Food poisoning treatment

## Procedure steps

### 1. Assessing the illness and degree of dehydration

Your physician asks about the suspect meal, when symptoms began and who else ate the same food, then checks pulse, temperature and blood pressure and assesses dry mouth, urine output and skin turgor. This assessment is what separates an illness that can be managed at home from one needing intravenous fluids or observation.

### 2. Oral rehydration

Oral rehydration solution is started in small amounts every few minutes, since large volumes provoke vomiting. Water alone is not enough because it does not replace the lost salts, and fizzy drinks and concentrated juices are avoided as they can worsen diarrhoea.

### 3. Intravenous fluids when required

With persistent vomiting that prevents drinking, moderate to severe dehydration, or dizziness on standing and reduced urine output, intravenous fluids with electrolyte replacement are given, and blood tests for kidney function and electrolytes may be requested. Response is judged by urine output, pulse and alertness returning to normal.

### 4. Deciding on tests and targeted treatment

In specific situations, such as bloody diarrhoea, high fever, symptoms lasting more than several days, recent travel, weakened immunity or pregnancy, a stool test or culture may be requested, and any targeted treatment is limited to what the assessment reveals. Anti-diarrhoeal medication is used with great caution and is avoided altogether when blood or fever is present.

### 5. Restoring nutrition and preventing recurrence

Once vomiting settles, food is reintroduced gradually in small, easily digested meals, temporarily avoiding fat and concentrated sweets. The likely source of contamination is discussed along with prevention rules: hand washing, separating raw from ready-to-eat food, cooking thoroughly, and refrigerating leftovers quickly rather than leaving them in the heat.

## About this service

Food poisoning is an acute gastrointestinal illness caused by eating or drinking something contaminated with bacteria, their toxins, viruses or parasites. Symptoms usually begin within hours to a day or two of the suspect meal and consist of nausea, vomiting, diarrhoea and abdominal cramps, sometimes with fever. The goal of treatment is not to stop the diarrhoea at any cost, but to protect the body from dehydration until the inflammation settles, and to recognise the minority of cases that need more than that.

For most people, treatment is careful fluid replacement. Fluids are taken in small, frequent amounts rather than one large glass that provokes vomiting, and oral rehydration solution is preferred because it replaces water and salts together; plain water alone does not restore the sodium and potassium being lost. When vomiting makes drinking impossible, or when clear signs of dehydration appear, intravenous fluids are given in the clinic or emergency department, with blood tests as the situation requires.

Eating is part of the treatment, not an exception to it. The myth of complete fasting prolongs weakness without shortening the illness. Start light, easily digested food as soon as vomiting settles and your stomach tolerates it, and temporarily avoid fatty and fried food, caffeine and concentrated sweets. A common mistake is reaching immediately for anti-diarrhoeal medication; these can be harmful precisely when fever or blood in the stool is present, because they slow the gut's clearance of the causative organism. Equally important, \*\*antibiotics are not a routine treatment for food poisoning\*\*: most episodes are viral or toxin-mediated and improve without them, unnecessary use prolongs carriage of the organism and drives resistance, and prescribing is limited to specific situations decided by a physician after assessment and possibly a stool test.

The limits of this treatment are clear: it supports the body rather than neutralising anything, and there is no medicine that cancels the toxin you swallowed. What matters most, therefore, is recognising the episode that has stopped being a simple gastroenteritis, and that is judged by clinical examination, the degree of dehydration, the patient's age group and any chronic illness, not by personal guesswork. Prevention remains the real long-term treatment: hand washing, keeping raw meat away from ready-to-eat food, thorough cooking, rapid refrigeration of leftovers, and caution with food left out for hours in the heat.

## Preparation

If you are heading to a clinic or emergency department, bring information that speeds up assessment: what you ate and drank over the past two days, when symptoms started, how many times you have vomited or passed diarrhoea, whether there is blood or mucus, and when you last passed urine. Tell your physician if anyone who shared the meal developed the same symptoms, and whether you have travelled recently. List your chronic conditions and all your medicines, especially diabetes, blood-pressure, diuretic, kidney and immune-suppressing treatments, since some need temporary adjustment during a dehydrating illness. Do not take leftover antibiotics from a previous prescription or an anti-diarrhoeal before assessment, and keep a stool sample if one has been requested.

## Aftercare

Keep drinking small, frequent amounts of fluid even after the diarrhoea stops, and use the colour and volume of your urine as a practical guide to whether you are drinking enough. Reintroduce food gradually, and avoid full-fat milk, fat and fried food for a few days if you notice they bring symptoms back. Do not prepare food for others until enough time has passed since your last episode of diarrhoea, and keep washing your hands, because the infection can spread to household members. \*\*Seek care immediately if you have blood in the stool or vomit blood, persistent high fever, vomiting that prevents you from drinking, no urine output or severe dizziness on standing, severe or worsening abdominal pain or pain localised to the lower abdomen, confusion, or symptoms lasting more than several days without improvement.\*\* Ask for urgent assessment without waiting for young children, pregnant women, older adults and anyone with weakened immunity or chronic illness, and also for any blurred vision, muscle weakness or difficulty swallowing after a suspect meal.
