan with family coaching

A written plan sets measurable functional goals with a timeframe, defines the type and number of therapy sessions, and trains the parents in short daily exercises embedded in routine: reciprocal play, naming objects, waiting for the child's turn, shared reading and reducing screen time. Home and nursery are partners in delivery, not just transport to sessions.

### 5. Re-measurement and adjusting the plan

The measurement tools are repeated at set intervals to compare progress with the child's own baseline rather than with peers, and goals are adjusted, sessions intensified, or a deeper evaluation arranged when progress is less than expected. School support and educational needs are also reviewed as the child grows.

نفّذ التمارين اليومية القصيرة بانتظام، فالتكرار في البيت هو ما يثبّت ما يُبنى في الجلسة، والانتظام أهمّ من طول الوقت. تكلّم مع طفلك وجهاً لوجه بجُمَل قصيرة واضحة، وسمِّ ما يراه، وانتظر ردّه دون أن تُكمل عنه، ولا تصحّح نطقه بتوبيخ بل بإعادة الكلمة صحيحة. قلّل وقت الشاشة قدر ما تستطيع واستبدله بلعب مشترك وقراءة يومية. سجّل ملاحظاتك بين الجلسات: مهارة جديدة، تراجع، نوم مضطرب، أو صعوبة أكل. لا تُوقف الجلسات عند أول تحسّن ولا تُقاطع الخطة لتجربة برنامج يَعِد نتائج سريعة قبل مناقشته مع فريقك. \*\*راجع فوراً عند فقدان مهارة كان الطفل يتقنها، أو غياب التواصل البصري والاستجابة للاسم، أو ارتخاء شديد أو تشنّج في العضلات، أو نوبات تشنّجية، أو عدم الجلوس بعد تسعة أشهر، أو عدم المشي بعد ثمانية عشر شهراً، أو غياب أي كلمة مفهومة بعد ثمانية عشر شهراً، أو ثبات محيط الرأس أو تسارع نموّه.\*\*

## About this service

Developmental delay means a child has not reached the skills expected for their age in one or more domains: gross motor skills such as sitting and walking, fine motor skills such as grasping and drawing, language comprehension and speech, social interaction, and self-care skills such as feeding and toilet training. Treatment is not a single medicine but an early-intervention programme that starts with a medical assessment looking for a treatable cause, then speech, occupational or physical therapy according to the delayed domain, together with parent coaching so that home becomes part of the therapy.

Delay is measured against established developmental milestones for the child's age, not against a sibling, cousins or neighbours' children; one family's pattern does not overturn a clinical standard. Early intervention genuinely changes the trajectory, because the brain is most adaptable in the first years, so "he will talk on his own" is not a treatment plan but a delay that wastes the best window. Watchful waiting is legitimate only as a documented medical decision with a date to re-measure, never as disappearing from follow-up.

Assessment begins by ruling out correctable causes. A hearing test is mandatory before any conclusion about speech delay, vision is checked, and anaemia, vitamin D deficiency and thyroid problems are considered, alongside a review of pregnancy and birth history, oxygen deprivation, jaundice and seizures, and questions about the languages spoken at home and screen time. Autism-spectrum evaluation, genetic or metabolic testing, or brain imaging may be requested when specific signs point that way, but these are not routine tests for every child.

The programme has clear limits. It does not cure a genetic condition or an earlier brain injury, it does not guarantee that a child will catch up with peers within a fixed period, and its results are not judged by one session or one month but by functional goals reviewed at intervals. Supplements, devices and programmes promising rapid results do not treat developmental delay, and screens do not teach language; heavy screen use is associated with greater language delay. What actually works is reciprocal play, directed talking, shared reading and daily repetition. The type and number of therapy sessions are decided by specialist assessment, not by reading.

## Preparation

Bring the vaccination record and pregnancy and birth reports, and before the visit write down the skills your child actually has and the age at which each appeared, because this matters more than a general description. Record short phone videos of your child playing, eating, moving and attempting to speak at home, since behaviour in the clinic may not be representative. Bring any previous hearing or vision report, laboratory results or earlier therapy assessments. Write down your questions and prepare a list of all medicines and supplements your child takes. Be honest about daily screen time, the languages used at home and who cares for the child most of the day. Choose an appointment when your child is rested and fed, bring a familiar toy and a snack, and do not hold back your own concerns or the observations of the nursery or teacher.

## Aftercare

Carry out the short daily exercises consistently, because repetition at home is what consolidates what the session builds, and regularity matters more than long sessions. Talk to your child face to face in short clear sentences, name what they see, wait for their response without finishing it for them, and correct pronunciation by repeating the word properly rather than by scolding. Cut screen time as far as you can and replace it with shared play and daily reading. Keep notes between sessions: a new skill, a regression, disturbed sleep or feeding difficulty. Do not stop therapy at the first improvement, and do not abandon the plan for a programme promising quick results before discussing it with your team. Seek care immediately if your child loses a skill they had mastered, stops making eye contact or responding to their name, develops marked floppiness or stiffness, has seizures, is not sitting by nine months, not walking by eighteen months, has no understandable word by eighteen months, or if head growth stalls or accelerates.
